Publication Metadata only Perı̇odontal olarak etkı̇lenmı̇ş dı̇şlerde stanöz florür içeren dı̇ş macunu ile dentı̇n tübüllerı̇nı̇n tıkanması: SEM çalışması(2021-12-25) DOĞAN, ÇİĞDEM; YILDIRIM, HATİCE SELİN; KURU, LEYLA; DOĞAN Ç., GÜNGÖRMEK H. S., GÜRSOY H., KURU L.Amaç: Periodontal doku kaybı nedeniyle ağız ortamına açılan dentin tübüllerinin termal, kimyasal veya mekanik uyaranlara maruz kalmasıyla dentin hassasiyeti ortaya çıkar. İçinde çeşitli etken maddeler bulunan diş macunları vasıtasıyla dentin tübüllerinin kapatılması, hassasiyet tedavisinde en çok kabul gören yöntemlerden biridir. Bu çalışmanın amacı periodontal harabiyetli dişlerde stanöz florür (SnF₂) ve sodyum florür (NaF) kombinasyonu içeren hassasiyet giderici diş macununun dentin tübüllerinin kapanması üzerine etkisinin Scanning Electron Microscopy (SEM) ile incelemektir. Gereç ve Yöntem: Çalışmaya periodontal harabiyet nedeniyle çekilmiş tek köklü premolar dişlerden elde edilen 30 dentin örneği dahil edildi ve 3 gruba ayrıldı, Grup I (kontrol): yalnızca sitrik asit; Grup II: SnF₂ ve NaF kombinasyonu; Grup III: NaF içeren diş macunu uygulandı. Uygulama elektrikli diş fırçası kullanarak 10 saniye süre ile günde 2 kez yapıldı, 7 gün boyunca tekrar edildi ve fırçalama aralarında örnekler yapay tükürükte bekletildi. Daha sonra örnekler 24 saat oda sıcaklığında kurutularak SEM ile incelendi. Alınan mikrofotoğraflarda x2000 büyütmede Image J yazılım programı kullanılarak açık ve kapalı dentin tübüllerinin sayımı yapıldı. Gruplar arası çoklu karşılaştırmalarda Kruskal Wallis testi ve ikili karşılaştırmada Bonferroni düzeltmeli Mann Whitney U testi kullanıldı. Bulgular: Grup II (%99.70±0.41) ve Grup III (%95.64±2.35)’ün Grup I'e (%0.84±0.19) göre tübül kapanma yüzdelerinin istatistiksel olarak anlamlı daha yüksek olduğu tespit edildi (p<0.001). 100 µm²’ye düşen açık tübül sayıları ise Grup I'de (1.33±0.44), Grup II (0.00±0.01) ve Grup III'e (0.06±0.03) göre istatistiksel olarak anlamlı daha az olduğu tespit edildi (p<0.001). Sonuç: Bu çalışmanın sınırları dahilinde, her iki macun da periodontal harabiyet nedeniyle çekilmiş dişlerde dentin tübüllerini kapamada etkili olmakla birlikte SnF₂ ve NaF kombinasyonu içeren diş macunu 7 günlük kullanımdan sonra en fazla sayıda dentin tübülünün kapanmasını sağlamıştır.Publication Metadata only Effect of topically applied hyaluronic acid on pain and palatal epithelial wound healing: An examiner-masked, randomized, controlled clinical trial(AMER ACAD PERIODONTOLOGY, 2018) DOĞAN, BAŞAK; Yildirim, Selin; Ozener, Hafize Ozturk; Dogan, Basak; Kuru, BaharBackground: This study aims to evaluate the effects of two different concentrations of topical hyaluronic acid (HA) on postoperative patient discomfort and wound healing of palatal donor sites after free gingival graft (FGG) surgery. Methods: Thirty-six patients requiring FGG were randomly assigned into three groups in an examiner-masked, randomized, controlled clinical trial. After harvesting palatal grafts, 0.2% and 0.8% HA gels were used in test groups 1 and 2, respectively. Gels were applied on donor sites and protected with periodontal dressing in the test groups, whereas the wound was covered only with periodontal dressing in the control group. On days 3, 7, 14, and 21, pain and burning sensation were recorded using a visual analog scale (VAS) as well as other parameters such as complete epithelization (CE) and color match on days 3, 7, 14, 21, and 42. Results: Test groups experienced less pain than the control group on days 3 and 7 (P < 0.001 and P < 0.001, respectively). Mean VAS score for burning sensation was higher in the control group on day 3 compared with test groups 1 and 2 (P = 0.03 and P = 0.02, respectively). CE in all patients was achieved on day 21 in both test groups, whereas it was achieved on day 42 in the control group. The test groups showed higher color match scores than the control group on days 21 (P < 0.001 and P < 0.001, respectively) and 42 (P = 0.004 and P = 0.002, respectively). Conclusion: Topical application of HA exhibits positive impact on postoperative pain and burning sensation, and accelerates palatal wound healing in terms of epithelization and color match.Publication Metadata only Treatment of Localized Gingival Recessions