    Effectiveness of some herbals on initial enamel caries lesion

    Objective: To evaluate the effectiveness of herbal medicaments such as ginger, rosemary and honey on remineralization of initial enamel lesion. Methods: Demineralized human enamel specimens were measured for baseline surface microhardness and fluorescence methods. Ten specimens in each of four groups were used in this in vitro recycling study with the following treatments which applied three times a day: 1) sodium fluoride toothpaste (Ipana, Procter & Gamble, Turkey), 2) ginger-honey (Arifoglu Herbals, Anzer Honey, Turkey), 3) ginger-honey-chocolate (Bind Chocolate, Turkey), 4) rosemary oil (Arifoglu Herbals, Turkey). Treatment regimens of demineralization and remineralization cycle were applied for 21 days. The post-treatment data were obtained by measurements of surface microhardness and fluorescence methods. Data were statistically analyzed by ANOVA test with Tukey's honest significant difference test. Results: Enhanced remineralization was observed with several of the treatment systems including ginger + honey and rosemary. Significant differences between treatments were observed by microhardness and FluoreCam fluorescence assesment, compared to the positive control group (NaF dentifrice). Significantly, greater remineralization was observed with the honey + ginger treatment regimen. No significant differences between groups were observed using the fluorescence assessment method, quantitative light-induced fluorescence. Conclusions: Herbals (ginger, honey and rosemary) have enhanced remineralization of initial enamel lesion.

    Digitally Guided Composite Restorations After Orthodontic Treatment: A Case Report

    Introduction: Orthodontic treatment can improve dental aesthetics and function, but it can also lead to dental problems, such as interproximal caries and tooth fractures. This case report presented the dental treatment of a 25-year-old female patient who had previously undergone a fixed orthodontic treatment for five years and resorted to Marmara University Department of Restorative Dentistry due to aesthetic concerns. The patient complained about interproximal caries and a fracture of the right maxillary lateral incisor, which negatively impacted her dental aesthetics and function. After a thorough clinical and radiographic examination, a treatment plan which included composite restorations for the interproximal caries and the fractured lateral incisor was created. Case Report: In the first session office bleaching was performed with 32% hydrogen peroxide gel (Fläsh; WHITEsmile, 3×15 minutes). Before starting the restorative treatment, palatal wall was designed using a software (Meshmixer, Autodesk). The software allowed for a virtual simulation of the desired palatal walls, which was then final guide model produced using a DLP 3D printer (Photon Ultra, Anycubic). Two weeks after bleaching, composite resin restorations were completed with body shade composite (Estelite Asteria A1B, Tokuyama Dental) under rubber-dam isolation. Universal adhesive (Bond Force II, Tokuyama Dental) was preferred and polymerization was completed with 1000 mW/cm2 lightcuring unit (Valo Grand, Ultradent). Finishing and polishing procedures was carried out using composite polisher (Lamello, Horico Dental). Follow-up sessions were conducted at 1 week, 1 month, and 6 months and evaluated for World Dental Federation (FDI) 2-steps criteria. Results: The clinical performance of the treated teeth evaluated for aesthetic properties, surface texture/staining, colour stability/translucency and anatomic form according to FDI criteria and scored as 1. Conclusion: The successful outcome of this case demonstrates the effectiveness of composite resin restorations for addressing functional, aesthetic, and satisfactory concerns. The use of modern technology, including digital design and 3D printing, allowed for improved accuracy and customization, resulting in a natural-looking and durable restoration.     Assessment of micro-hardness, degree of conversion, and flexural strength for single-shade universal resin composites

    © 2022 by the authors.Single-shade universal resin composites (SsURC) are preferred in clinical practice to reduce time for shade selection and obtain good esthetic results. In this study, the static mechanical properties of seven new SsURCs were investigated, their spectral analyzes were performed and scanning electron microscopy (SEM) evaluations were presented. Charisma Diamond One/DO, Admira Fusion x-tra/AFX, Omnichroma/OC, OptiShade/OS, Essentia Universal/EU, Zenchroma/ZC, Vittra APS Unique/VU were used in a three-point bending test to determine flexural strength (FS) and elastic modulus (EM); Vickers micro-hardness (VHN) and hardness-ratio (HR) were performed with a micro-hardness tester from top/bottom after 24-h/15-days of storage in distilled water at 37 °C (±1 °C). The degree of conversion (DC) was assessed by using Fourier transform infrared spectroscopy (FTIR). The structure of the resin matrix and filler content were assessed by SEM. Data were analyzed using IBM SPSS V23 and the R program and the significance level was taken as p < 0.05. The main effect of the tested SsURCs was found to be statistically significant on FS, EM, VHN, and DC values (p < 0.001). Bis-GMA free SsURCs (AFX, DO, VU) showed better DC and HR except for OC. All seven tested SsURCs conform to the requirements of ISO standards for dental resin composites for all tested categories.

Alınan anamnezde 12 sene önce sabit ortodontik tedavisinin bittiği öğrenildi. Ayrıca hasta büyükazı ve santral dişleri sürdüğünden beri diş yüzeylerinde beyaz lekeleri olduğunu belirtti. Klinik muayenede özellikle maksiller sol santal ve lateral dişlerinde olmak üzere beş anterior dişinde mine defektleri ve hipomineralize alanlar tespit edildi. Hasta tercihen olabildiğince minimal invaziv tedavi istediğini bildirdi.Rezin infiltrasyon tedavisinden iki hafta önce %32’lik hidrojen peroksit jel (Fläsh; WHITEsmile, Germany, 3×15 dakika) ile ofis tipi beyazlatma tedavisi uygulandı. Dişler izole edilirken palatinal bölgedeki retainer nedeniyle split dam izolasyonu tercih edildi. Ardından %15’lik hidroklorik asit (Icon Etch, DMG, Germany) opak alanlara iki dakika boyunca uygulandı. Asitli yüzey 30 saniye boyunca su ile yıkandı ardından susuz hava ile kurutuldu. Sonrasında demineralize lezyondaki mikropörözitelerde bulunan suyu uzaklaştırarak lezyonu gözle seçilemeyen hale getiren etanol (Icon Etch, DMG, Germany) 30 saniye süre ile uygulandı. Bu işlemler 4 kez uygulandıktan sonra kalan opak dokular son asitlemeden önce ince grenli bir frez (837KR, Frank Dental, Germany) yardımıyla uzaklaştırıldı. Ardından rezin infiltrant (Icon Infiltrant, DMG, Germany) ilk uygulamada üç dakika, ikinci uygulamada bir dakika uygulanarak her seferinde 40 saniye boyunca ışık cihazı (Valo Grand, Ultradent, USA) ile polimerize edildi. Uzaklaştırılan mine dokusu mine rengi bir kompozit (Estelite Asteria NE, Tokuyama Dental, Japan) ile restore edildi. Hasta 1 hafta ve 1 aylık randevular ile kontrole çağrıldı.Bulgular: Hastanın tedaviyle ilgili bir şikayeti ve sorunu olmamıştır. FDI’nın estetik kriterleri olan yüzey parlaklığı/renklenme, renk uyumu/translusensi ve anatomik forma göre değerlendirilen dişlerin skoru 1 olarak belirlendi.Sonuç: Molar insizal hipomineralizasyonunu (MIH) estetik olarak idame ettirebilmek için rezin infiltrasyon yöntemi tercih edilebilir. Ancak bu işlem bazı limistasyonlara sahiptir. Beyazlatma ve kompozit tabakalama işlemleri ile bu limitasyonlar minimal invaziv olarak aşılabilir.
