There is a need to implement non-stigmatizing interventions into existing health systems which reduce psychosocial distress during pregnancy and prevent perinatal depression. We adapted the WHO-endorsed Thinking Healthy Programme (THP) to be delivered universally to all women attending routine online pregnancy schools in Istanbul, Turkey. This study aimed to evaluate the feasibility and acceptability of this intervention. Methods: This mixed-methods study incorporated a two-arm pilot randomized controlled trial and qualitative evaluation of the feasibility and acceptability of the adapted THP – Brief Group version (THP-BGV) to a range of stakeholders. We recruited pregnant women at 12-30 weeks’ gestation through pregnancy schools within the University Hospital’s catchment area. Women in the intervention arm received five online sessions of the THP-BGV delivered by antenatal nurses. The intervention employed principles of cognitive behaviour therapy to provide psychoeducation, behaviour activation, problem-solving strategies and group support to participants. In the control arm, women received usual care consisting of routine online educational pregnancy classes aided by the antenatal nurses. The women were assessed for depressive symptoms with the Edinburgh Postnatal Depression Scale at baseline and 4-6 weeks post-intervention and also evaluated for anxiety, perceived social support, partner relationship, level of disability and sleep quality. In-depth interviews were conducted with women and other key stakeholders. Results: Of the 99 consecutive women referred to the pregnancy schools, 91 (91.9%) were eligible and 88 (88.8%) consented to participate in the study and were randomized. Eighty-two (83%) completed the final assessments. Our main findings were that this preventive group intervention was feasible to be integrated into routine antenatal educational classes and it was valued by the women and delivery-agents. While the study was not powered to detect differences between intervention and control conditions, we found small trends towards reduction in anxiety and depressive symptoms favoring the intervention arm. No serious adverse events were reported. Conclusions: Given the paucity of preventive interventions for perinatal depression in low and middle-income countries, a fully powered definitive randomized controlled trial of this feasible and acceptable intervention should be conducted. S., Save D.Background and Objective: Restrictions in the COVID-19 pandemic have affected access to healthcare and caused delays in the routine care of people with chronic diseases. The aim of our study is to reveal the impact of the COVID-19 pandemic on routine care of people with chronic diseases. Methods: Our qualitative study was carried out with 12 people who came to the Cardiology outpatient clinics of tertiary Training and Research Hospital in June 2022 for a routine care due to their chronic diseases. As a data collection tool, a semi-structured questionnaire was used. The data were collected through face-to-face interviews with the participants by taking the necessary protective measures. Thematic analysis method was used in the analysis of the data. After the interviews were transcribed, each transcript was coded using the ATLAS.ti program. The ethics committee approval of the study was obtained. Results: The ages of participants were between 34-73, and 8 of them were male. Participant experiences were categorised into four themes: General Health Status, Routine Care during the Pandemic, Chronic Disease Management during the Pandemic and Patient Expectations of Healthcare. Most of the patients delayed their routine care during the pandemic and their chronic diseases worsened. Among the reasons for not going to the controls, the fear of contagion came to the fore, while some participants mentioned that they could not find an appointment. In terms of their expectations of healthcare, they stated that health institutions should give priority to those with chronic diseases and home health services and online services will provide convenience for them. Conclusion: Delaying hospital admissions for routine care made the management of chronic diseases difficult and worsened the chronic diseases of patients in the long term. During the pandemic students individual, mental and social lives are highly affected. Ş., Özer F., Tatar ., Kılınckıran M., Can N., Save D.Objective: In 2011, Turkey started the nationwide \"Smoking Cessation Treatment Support Program\" via Smoking Cessation Clinics (SCC). This study examined the