    Serebral palsili çocuğu olan ebeveynlerde bakım yükü, psikolojik durum ve yaşam kalitesi ilişkisi

    Amaç: Serebral Palsili (SP) bir çocuğa bakım veren ebeveynlerin bakım yüklerinin sağlıklı bir çocuğa bakım veren ebeveynlerden fazla olduğu bilinmektedir. Bu çalışma ile birincil bakım veren ebeveynlerin bakım yükü (BY) ile psikolojik durumları (PD) ve yaşam kaliteleri (YK) arasındaki ilişkinin incelenmesi hedeflenmiştir. Araçlar ve Yöntem: Çalışma, SP'li çocuğa sahip 101 ebeveyn ile prospektif olarak gerçekleştirilen bir kesitsel çalışmadır. Bu çalışmada ebeveynlerin BY, PD ve YK sırasıyla Zarit Bakım Verme Yükü Ölçeği, Beck Depresyon Envanteri ve Kısa Form-36 ile ölçüldü. Ayrıca SP'li çocukların YK’sı Katz Günlük Yaşam Aktiviteleri Ölçeği ile fonksiyonellikleri ise Pediatrik Fonksiyonel Bağımsızlık Ölçümü ile değerlendirildi. Bulgular: Analiz sonuçlarına göre bakım verenlerin büyük çoğunluğu annelerden oluşmaktaydı (%90.09). Değerlendirmeler sonrasında çocukların günlük yaşam aktiviteleri (GYA) ve fonksiyonellik seviyeleri orta düzeyde bulundu. Bununla paralel olarak ebeveynlerin bakım yükleri orta-ciddi düzeyde iken psikolojik durumları hafif depresif olduğu görüldü. Ebeveynlerin BY ile PD arasında orta düzey (r=0.472, p<0.001), YK alt boyutu olan fiziksel fonksiyon ile negatif yönlü orta düzey (r=-0.475, p<0.001) bir korelasyon vardı. Çoklu regresyon modeline bakıldığında ise çocukların GYA seviyeleri ve ebeveynlerin PD durumları bakım yükünün %27.4'ünü açıklamaktadır. Sonuç: SP'li çocuğa bakım veren ebeveynlerin bakım yüküne bağlı olarak psikolojik sağlıkları olumsuz yönde etkilenmekte ve yaşam kaliteleri düşmektedir. Bu sebeple bakım planlamaları ve hizmetleri sadece SP'li çocuklara yönelik değil bakım veren ebeveynleri de kapsayıcı şekilde olması faydalı olacaktır.

    The effect of kinesiological taping on pain, function, and disability in subacromial impingement syndrome- a randomized controlled study

    Objective: Subacromial Impingement Syndrome (SIS) is one of the most common shoulder pathologies that cause pain and lose of function. This study aimed to compare the effects of kinesiological taping and conventional physiotherapy (CP) on pain, functionality, and disability in SIS patients. Materials and Methods: The study was completed with 54 patients aged 18-60 years. By randomization, the participants were divided into two groups: The Intervention Group (IG) and the Control Group (CG). While the CP was performed for ten sessions, kinesiological taping was repeated every three days for the IG. The CG was taken to only ten sessions of CP. The CP was applied for 60x5x2 minutes/day/week. Pain severity, function, and disability were assessed before and after the treatment. Results: There was no difference between the groups in pain severity (p=0.074), function (p=0.565), and disability (p=0.315) scores. On the other hand, there were significant improvements in pain (p=0.001), function (p=0.001), and disability (p=0.005) scores of IG according to intra-group analysis. Similar results were obtained in the CG. However, when the effect sizes of these treatment methods were examined, they did not create a clinically significant superiority compared to each other. Conclusion: Kinesiological taping has been widely used in patients with orthopedic problems. According to our results, kinesiological taping did not have a clinical effect in reducing pain, improving function and disability when applied with CP compared to CP alone. We think that only kinesiological taping may not be sufficient be an alternative to traditional methods in the early term.

