Publication: Normal işiten bireylerde kontralateral supresyon testinin uyaran türü ve süresi cinsinden standardizasyonun yapılması
Abstract
Amaç: Çalışmamızın amacı, klinikler tarafından efferent sistemin değerlendirilmesini sağlayacak, standart bir uygulama prosedürüne sahip, kontralateral TEOAE supresyon test yöntemi ve uyaran-gürültü parametrelerinin elde edilmesidir.Gereç ve yöntem: Çalışma, 2 aşamada ve 2 farklı katılımcı grubu ile yürütülmüştür. İlk aşamada, 18-32 yaş arası normal işiten 20’si kadın 9’u erkek olan 29 bireyçalışmaya dahil edilmiştir. ILO 292-II çift proplu otoakustik emisyon cihazı kullanılarak 60, 65, 70, 75 ve 80 dB peSPL uyaran şiddetleri ve 65 dB SPL geniş bantlı gürültü kullanılarak maksimum supresyonun elde edildiği değerler araştırılmıştır. Çalışmanın ikinci aşamasında ise, 19-34 yaş arası normal işiten 16’sı kadın 5’i erkek olan 21 birey çalışmaya dahil edilmiştir. Bu aşamada, çalışmanın ilk aşamasında elde edilen uyaran ve gürültü şiddetleri kullanılarak ideal kontralateral gürültü sunma şekli (süre veya trase cinsinden) ve süresi/ trase sayısı araştırılmıştır. Bulgular: Çalışmanın ilk aşamasında, istatistiksel anlamlılığa sahip olmasa da maksimum supresyon, 65 dB peSPL click uyaran ve 65 dB SPL geniş bant gürültü kullanılarak elde edilmiştir. İkinci aşamada, süre cinsinden kontralateral maskeleme yapılması ve sürenin de 30 saniye olarak ayarlanması, maksimum supresyona erişmeyi sağlamıştır.Sonuç: ILO 292-II otoakustik emisyon cihazının orijinal kontralateral supresyon yazılımında; lineer uyarım modu, 65 dB peSPL click uyaran ve 65 dB SPL geniş bant gürültü kullanımı önerilmektedir. Ayrıca, süre cinsinden maskeleme ve 30 saniye maskeleme süresi seçilmelidir.

In the second stage, the contralateral noise presentation method and its duration/ number of the sweep were examined with 21 participants. 29 young adults aged 18–32, 20 females and 9 males with normal hearing were included in the first study. Also, 21 young adults aged 19–34 years, 16 females and 5 males with normal hearing were involved in the second stage.Results: In the first stage, maximum suppression was achieved using 65 dB peSPL click stimulus and 65 dB SPL broadband noise. In the second stage, by performing contralateral masking in terms of “duration” and setting the time to 30 seconds, maximum suppression was obtained.Conclusions: For standardized use in clinics, the original contralateral suppression software of the ILO 292-II otoacoustic emission device recommends the use of 65 dB peSPL click stimulus and 65 dB SPL broadband noise for the linear stimulation mode. Moreover, masking in terms of “time” and 30 seconds of masking time should be selected.
