    Effect of the use of remineralization agents before resin infiltration on the treatment of initial enamel lesions: OCT, ultrasound, and SEM evaluation

    Aim: It was aimed to investigate the effect of using remineralizing agent before resin infiltration application on the treatment of initial enamel lesions with OCT (Optical Coherence Tomography), Ultrasound and SEM (Scanning Electromicroscope). Materials and Methods: 80 enamel samples were kept in the demineralization solution (pH 4.5, 37°C) for 96 hours. Demineralized enamel samples will be randomly divided into 8 groups (n=10) including a remineralizing agent(Toothmousse/GC, Remin Pro/VOCO, Medical Mineral Gel/ROCS), resin infiltration (ICON/DMG), and a combined treatment of both. All remineralization agents were applied in remineralization cycle 2 times a day for 1 week.Measurements made from the enamel samples at the baseline, after demineralization and remineralization procedures evaluated with the OCT (Topcon3D OCT-1 Maestro) and ultrasonic system (Novascope 4500:NDT) lesion depths were measured. The sample surfaces were examined under SEM/EDS (Zeiss EVO-MA10) at x1000, 2500, 5000 and 10000. Data were analyzed in R program with WRS2. Two-way Robust Anova, Benferroni Test, and ICC (Intra-Class Correlation Coefficient) was used to examine the agreement between measurements. Significance level was set at p<0.05. Results: There was no statistical difference between the groups in OCT (p=0.884) and Ultrasound (p=0.051) median values after the protocols. In both measurement methods, the effect of remineralization treatments was statistically significant for lession depth (p<0.01). OCT median values were 104.01 in baseline, 144.78 in post-demineralization, and 90.5 in post-remineralization. Ultrasound median values were 1.34 in baseline, 1.1 in postdemineralization, and 2.36 inpost-remineralization. The highest ultrasound median value was obtained in the Remin Pro+ICON after remineralization (2.67), while the lowest value was obtained after demineralization (1.04). As a result of EDS analysis, the lowest Ca ratio (%atomic) was observed on the grup 5-ICON applied enamel surface (9.35). Conclusion: Although there is no significant difference between the groups, all groups were found efficient for remineralization treatments according to OCT, Ultrasound, and SEM/EDS analysis.

    Beyazlatma özellikli bir diş macununun in vitro olarak değerlendirilmesi

    Amaç: Beyazlatıcı içerikli deneysel diş macununun, minedeki renk değişimine ve yüzey pürüzlülüğüne olan etkisini değerlendirmektir. Gereç ve Yöntemler: Çalışmada 30-adet sağlam, çürüksüz, boyutları birbirine yakın daimi dişler kullanıldı. Dişler rastgele olarak 6 deneysel gruba ayrıldı (n=5); Grup 1: Beyazlatıcı ajan içeren deneysel diş macunu (Zubio Test-1, Dar Kozmetik, İstanbul, Türkiye); Grup 2: Beyazlatıcı ajan içeren deneysel diş macunu (Zubio Test-2); Grup 3: Beyazlatıcı ajan içeren diş macunu (İpana 3D White Luxe, İpana P&G, ABD); Grup 4: Bitkisel içerikli bir diş macunu (Gumgumix, Beka ilaç, İstanbul, Türkiye); Grup 5: Kahvede bekletilen; Grup 6: Suda bekletilen. Tüm örneklerde 3 farklı zaman aralığında (T1-T2-T3) renk ve yüzey pürüzlülüğü ölçümleri yapıldı. T1: Tüm örnekler 24-saat kahvede bekletildikten ve renklenmeleri sağlandıktan sonra; T2: Her gün 15 dakika (3 kez-5’er dakika) kahvede bekleme ve 10 saniye (2 kez-10’ar saniye) fırçalama işlemi yaptıktan 7 gün sonra; T3: Her gün 15 dakika (3 kez-5’er dakika) kahvede bekleme ve 10 saniye (2 kez-10’ar saniye) fırçalama işlemi yaptıktan 14 gün sonrayı göstermektedir. Veriler Kruskal-Wallis ve Friedman istatistik testleri ile değerlendirildi (P=0.05). Bulgular: Tüm test gruplarında renk değişimi görüldü ancak test gruplarının aralarında farklılıklar oluşmadı (P>0.05). Grup 2, sayısal olarak olumlu yönde en büyük değişimi oluştururken, diğer gruplarla arasında istatistiksel bir farklılık ortaya çıkarmadı. Zubio Test-2 diş macununun beyazlatıcı etkisi İpana 3D White luxe ve Gumgumix diş macunları ile benzer bulundu. 14 günlük araştırmanın sonucunda Zubio Test-2, Test-1’e göre daha yüksek yüzey pürüzlülük değerleri sergiledi. Sonuç: Zubio Test-2’nin, kahvenin neden olacağı renklenmeye karşı geleneksel beyazlatıcı diş macununa benzer koruyucu etki göstererek, renklenmeyi önleyici yönde etkili olabileceği söylenebilir.