    First exercise reports of diabetes center in Turkey: effects of exercise on metabolic parameters in patients with type-2 diabetes mellitus

    BACKGROUND: The current study aimed to show the efficacy of the exercise program performed by the type-2 diabetic patients in the exercise center related to the diabetes clinic. METHODS: This was a retrospective study, conducted on 77 (18-65 years old) diabetic patients who were attending the exercise center of the outpatient diabetes clinic. Patients who completed 3 months of supervised combined exercise program and having no chronic drug adjustment for 3 months at the beginning and during the exercise interval were recruited to the study. RESULTS: After 3 months, supervised combine exercise program, there were significant improvement of BMI (34.62 vs. 33.14 kg/m(2), P<0.001), fat mass (34.95 vs. 33.83 kg, P<0.001), fasting blood glucose (124.59 vs. 117.39 mg/dL, P=0.015), HbA1c (6.74 vs. 6.45, P<0.001), and triglyceride levels (177.78 vs. 167.50 mg/dL, P=0.038). CONCLUSIONS: In conclusion, as a real-life data the supervised combine exercise program, performed in the exercise center of the diabetes clinic, significantly improved the metabolic parameters of the patients. We believe that diabetes clinics should include exercise centers, which are controlled by experts, to provide more qualified management for diabetic patients.

    Effect of the residential areas of geriatrics living in turkey on physical activity, depression, and quality of life during the covid-19 pandemic

    Objective: The quarantine precautions implemented during the new type of coronavirus epidemic caused some changes in the daily lives of geriatric individuals. This study aimed to examine whether living in rural or urban areas affects physical activity, depression, and quality of life (QoL) in Turkish geriatrics during pandemic. Method: One hundred three geriatrics were assessed with the videoconferencing method, which was performed once. Participants were divided into two groups according to the area where they lived as rural (n:41) or urban (n:62). In addition, physical activity, presence of depression, and QoL of individuals were assessed with the Physical Activity Scale for the Elderly (PASE), Geriatric Depression Scale (GDS), and Short Form-36 (SF-36), respectively. Results: According to the results, PASE (p=0.677), GDS (p=0.742), and all sub-dimensions of SF-36 (p>0.05 for all) except pain (p=0.033) of geriatrics were similar in both groups. Also, while there is a low-level relationship between the GDS scores of those living in rural areas and the sub-dimensions of SF-36 physical function (r=- 0.381), mental health (r=-0.381), and social functionality (r=-0.395), there was a moderate correlation between the vitality (r=-0.529) and pain (r=-0.536) sub-dimensions. In addition, a strong correlation was found between GDS and the general health perception sub-dimension of SF-36 (r=-0.611). Conclusion: The pandemic-induced quarantine precautions, which were put into effect throughout Turkey, negatively affected the daily life of all people, especially elderly individuals. Because of these precautions, the similarity of daily living activities of the elderly living in rural and urban areas may have caused similar physical activity, depression, and QoL scores.

    Is the robotic rehabilitation that is added to intensive body rehabilitation effective for maximization of upper extremity motor recovery following a stroke? A randomized controlled study

    Background: Trunk stabilization, which is a factor that directly affects the performance of affected upper-limb movements in stroke patients, is of critical importance in the performance of selective motor control. Aims: This study aimed to investigate the effects on upper-limb motor function of the addition of robotic rehabilitation (RR) and conventional rehabilitation (CR) to intensive trunk rehabilitation (ITR). Methods: A total of 41 subacute stroke patients were randomly allocated to two groups: RR and CR. Both groups received the same ITR procedure. Following ITR, a robot-assisted rehabilitation program of 60 min, 5 days a week, for 6 weeks, was applied to the RR group, and an individualized upper-limb rehabilitation to the CR group. Assessments were made at baseline and after 6 weeks using the Trunk Impairment Scale (TIS), Fugl-Meyer Upper Extremity Motor Evaluation Scale (FMA-UE), and Wolf Motor Function Test (WMFT). Results: Improvements were obtained in the TIS, FMA-UE, and WMFT scores for both groups (p 0.05). The RR group scores were relatively high, but not to a statistically significant. Conclusions: When added to intensive trunk rehabilitation, the robot-assisted systems, which are recommended as a stand-alone therapy method, produced similar results to conventional therapies. This technology can be used as an alternative to conventional methods under appropriate conditions of clinical opportunity, access, time management, and staff limitations. However, when RR is combined with traditional interventions such as intensive trunk rehabilitation, it is essential to investigate if the real effect is due to the robotic rehabilitation or the accumulation of positive effects of excessive movement or force spread associated with trained muscles.
