    Accuracy of transrectal ultrasound guided prostate biopsy: Histopathological correlation to matched prostatectomy specimens

    Background: The Gleason grading system is currently the world's most commonly used histological system for prostate cancer. It provides significant information about the prognosis. Therefore, Gleason score is accepted as an important factor in therapeutic decision-making for prostate cancer. This retrospective study assessed the correlation of transrectal ultrasound (TRUS) guided biopsy and radical prostatectomy specimens in terms of Gleason scores. Methods: We reviewed the records of 103 patients who underwent radical prostatectomy due to clinically localized prostate cancer. The Gleason scores of the TRUS biopsies were compared with the respective Gleason scores of surgical specimen. Results: In 28.7% of cases, the TRUS biopsy score was the same as that of the radical prostatectomy specimen. The most significant discordance was the upgrading of well-differentiated tumors after surgery in 71.7% of cases. However, in 81.8% of cases with high Gleason score on TRUS, biopsy was correlated with poorly differentiated tumor after surgery. Conclusions: Well-differentiated tumors on TRUS biopsy did not correlate with the grades of final pathology in the majority of cases; however, a high Gleason score on TRUS biopsy usually indicated a poorly differentiated tumor on prostatectomy specimen. Therefore, the treatment algorithms for particularly well-differentiated tumors should not be deduced from biopsy histology alone.

    The Efficacy of Early Mobilization after Urologic Radical Surgery

    Contemporary evidence based trials suggest that same changes in the traditional approach for the care of patients provide a rapid recovery and return to daily life after surgery. Consequently, a decrease in the surgical complications and shorter hospital stay would be provided. These new multi-perspective approaches which are different from the classical approach are defined as fast track surgery. Early mobilization besides early extubation and rapid discharge from extensive care unit constitute the major components of fast track surgery in order to bring a shorter hospital stay. Nurses together with surgeons and anesthesiologists are all active members of the care team which may also include the patient by him or herself. The aim of this review was to discuss the efficacy of early mobilization after urologic surgeries.

    Contemporary Trends in Adjuvant and Neoadjuvant Treatment for Renal Cell Carcinoma

    Renal cell carcinoma is an increasingly significant cancer in which surgical resection is still the sole curative approach. There is a risk of recurrence in one-third of patients after surgery. Successful experiences with some solid organ cancers and effective treatment response to targeted agents in metastatic cases have suggested a similar adjuvant approach for renal cell carcinoma. Consequently, placebo-controlled adjuvant trials have been reported and the Food and Drug Administration approved sunutinib as an adjuvant treatment after nephrectomy in high-risk patients, with the risk of treatment-related side effects. Several clinical series have indicated that neoadjuvant application can provide significant downsizing of the cancer mass in complex cases and enable radical surgery. Similarly, neoadjuvant therapy could enable nephron-sparing surgery for certain patients. Both adjuvant and neoadjuvant approaches for renal cell carcinoma require further trials with larger patient numbers. This review presents contemporary experience on adjuvant and neoadjuvant treatment for renal cell carcinoma.

    The Effects of Hormonal Therapy on Quality of Life in Prostate Cancer

    The incidence of prostate cancer is dramatically increasing. Androgen blockade is widely utilized in the management of metastatic and locally advanced prostate cancer. Despite the benefits of this treatment, side effects and shortcoming of quality of life (QoL) constitute the major drawbacks of the hormonal treatment. The proper approach includes precautions in order to provide a lesser impairment in QoL. Therefore, physicians should consider the complications of hormonal treatment and should inform their patients. This review was prepared to point out the QoL changes regarding the hormonal treatment.

