    Kardiyovasküler komorbiditesi bulunan ve bulunmayan esansiyel hipertansiyon hastalarında ilaç kullanımının araştırılması

    Amaç: Hipertansiyon, komorbiditesi sık bir hastalık olup bu durum tedaviyidüzenlenmede belirleyicidir. Bu çalışmada hipertansif hastalarda başkakardiyovasküler sorunların olup olmama durumuna göre ilaç kullanımınınincelenmesi amaçlandıGereç ve Yöntem: Bu retrospektif çalışmaya Türk Kalp Vakfı Tıp Merkezi’ninOcak 2016-Haziran 2018 arasındaki hasta verileri dahil edildi (n=11.085).Hastalara ait kayıtlı tanılar içerisinde (n=26.699) en sık karşılaşılan üç tanı olanhipertansiyon (HT), iskemik kalp hastalığı (İKH) ve dislipidemi (DL)bulunanların tedavilerindeki ilaçlar, tıbbi ve demografik özelliklerideğerlendirildi. Hipertansiyon verileri ile buna eşlik eden İKH ve DL verilerikarşılaştırıldı.Bulgular: Çalışma periyodunda oluşturulan protokollerin %22,2’sini (n=5929)HT oluşturmaktaydı. Toplam 3601 HT tanılı hastanın %43,7’sini (n=1572) tektanılı olanlar, %36,7’sini ise (n=1321) eşlik eden iki komorbiditenin en az biriniiçeren hastalar oluşturuyordu. İncelenen tüm tanı gruplarında en sık karşılaşılanilaç asetilsalisilik asit idi. En sık karşılaşılan antihipertansif ana ilaç grubu reninanjiyotensin sistemi (RAS) blokerleri, etkin maddeler ise metoprolol veamlodipin idi. Bunu tek başına HT ve HT’ye DL’nin eşlik ettiği grupta“valsartan+hidroklorotiyazid”, İKH’nin eşlik ettiği gruplarda ramipril izliyordu.Beta-bloker kullanımı ≥65 yaş hastalarda genç hastalara kıyasla daha düşüktü(sırasıyla %13,8 ve %16,1, p<0,05).Sonuç: Bu çalışmada en çok tercih edilen antihipertansif grubun, anjiyotensinreseptör blokerlerinin baskınlığında, RAS üzerinden etkililer, en çok tercih edilenilacın ise metoprolol olduğu göze çarpmaktadır. Son yıllardaki kılavuzlarda betabloker kullanımının nispeten geri plana düşmüş olmasına rağmen, bu çalışmadahipertansif hastalarda yaygın olarak kullanıldığı görülmektedir. Hipertansiyonaeşlik eden iki durumda da en sık kullanılan ilaçların genel olarak değişkenlikgöstermediği anlaşılmaktadır.

    Evaluation of websites reached using Google in the modern digital era related to approach to cholesterol

    Objective: The Google search engine is widely used as a source of medical information; however, legal and medical governance of the accuracy of the content retrieved is lacking. The aim of this study was to assess the most read Turkish-language texts related to cholesterol during a specific period according to the validity of the content. Methods: Google Trends was queried on January 5, 2019 for the search term cholesterol and the 9 other most popular search phrases used in Turkey that included the word cholesterol. In all, 100 links were obtained for each phrase, generating a total of 1000 links. Once duplicates were eliminated, a total of 604 links was used for the study. Since there is currently no validation scoring system for this purpose in the literature, the authors created a checklist according to well-accepted recent guidelines focused on cholesterol. The content of the texts acquired was classified as misleading, insufficient but favorable, or sufficient and favorable. Results: The source of the online texts studied was universities (n=8, 1.3%), hospitals (n=6, 0.9%), personal blogs (n=200, 33.1%), health websites (n=183, 30.2%), and medical journals (n=207, 34.2%). In all, 235 texts (38.9%) were classified as sufficient and favorable and 35 (5.7%) were categorized as misleading. A medical practitioner was named in 378 texts (62.5%). All of the results from universities and hospitals were ranked in the favorable group. A statistical difference in the word count was seen in a comparison of the misleading and favorable texts. Conclusion: Google can connect users to a significant quantity of material related to cholesterol that includes a wide range from misleading information to sufficient and favorable texts. The variation in the quality of the content on websites accessible via Google necessitates that cholesterol resource material should be selected with great care.

