    Evaluation of a Standardized Bakery Product (SUTMEK) as a Potential Tool for Baked-Milk Tolerance and Immunotherapy Research Studies

    Background and Objectives: About 65-80% of children with IgE-mediated cow's milk allergy (CMA) can tolerate extensively heated milk. We have invested in the mass fabrication of a test product containing milk protein baked at 180 degrees C for 30 min (SUTMEK-milk) and a milk-free placebo (SUTMEK-placebo) to carry out a standardised double-blind placebo-controlled food challenge (DBPCFC) test in patients with CMA. Methods: We studied children with IgE-mediated CMA between 13 and 48 months of age. Specific IgEs (spIgE) to milk proteins were quantified. A DBPCFC with our bakery products was performed, and factors determining reactivity to extensively heated milk were evaluated. We also tested the applicability of SUTMEK products in baked-milk oral immunotherapy in a pilot assessment. Results: We studied 15 children (8 girls, 7 boys) with a median age of 26 months (range: 13-48 months). Nine (60%) patients tolerated a challenge with extensively heated milk, while 6 (40%) were found reactive (anaphylaxis: 2, wheezing: 2, urticaria: 2). spIgE to milk, alpha-lactalbumin, and casein, and the wheal diameter on skin prick testing were higher in the reactive group than the tolerant groups (p = 0.001, p = 0.001, p = 0.002, and p = 0.048, respectively). Receiver-operating characteristic curve analyses yielded the following cut-off values for spIgEs that would predict a reactivity to extensively heated milk; milk: 25 kU/L (area under curve, AUC: 0.981), casein: 32 kU/L (AUC: 0.983), and alpha-lactalbumin: 17 kU/L (AUC: 0.981). Nine patients have tolerated well a continued daily consumption of SUTMEK-milk or -placebo for 6 months at the desired doses. Conclusions: Our bakery products were successfully used in DBPCFC studies and qualified as an acceptable tool for use in the research of interventional tolerance induction. Although spIgE appears useful in determining children at high risk of reacting to extensively heated milk, the predictive cut-off values are still far from being perfect. (c) 2018 S. Karger AG, Basel

    Nutrients in schizophrenia: A focus on the pathophysiological pathway

    Objectives The aim of this article is to explain the nutrients that play an active role in the pathophysiology of schizophrenia. Methods This paper is a narrative literature review of relevant articles and prior works that have been central to the topic including the active nutrients in the pathophysiology of schizophrenia. Results The findings are compiled under six headings. The changes in the antioxidant defense system, dopamine pathway, serotonin pathway, gamma-aminobutyric acid (GABA) pathway, glutamate pathway, the endocannabinoid system, and metabolomic profile were investigated in relation to nutrients. Conclusions This review provides an update of scientific knowledge on the growing role of nutrition in schizophrenia. Nutrient deficiencies that occur frequently in these patients should be followed and eliminated to ensure the correct functioning of the pathophysiological pathways of the disease. © 2021 Pacini Editore S.p.A..     Atletlerde Yeme Bozuklukları

    Dünyada en yaygın görülen yeme bozuklukları anoreksiya nervoza ve bulimia nervoza olup;genel nüfusun yaklaşık %1'i anoreksiya nervoza, %4’ü ise bulimia nervoza belirtileri sergilemektedir.Anoreksiya nervoza çok düşük vücut ağırlığı, vücut ağırlığına ilişkin çarpık bir algı ve kilo almayadair yoğun korku ile karakterize bir yeme bozukluğudur. Bulimia nervozada ise kişiler gizliceçok büyük porsiyonlarda besin tüketerek, ardından duydukları pişmanlıkla sağlıksız bir şekilde bubesinleri geri çıkarmaya çalışmaktadırlar. Başka türlü adlandırılamayan yeme bozukluklarındanbiri olarak kabul edilen tıkınırcasına yeme bozukluğu ise kişinin yeme kontrolünü kaybettiği tekrareden tıkınırcasına yeme atakları ile karakterizedir. Yeme bozuklukları ve düzensiz beslenmeatletizm branşı sporcularında oldukça sık görülen durumlardandır. Atletlerde yeme bozukluğu görülmesıklığı %1-62 arasında değişmekte olup, kadınlarda bu sıklığın daha yüksek olduğunu gösterençalışmalar mevcuttur. Sporcunun beslenme ve kilo kontrolü davranışlarının geçici olupolmadığını belirlemek için erken teşhis ve tedavi atletik programlar için öncelikli olmalıdır.Yeme bozukluğunun erken dönemde saptanması, hem sağlık sorunlarının ortaya çıkmasını hemde atletin performansının olumsuz yönde etkilenmesini engellemektedir. Bu amaçla; Kadın AtletTarama Aracı, Atletik Ortam Anketi ve Fizyolojik Tarama Testi gibi kadın atletler için geliştirilmişbazı tarama testleri bulunmaktadır. Atletlere yeme bozukluğu tanısı konulduktan sonra,bilgili ve deneyimli sağlık çalışanları ile multidisipliner ve hasta merkezli bir bakım programı oluşturulmasıönem arz etmektedir. Özellikle kadınlarda olmak üzere, atletlerde yeme bozukluklarınınciddi sağlık sorunlarına yol açması ve spor performansını olumsuz şekilde etkilemesi, atletlere özgüyeni tarama araçlarının geliştirilmesi ve uygulanmasına daha da önem verilmesi gerektiğini göstermektedir.