    The efficacy of neoadjuvant chemotherapy in invasive bladder cancer

    Radical cystectomy is the gold standard in the treatment of invasive bladder cancer. However, five-year disease-free survival is low most probably due to micrometastatic disease at the time of surgery. The neoadjuvant chemotherapy may be performed as the first line management for invasive bladder tumors in order to treat micrometastases found at the diagnosis and improve resectability of larger neoplasms. A total of 43 patients diagnosed with invasive bladder tumors and 11 patients received neoadjuvant chemotherapy. The mean age of patients was 64 (43-74) years, and mean follow-up period was 52 months (12-114). Neoadjuvant chemotherapy protocol consisted of methotrexate, vinblastine, doxorubicin, and cisplatin (MVAC) or cisplatin, methotrexate, and cisplatin (CMV). All patients in neoadjuvant chemotherapy group underwent radical cystectomy. There was no significant difference between the groups with respect to disease-free survival time and overall survival time. In patients who received neoadjuvant chemotherapy, the respective disease-free and overall survival times were 31 months and 36 months versus 30 months and 35 months in patients who were treated with surgery only (p > 0.05). Five-year survival rates were 36% and 31% in the chemotherapy and no-chemotherapy groups, respectively. In the present study, 5-year survival rate was not affected by neoadjuvant chemotherapy in invasive bladder tumor. Complete pathological remission (stage p0) was found in 28% and pathological downstaging (stage < T2) was seen in 9% of patients in the neoadjuvant chemotherapy group. Five-year survival rates were 75% and 14.2% in patients who responded to chemotherapy, and in patients with no response, respectively (p < 0.05). The most favorable prognostic factor in this study was the response to neoadjuvant chemotherapy revealed as complete remission or pathological downstaging. The most important issue remains the prediction of patients who would respond and benefit from neoadjuvant chemotherapy.

    The role of reactive oxygen species and apoptosis in the pathogenesis of varicocele in a rat model and efficiency of vitamin E treatment

    We investigated role of reactive oxygen species (ROS) and apoptosis in the pathogenesis of infertility in experimental model of varicocele. The protective effect of vitamin E was also examined. Three groups of rats were constructed as the first group had sham operation, experimental varicoceles were established by partial ligation of the left renal vein in later two groups. Third group had received vitamin E. Production of ROS was determined by chemiluminescence assay (CL). The in situ end labelling technique was utilized to investigate apoptosis. Tissue vitamin E levels were measured by high performance liquid chromatography. The differences between luminol enhanced CL levels of groups were not statistically significant. However, the difference between CL levels of lucigenin probe in left testicles of sham and varicocele groups were statistically significant (p = 0.0007). Similarly, the results of the third group receiving vitamin E significantly differed from the varicocele group (p = 0.0025). The difference of apoptotic index was also statistically significant between sham and varicocele groups (p = 0.0038). Although the values of apoptotic index detected in the vitamin E group were lower compared with the varicocele group, the difference was not significant. This study proposes that ROS production and apoptosis in the testicles were induced with experimental varicocele. Vitamin E had a protective role. An increased rate of apoptosis with experimental varicocele suggests a molecular alteration, which may involve ROS overproduction as the triggering mechanism. Consequently, this study indicates an association between varicocele and infertility at molecular level through stimulation of ROS and apoptosis.

    Management of Neonatal Priapism: Report of Two Cases and Review of the Literature

    Priapism in a newborn is a rare entity with only 15 cases reported in the literature since 1879. The most commonly detected etiologic factor is polycythemia, but most of the cases are idiopathic. Conservative treatment seems to be sufficient and surgical approach is usually unnecessary. Here, we present a prolonged erection in two newborns which occurred at the first day of life and detumescence was achieved with conservative approach at the 6th and 7th day of life, respectively. We also reviewed the literature to investigate the most adequate evaluation and management criteria.

    Prostatic Carcinosarcoma: A Case Report

    Prostatic carcinosarcoma is a rare malignancy composed of sarcomatous and carcinomatous elements. Predominantly, they are very aggressive tumors with a poor prognosis. In this case report, a 78 year-old male patient with prostate adenocarcinoma developing prostatic carcinosarcoma after using androgen blockade for 3 years was presented. We tried to define the clinical and pathological features of this rare malignancy with current information.