    Baseline characteristics predicting clinical outcomes and serious adverse events in middle-aged hypertensive women: a subanalysis of the SPRINT in women aged < 65 years

    Background/aim: The predictability of clinical outcomes in hypertension in specific patient groups, especially underrepresented populations is the key to rational treatment. This study aimed to investigate the impact of baseline characteristics of <65-year-old hypertensive women with an increased risk of cardiovascular events, managed with standard- or intensive-approach, on their clinical outcomes and serious adverse events (SAEs). Materials and methods: Baseline characteristics of <65-year-old hypertensive women (n = 1247) in SPRINT, a multicenter randomized trial to compare standard and intensive antihypertensive treatment, were analyzed with Cox-regression method to determine potential predictors of the clinical outcomes and SAEs. The primary outcome was the composite of myocardial infarction (MI), non-MI acute coronary syndrome, stroke, heart failure, or cardiovascular death. Results: The primary outcome occurred in 3.1% and SAEs in 27.6% of the population. The treatment groups were similar in terms of the primary outcome, SAEs, or their individual components. The primary outcome occurred significantly more in current smokers vs. nonsmokers (HR: 2.85, 95% CI: 1.34-6.09). The subjects who were on aspirin in the intensive-group were significantly more likely to develop the primary outcome (HR: 3.17, 95% CI: 1.23-8.19) and MI (HR: 10.15, 95% CI: 1.19-86.88) compared with those not using aspirin. The risk of overall SAEs was significantly higher in blacks vs. nonblacks (HR: 1.27, 95% CI: 1.01-1.58), in current-smokers vs. nonsmokers (HR: 1.59, 95% CI: 1.23-2.05), and those with vs. without chronic kidney disease (CKD), (HR: 1.38, 95% CI: 1.08-1.77). The likelihood of SAEs significantly increased with age (HR: 1.04, 95% CI: 1.01-1.07). Conclusion: Smoking, aspirin, CKD, black race, and age seemed as important baseline characteristics in follow-up of <65-year-old hypertensive women, also depending on therapeutic strategy. Clinicians are expected to consider these critical parameters for effective antihypertensive management that promotes better outcomes in this middle-aged female population.

    The role of myocardial performance index and Nt-proBNP levels as a marker of heart dysfunction in nonalcoholic cirrhotic patients

    Objective: Cardiac dysfunction has been reported in both cirrhotic and alcoholic patients. Our aim was to determine the relation of serum N-terminal pro-B-type natriuretic peptide (Nt-proBNP) levels to myocardial performance index (MPI) and disease severity in nonalcoholic cirrhotic patients. Patients and Methods: In this prospective study including 25 cirrhotic patients and 27 healthy controls, MPI was assessed by pulsedwave tissue Doppler imaging (PW-TDI). The disease severity was determined by Child–Turcotte–Pugh (CTP ) and model for endstage liver disease (MELD) scores. Results: There were no statistically significant differences in MPI levels between patients and controls (p< 0.246). Nt-proBNP levels (p< 0.0003), cardiac output (CO) (p< 0.0002), left ventricular end-systolic (LVES) volume (p< 0.031) and QT interval (p< 0.0001) increased and left ventricular systolic function was normal in all cirrhotic patients when compared to controls. Nt-proBNP levels were positively correlated with MELD scores (p< 0.0001, r= 0.59), QT duration (p< 0.0001, r= 0.59), CO (p= 0,001, r= 0.44), right atrial (RA) area (p= 0.026, r= 0.31) and negatively correlated with diastolic BP (p= 0.015, r= – 0.34). Conclusion: We conclude that in nonalcoholic cirrhotic patients, left ventricular MPI and systolic function were normal. Nt-proBNP levels were correlated with the disease severity and hyperdynamic circulation.

    The effectiveness of a fall detection device in older nursing home residents: a pilot study

    Background: Real-world research to evaluate the effect of device technology in preventing fall-related morbidity is limited. This pilot study aims to investigate the effectiveness of a non-wearable fall detection device in older nursing home residents. Methods: The study was conducted in a nursing home with single-resident rooms. Fall detection devices were randomly set up in half of the rooms. Demographic data, comorbidities, lists of medications, and functional, nutritional, and frailty status were recorded. The residents were followed up for 3 months. The primary outcome was falls and the secondary outcome was all-cause mortality. Results: A total of 26 participants were enrolled in the study. The study group consisted of 13 residents who had a fall detection device in their rooms. The remaining 13 residents on the same floor formed the control group. Participants had a mean age of 82 ± 10 years and 89% of the residents were female. The most prevalent comorbidity was dementia. Two residents from the control group and one resident from the study group experienced a fall event during follow-up. The fall events in the control group were identified retrospectively by the nursing home staff, whereas the fall in the study group received a prompt response from the staff who were notified by the alarm. One resident was transferred to the hospital and died due to a non-fall related reason. Conclusion: Device technology may provide an opportunity for timely intervention to prevent fall-related morbidity in institutionalized older adults.