Using Gingival Unit Grafts: A Randomized Controlled Clinical Trial(WILEY, 2013) YILDIRIM, HATİCE SELİN; Kuru, Bahar; Yildirim, SelinBackground: One of the success factors in periodontal plastic surgery is the synergistic relationship between involved tissues and vascular supply. Gingiva as a functional unit is unique with a specific vascular configuration and contains the supracrestal portion naturally created to survive over avascular root surfaces. The aim of this randomized controlled trial is to clinically evaluate the treatment of localized gingival recessions by using gingival unit grafts (palatal tissue involving marginal gingiva and papillae) compared with conventional palatal grafts. Methods: Seventeen patients with Class I to II recession defects on mandibular anterior teeth were included and randomly divided into two groups. Recessions were treated with gingival unit grafts in group 1 (n = 8) and with palatal grafts in group 2 (n = 9). Clinical parameters including vertical recession (VR), probing depth, keratinized tissue (KT), and attachment level were recorded at baseline and 8 months after surgery. Results: Both treatments produced significant clinical improvements within the groups. Intergroup comparison revealed significantly higher VR reduction, attachment, and KT gain in group 1 than in group 2; mean percentages of the defect coverage were 91.62% +/- 9.74% and 68.97% +/- 13.67%, respectively (P <0.05). Healing of the gingival unit donor site was uneventful. Conclusion: Within its limits, this study demonstrates the possibility of treating buccal recessions with gingival unit grafts as an alternative technique using gingival donor graft of site-specific vascular configuration, with better defect coverage, clinical, and esthetic improvements compared with palatal grafts. S., Ateş M., Oktay Gün I., Kuru B., Çakırer B., Kuru L.Background: Piezocision, a minimally invasive surgical procedure, has been used to accelerate tooth movement. Aim: The aim of this randomized split‑mouth study was to evaluate gingival crevicular fluid (GCF) osteocalcin (OC) and type I collagen cross‑linked C‑terminal telopeptide (ICTP) levels during canine distalization with and without piezocision acceleration. Material and Methods: Fifteen systemically healthy subjects (M:F 7:8, 16.27 ± 1.14 years) requiring extraction of maxillary first premolars before retraction of canines were included in the study. Piezocisions were randomly carried out on one of the maxillary canines while bilateral canines served as controls. Canine distalization was conducted using closed‑coil springs applying a force of 150 g/side by using miniscrews as anchorage. GCF sampling was performed from maxillary canine mesial and distal sites at baseline, 1, 7, 14, and 28 days. The GCF levels of OC and ICTP were detected by enzyme-linked immunosorbent assay (ELISA). The rate of tooth movement was evaluated at 2‑week intervals. Results: The amounts of canine distalization from baseline to 14 and 28 days in the piezocision group were significantly higher than the control group (P < 0.05). The GCF OC level of the piezocision group on the tension side and the ICTP level of the same group on the compression side were higher than the respective sides of the control group on day 14 (P < 0.05). S., KALKAN Y., İYİGÜN S., DOĞAN B.Background & Aim: Interleukin (IL)-13 is an anti-inflammatory and fibrogenic cytokine activated by T helper 2 cells. It has been shown that IL-13 inhibits pro-inflammatory cytokine synthesis and osteoclastogenesis. The aim of the study was to determine the effect of non-surgical periodontal treatment (NSPT) on salivary IL-13 levels in periodontitis patients with and without type 2 diabetes mellitus (T2DM). Methods: A total of 48 subjects, 12 systemically and periodontally healthy (H), 12 periodontitis with well controlled T2DM, 12 periodontitis with poorly controlled T2DM and 12 systemically healthy periodontitis, were recruited for the study. All periodontitis patients belonged to either Stage III or IV periodontitis. At baseline, all clinical parameters including plaque index, gingival index, bleeding on probing, probing depth and clinical attachment loss were recorded, and the saliva samples were collected from all subjects, at 