    Nutritional label use among patients with type 1 diabetes mellitus

    Background and Aims: In this study, it was aimed to evaluate the nutritional label use of individuals with type 1 DM. Material and Method: This cross-sectional descriptive study was conducted in November 2017 - June 2018. Diabetic individuals were reached through hospitals and social networks. The study was conducted with 105 (64 female, 41 male) individuals with type 1 DM living in Istanbul. A questionnaire about sociodemographic characteristics, biochemical parameters, medical treatment, food label reading status, food label reading reasons were applied in the study. Data were analyzed using SPSS V 16.0 program. Results: 85.7% of the participants read the food label. No significant difference was found when age group and nutritional label reading status were examined (p > 0.05). Besides, information was given to 67.6% of the participants about reading the nutritional label. A significant association was found between nutritional label reading knowledge and carbohydrate counting application and stages (P=0.0001). However, there was no significant difference between biochemical parameters and reading label status (p>0.05). Conclusions: Nutritional label reading should be mentioned more in the medical nutrition therapy and diabetes education. © 2021 The Authors.     İnek Sütü ve Prostat Kanseri İlişkisi

    Prostat kanseri, prostat bezinin herhangi bir yerinde başlayabilen, ilk 5-10 yılında yavaş, sonrahızla büyüyen ve diğer organlara yayılabilen tedavi edilmesi güç bir hastalıktır. Türkiye’de 2014 yılındaerkeklerde en sık görülen 10 kanser sıralamasında akciğer kanserini takiben ikinci sıraya yerleşenprostat kanseri, tüm yaş grupları sıralamasında da ikinci sırada yer almaktadır. Bu derlemede, prostatkanserli bireylerin inek sütü tüketiminin hastalığın seyrine olan etkisi ve inek sütünün kanser hücre gelişimiüzerine etkisini açıklamak amaçlanmıştır. Bunun yanında, prostat kanseri görülme sıklığı ve ölümoranı ile ne kadar ilişkili olabileceği sorgulanmıştır. Süt tüketimi ve prostat kanseri ile ilişkili olduğu düşünülenmuhtemel mekanizmalardan kalsiyum, insülin benzeri büyüme faktörü, DNA metiltransferaz 1sinyalini zayıflatan süt kaynaklı mikroRNA’lar ve yüksek inek sütü tüketimi ile artan mTORC1 sinyalizasyonuaçıklanmıştır. Güncel sonuçlar çocukluk dönemindeki süt tüketimini prostat kanseri riski ile ilişkiligörmeyip, erişkinlikteki süt tüketiminin biyolojik bir temeli olabileceğine işaret etmektedir. DünyaKanser Araştırma Fonu’nda süt ve süt ürünleri üzerine mevcut kanıtlar sınırlı olarak raporlanıp, süt vesüt ürünlerinin prostat kanseri risk artışına neden olduğuna vurgu yapılmıştır. İnek sütündeki mikroRNA’larıninsan besin zincirinden çıkarılması, prostat gelişimi ve farklılaşmasının mTORC1'e bağlı gerçekleşenfazları sırasında inek sütü alımının kısıtlanması ve mTORC1 sinyallerini zayıflatıcı yönde diyetmüdahaleleri prostat kanserinden korunma amaçlı girişimler olarak uygulanabilir.

    Energy and nutrient content of weight-loss diets published in high-circulation newspapers

    Introduction: The press is one of the most effective methods to provide the public with health education and to increase knowledge levels related to health in society. Nutritional advice and weight loss diets are also frequently featured in newspapers. However, diets in newspaper limit food groups and/or nutrients to achieve faster results. This study aimed to assess diet lists published in newspapers in terms of energy and nutrient contents and to compare levels with requirements. Material and Methods: From 1th February 2018 to 31th January 2019, nutritional and diet news included in 10 newspapers with high circulation in Turkey were investigated and diets published in these newspapers were investigated. The energy, macro, and micronutrient contents were compared with the Turkey Nutrition Guide recommendations. Results: It appeared diet lists met 55.5% of energy requirements for women and 44.5% of energy requirements for men. Carbohydrate and protein contents of diets were below recommendations, while fat contents were identified to be above-recommended levels. Dietary fiber was determined to meet 81.2% of recommendations. For micronutrients, calcium and iron contents were below recommended levels. Conclusions: It appears weight-loss diets in newspapers are insufficient in terms of energy and some nutrients. Though these diets, which are easily accessible by a large portion of society, without expert control they may cause nutrient deficiencies in the long term.