    Microleakage Evaluation of Photopolymerization Efficiency in Different Layering Methods of New Generation Led Light Devices

    Background/Aim: The aim of this study was to examine the microleakage values after restoring the prepared Class II box cavities using two different composite resins with three different layering methods and polymerizing them with five different LED light devices. Material and Methods: Class II box cavities were prepared in 150 extracted mandibular molars. Nanoceramic composite resin (Ceram.x SphereTEC one universal A2, Dentsply, Germany) and bulk fill composite resin (SDR flow+ A2, Dentsply, Germany) were used for the restoration of the cavities. Teeth were restored with three different layering methods (bulk fill, horizontal layering, and centripetal buildup technique) and five different LED light devices [(Smartlite Focus, Dentsply, USA), (Led.E, Woodpecker, China), (Valo Cordless, Ultradent, USA), (Bluephase N, Ivoclar Vivadent, Liechtenstein), (D-Light Pro (GC, USA)] and then subjected to microleakage analysis. Kruskal Wallis and Mann Whitney U test were used for statistical analysis. Th e data was evaluated under p<0.005 significant level. Results: Using different layering methods did not affect microleakage scores statistically (p=0.7683). Applying bulk-fill composite resin with 2 mm horizontal layers or using the centripetal buildup technique did not show significant differences in microleakage. Second- and third-generation light devices demonstrated no statistical difference in microleakage (p=0.9075). Conclusions: Using different layering methods and different curing units did not make any difference in microleakage.     Remineralization efficiency of herbal products

    Dental caries is one of the most common chronic diseases in the world and might cause tooth loss when not treated properly. The concept of modern dentistry aims to prevent the progression of caries with early diagnosis, and to improve the aesthetics and function of the tooth with the remineralization process. Thus, caries lesions without cavity are treated with a noninvasive method (Nagarathana et al., 2015). Demineralization occurs through the leaving of mineral ions in dental hard tissues from hydroxyapatite crystals. Restoring these mineral ions to hydroxyapatite crystals is called remineralization. Both processes take place on the tooth surface and a large number of mineral ions can be lost from hydroxyapatite without deterioration of tooth integrity. As a result of the deterioration of the hydroxyapatite structure, cavities are formed. Demineralization is a reversible process; therefore, partially demineralized hydroxyapatite crystals in teeth can be remineralized (Higham, 2016). Enamel caries diagnosed at the initial stage can be treated by regulating diet, controlling plaque formation, and applying appropriate antibacterial agents, thus, demineralized dental tissue can become remineralized (Guerrieri et al., 2012).

    The Effect of Dental Paste with Herbal Content on Remineralization and the Imaging with Fluorescent Technique in Teeth with White Spot Lesion

    Objective: To evaluate the effect of Gumgumix herbal toothpaste on remineralization of white spot lesions with a laser fluorescent system called FluoreCam. It is a pilot study. Methods: The FluoreCam system was used for the fluorescence imaging of the fifteen teeth with white spot lesions. During the study, the patients continued to use the previously owned toothpastes, addition to Gumgumix. Gumgumix was applied on the lesion area with a finger pressure twice a day and was not rinsed mouth after, only a limited spit and was prevented from taking any fluid during 30 minutes. The Compare option in the FluoreCam system was used to differentiate the situation before and after the use of the paste; if the marked area were yellow No Change; blue Sound Surface; green Improving; light blue Mild Improving and red meant Worsening. Results: Of the 13 lesions that were Suspect Surface initially, 46% were identified as Mild Improving, 7% as Moderate Worsening, 30% as No Change, and 15% as Sound Surface. Two of the white spot lesions were recovered totally as 100%. Conclusion: After use of Gumgumix for one week, remineralization was observed on teeth with white spot lesion. The white spot lesions on two teeth were totally healed.