3 months following the NSPT only from periodontitis groups. Salivary IL-13 levels were analysed by enzyme-linked immunosorbent assay. This study was supported by the grant from Marmara University Scientific Research Projects Coordination Unit #SAG-A-080519-0172. Results: All clinical parameters were higher in all periodontitis groups than the healthy subjects at baseline (p < 0.05) and improved 3 months after NSPT in all periodontitis groups (p < 0.05). At baseline, salivary IL-13 levels were lower in all periodontitis groups compared to H group (p < 0.05) but were similar among 3 periodontitis groups (p > 0.05). At 3 months following NSPT IL-13 levels were increased in all 3 periodontitis groups compared to baseline (p < 0.05). Conclusions: This study has revealed that low levels of salivary IL-13 are associated with periodontitis not T2DM. S.AMAÇ: Çalışmamızda, sağlıklı ve deneysel olarak diyabet oluşturulmuş sıçanlarda, hyalüronik asitin (HA) gargara formunun topikal olarak uygulamasının palatinal yara iyileşmesi üzerine etkilerinin klinik ve histomorfometrik olarak incelenmesi amaçlandı. GEREÇ VE YÖNTEM: Çalışmamızda 60 adet erkek Wistar albino sıçan kullanıldı. Sıçanlar sağlıklı ve deneysel diyabet oluşturulan 2 ana gruba ayrıldı. Rastgele seçilen 30 sıçana intraperitoneal olarak 110 mg/kg nikotinamid uygulanmasından 15 dakika sonra distile su içinde çözündürülen streptozotosin tek doz 65mg/kg intraperitonal olarak enjekte edilerek diyabet oluşturuldu. Her iki gruptaki sıçanların palatinal bölgelerinin ortasında 5 mm çapında kemiğe kadar mukozal defekt oluşturuldu. Takiben her iki gruptan 6’şar sıçan 0. günde sakrifiye edildi. Ardından sağlıklı ve diyabetik gruplar kendi içlerinde HA ve salin uygulanan 2 alt gruba ayrıldı. Yara bölgesine günde iki kez 1 ml HA veya salin uygulandı. Defektler oluşturulduktan 0, 7 ve 14 gün sonra, yara alanları (YA) fotoğraflar üzerinden ölçüldü. Örneklere rutin histolojik doku takibi uygulandıktan sonra alınan kesitlere hematoksilen-eozin boyaması yapıldı ve epitelyal kenarlar arasındaki ortalama mesafe (EKM) histomorfometrik olarak belirlendi. BULGULAR: Ortalama YA tüm gruplarda zamanla istatistiksel olarak azaldı (p<0.05). Sağlıklı ve diyabetik HA grupları, sağlıklı ve diyabetik salin grupları ile karşılaştırıldığında YA’ da önemli bir azalma gözlendi (p<0.05). EKM, diyabetik salin grubu hariç tüm gruplarda başlangıca kıyasla önemli ölçüde azaldı (p<0.05). Sağlıklı ve diyabetik HA grupları, sağlıklı ve diyabetik salin gruplarına kıyasla EKM ‘de önemli bir azalma gösterdi (p<0.05). S.Objective: Evaluation of dental treatment of intellectually disabled (ID) people is often confusing, since the wide range of ages (children,teenagers and adults), oral health problems and concomitant diseases. There have been many studies in pediatric ID patients but little is known about the differences between age groups in children or adults. Materials-Methods: We hypothesize that children and adults with ID may have different oral health outcomes and that differences will be statistically significant. The DMFT/DMFS results, Community Periodontal Index of Treatment Needs (CPI-TN), functional occlusal pairs and units.Study population is the ID patient operated in general anesthesia between 2021-2023. All patients aged 14-65 were before general anesthesia and DMFT/DMFS, CPI, TN, occlusal unit and functional occlusal pairs data was collected. The patients were grouped according to ages on 14-24(group A) and >25 (group B) years. Study parameters were compared between this study groups. Results: The number of patient of groups A and B were 17 and 21, respectively. The values of DMFT, DMFS, occlusal unit, functional occlusion pairs, number of missing teeth and number of missing tooth surfaces in our patients were in group A and group B have no difference between before surgery, after surgery and difference after surgery-before surgery. CPI data have difference between groups. TN data have no difference between groups Conclusion: It was concluded that if patients are given tooth brushing education at an early age or if they are taken to multidisciplinary surgery, better results will be obtained in terms of the health of periodontal tissues and the treatment needs of patients will be reduced.