    The Relationship Between University Students' Nutritional Status, Cardio-Metabolic Biomarkers and Physical Activity Levels

    Objective: This study aimed to evaluate the relationship between nutritional status, cardiometabolic biomarkers and physical activity levels in university students. Methods: Firstly, fasting plasma total cholesterol, triglyceride, high-density lipoprotein cholesterol (HDL-C) levels were analyzed from participants' blood samples, and the homeostatic model assessment of insulin resistance (HOMA-IR) values were calculated after measuring fasting plasma glucose and insulin. Participants' weight, height, percentage of body fat, waist and hip circumference were measured and body mass index (BMI) was calculated. The International Physical Activity Questionnaire (IPAQ)and a questionnaire on socio-demographic characteristics were applied to participants. Energy and nutrient intakes were calculated from participants'24-hour dietary recall records. The statistical analysis of data was performed with SPSS version 21 software. For statistical significance, the total type-1 error level was determined as 5%. Results: Most of the participants' energy, dietary fibre, vitamins D and B1, folate, calcium and iron intakes were below recommended levels. HDL-C level was shown to be negatively correlated with total fat and saturated fatty acid intake (p<0.05). Omega-3 intakes of participants were negatively correlated with fasting plasma insulin and HOMA-IR (p<0.05).HDL-C was negatively correlated with BMI and waist-to-hip ratio, while fasting plasma insulin, triglyceride levels and HOMA-IR values were positively correlated with them (p<0.05). According to the IPAQ, it was found that 15.9% of participants had a low level of physical activity and 66.1% of them had a moderate level of physical activity. Conclusion: It is concluded that providing nutrition education and counselling services to students, improving campus facilities for physical activity are necessary for cardiometabolic health.

    Development and Validation of a Semi-Quantitative Food Frequency Questionnaire to Assess Dietary Intake of Turkish School-Aged Children

    The aim of this study was to develop and validate a food frequency questionnaire (FFQ) on the dietary intake of Turkish school-aged children. Fifty randomly selected students aged 7-12 from urban areas of Istanbul were included in this study. An FFQ, containing a list of 138 frequently consumed foods was developed. Dietary records (DRs) including three days, and FFQs were collected during autumn and spring. Daily consumption of each food group was assessed and the nutrient compositions of diet were calculated. The Pearson correlation coefficient, weighted kappa, the Bland-Altman scatter plots between averages of the reported (FFQ) and the references method (DR) were used as validity coefficient. The Pearson correlation, energy adjusted and attenuation coefficients of nutrient intake were calculated for energy (0.29; 0.11), protein (0.36-0.32; 0.21), carbohydrate (0.25-0.09; 0.15), fat (0.25-0.05; 0.13), and as for food groups, for meat group (0.29-0.21, 0.09), oil and margarine groups (0.21-0.02, 0.08), dairy group (0.52-0.58, 0.31), vegetable group (0.14-0.11, 0.09), fruit group (0.31-0.40, 0.09) and cereal group (0.33-0.34, 0.10), respectively. According to the kappa values, there is moderate and fair agreement between two methods. The Bland-Altman scatter plots showed an acceptable level of agreement between the two methods that DRs may be replaced with FFQ. The performance of the FFQ is likely to allow detection and sufficient assessment of nutritional status. The developed FFQ had validity coefficients similar to those of FFQs in previous studies.

    The relationship between social media use, eating attitude and body mass index among nutrition and dietetic female students: A cross-sectional study

    Introduction: This study aimed to investigate the relationship between media using habits, frequency and duration, and body mass index on eating attitudes. Material and Methods: The cross-sectional study was conducted with female students who received nutrition and dietetics education in Istanbul. A questionnaire consisting of a socio-demographic questionnaire, Eating Attitude Test (EAT-40), and Social Media Attitude Scale was applied to 1248 female volunteers by the face-to-face interview method. Results: It was found that 35.7% of the participants had abnormal eating attitudes and 89.6% showed a positive attitude towards social media. 93.7% of the group with abnormal eating attitudes indicated a positive attitude towards social media, 0.4% had a negative attitude and the difference was statistically significant (p=0.001). It was observed that a significant difference between EAT-40 and thought of an eating problem, having a social media membership, and spending time on social media (p<0.05). Conclusions: Social media have an effect on nutrition dietetics students on abnormal eating attitude; it did not show a significant effect on BMI.