accessibility of these clinics using Levesque’s framework of access to healthcare services. From the (potential) users’ perspectives, it was aimed to find the challenges they face in accessing these clinics, and it proposed solutions for better implementation. Methods: This study was conducted among 304 smokers who visited four Family Health Centers in a geographically defined area in Istanbul, Turkey, in 2018. Result: In this study, it was found that 74% of smokers wished to quit smoking. Within them, 21% didn’t know of SCCs, and half of them didn’t know what services were provided there. Among those who knew of SCCs, 30% used them to quit smoking. The majority (75.7%) of the SCC users found the medications expensive; 41% didn’t use them, and 64% didn’t re-visit the clinics for follow-ups. Eventually, 4.3% of smokers quit smoking through SCCs. Utilization of these clinics were significantly higher among women (35%) than men (18.6%) (p=0.024). T; and there was no other differences in terms of age, marital status, income, and education level. Conclusion: Smoking cessation programs in Turkey and other countries can be implemented more efficiently if the sufficient number of clinics are opened and are accessible at the local level; more health professionals are trained to practice; the clinics are made more visible; smoking cessation medications are made available constantly; and closer monitoring of service users is ensured. Sağlıkla iç içe iki bölümünde okuyan öğrencilerin beslenme ve fiziksel aktivite düzeylerinin değerlendirilmesi sağlıkla ilgili sorunları önlemek için önemlidir. Bu çalışmayla spor bilimleri fakültesi ve tıp fakültesinde okuyan öğrencilerin beslenme ve fiziksel aktivitelerinin değerlendirilmesi amaçlanmıştır.Yöntem: Bu çalışma tanımlayıcı tipte bir araştırmadır. Örneklem seçilmemiş olup üniversitenin tıp fakültesi 1. 2. ve 3. sınıf öğrencileri (n=642) ile spor bilimleri fakültesi 1. 2. ve 3. sınıf öğrencilerine (n=773) çevrimiçi ortamdan anketler iletilmiştir. Anket bireysel sorular, Beslenme ve Egzersiz Davranış Ölçeği (BEDÖ) ve Uluslararası Fiziksel Aktivite Anketi (UFAA) olmak üzere 3 bölümden oluşmaktadır. Verilerin analizi Jamovi programında yapılmıştır, istatistiksel anlamlılık değeri p <0.05 olarak kabul edilmiştir.Bulgular: Çalışmaya 233 öğrenci katılmıştır. Katılımcıların yaş ortalaması 20,04±2,01. Çalışmaya katılan tıp fakültesi öğrencilerinin 65’i erkek (%27,8), 83’ü kadın (%35,6); spor bilimleri (SBF) öğrencilerinin 50’si (%21,4), erkek 35’i kadındır (%15,0). Tıp ve spor bilimleri fakültesi öğrencilerinin BEDÖ puanları tüm alt boyutlarda istatistiksel anlamlılık göstermektedir. Spor bilimleri fakültesi öğrencilerinin UFAA skorlarının medyan değeri 2922,0 MET dk/hafta (çok aktif) iken tıp fakültesi öğrencilerinin UFAA skorlarının medyan değeri 1459,5 MET dk/haftadır (minimal aktif) (p<0,001). Erkekler daha sağlıklı beslenme davranışı ve psikolojik yeme davranışını sergilemekle birlikte fiziksel olarak daha aktiftirler.Sonuç: Spor bilimleri fakültesi öğrencileri beslenme davranış puanları tıp fakültesi öğrencilerine nazaran daha yüksektir. P., Sarioz A., SAVE D.Publication Open Access Asemptomatik covıd-19 olgularında bulaştırıcılığın ve ilişkili faktörlerin değerlendirilmesi(2022-01-01) GİRGİN, ŞEVKET; TOPUZOĞLU, AHMET; SAVE, DİLŞAD; Peker Ş., Kurnaz Ay M., Girgin Ş., Topuzoğlu A., Save D.COVID-19 (SARS-CoV-2) pandemisi; prevalansı, ciddi klinik sonuçları ve kolay bulaşan bir enfeksiyon olması nedeniyle günümüzün en öncelikli halk sağlığı sorunları arasında bulunmaktadır. Enfeksiyonun kliniği asemptomatik düzeyden çoklu organ yetmezliğine değişiklik göstermektedir. Araştırmalara göre COVID-19 enfeksiyonu geçirenlerin %17,9-%80,0’inin asemptomatik olgular olduğu bildirilmiştir. Asemptomatik olguların önlem alınmaması halinde COVID-19’u geniş kitlelere bulaştırma potansiyeli üzerine tartışmalar sürmektedir. Ayrıca asemptomatik olgularda hastaların viral yükü, viral bulaşma modelleri ve potansiyel enfektivitesi ile ilgili belirsizlikler mevcuttur. Serolojik testlerle hastalığın immün sistem üzerindeki etkileri izlenebilmektedir fakat enfeksiyonu geçiren bireylerde bağışıklık süresine ilişkin net bir uzlaşma sağlanamamıştır. Bu belirsizliklerin açığa kavuşturulması salgınla mücadele açısından önemli bir yere sahiptir.Bu makalede; COVID-19 enfeksiyonunu asemptomatik olarak geçiren olgularla ilgili yapılmış olan araştırmalarderlenerek olguların tespiti, hastalığı bulaştırma düzeyleri, viral yükleri, bulaştırıcılık süreleri ve potansiyel enfektiviteleriile ilgili konular tartışılmıştır.
