    Usage of NEWS for prediction of mortality and in-hospital cardiac arrest rates in a Turkish university hospital

    Background/aim: Early warning scores (EWS), widely used around the world but not yet in Turkey, are composed of physiological parameters designed to determine potentially worsening patients to perform necessary interventions in time. The aim is to determine the national EWS (NEWS) of the patients to assess the relation between this score and length of hospital stay (LOHS), transfer to the ICU, 24-h and 28-day mortality rates, and the frequency of in-hospital cardiac arrest (IHCA). Materials and methods: NEWS of all the patients in the internal medicine clinics were calculated via a point prevalence study. The LOHS, transfer to the ICU, rates of mortality in the 24-h and 28-day period, IHCA rate, and the period of the stay in the ICU parameters were determined. Results: Out of 104 patients, 84 of them had low scores (<5), while 20 had mid/high scores. In mid/high-score group (score>5), transfer to the ICU, IHCA rate, and mortality rates within 24 h and 28 days were significantly higher. Conclusion: In this, the first prospective study about EWS in Turkey, 24-h and 28-day mortality rates, transfer to the ICU, and IHCA frequency of the patients with mid/high NEWS were higher.

    Research publications from the ATS MECOR program in Turkiye

    Background: The American Thoracic Society (ATS) Methods in Epidemiologic, Clinical, and Operations Research (MECOR) Program aims to build research capacit in low and middle-income countries. MECOR has three levels, during which students learn to develop a research protocol and write a manuscript. MECOR Turkiye has been offered every year since 2008. Objective: The aim of this paper is to report the number and impact of published articles generated from research questions developed by students in levels 1, 2, and 3 of the ATS MECOR Program in Turkiye between 2008 and 2018. Methods: We collected the research questions developed in all levels of the ATS MECOR Program in Turkiye between 2008 and 2018. We searched Google Scholar, PubMed, Web of Science, and ResearchGate in April 2022 to see how many of these research questions were published as articles and, if published, in which journals. Results: Of the 176 research questions collected, 49 had been developed in level 1, 82 had been developed in level 2, and 45 had been developed in level 3. Of those 176 research questions, 55 (31.3%) generated articles that were accepted for publication. The frequency of published articles based on MECOR-developed research questions increased linearly as the course level in which they were developed increased (18.4% in level 1, 30% in level 2, 46.7% in level 3; P = 0.012; linear-by-linear association, P = 0.003). The median time from the development of the research question to publication was three years overall and did not differ significantly among the course levels (P = 0.36). Of the research questions developed, 43 were published in Science Citation Index or Science Citation Index Expanded–indexed journals. Conclusion: Acceptance of an article for publication is one way to measure the impact of the ATS MECOR Program. Our data describe significant research output among our participants, which increases with their length of participation in the program.

    Turkish thoracic society early career members task force group's virtual congress notes: european respiratory society international congress 2020

    In this article, Early Career Task Force Group members of the Turkish Thoracic Society summarize the European Respiratory Society 2020 virtual congress. Current developments in the field of respiratory diseases were compiled with the addition of sessions specific to coronavirus disease 2019 this year. Almost all of the congress sessions were examined, and the important and striking results of the congress were highlighted. Congress sessions were attended by expert researchers, and the prominent messages of each session were highlighted in short summaries. They were then grouped under relevant titles and ranked in order of meaning and relation. It was finalized by a team of researchers.

    The role of procalcitonin as a biomarker for acute pulmonary exacerbation in subjects with cystic fibrosis and non-cystic fibrosis bronchiectasis

    Objective: Patients with cystic fibrosis (CF) and non-CF bronchiectasis are prone to exacerbations of pulmonary infections. C-reactive protein (CRP) and procalcitonin (PCT) are inflammatory markers. The aim of this study is to evaluate the role of CRP and PCT on exacerbations of CF and non-CF bronchiectasis. Patients and Methods: The medical records of 18 CF (52 hospitalizations) and 20 non-CF bronchiectasis patients (51 hospitalizations) were reviewed retrospectively. CRP, PCT levels and, white blood cell (WBC) counts on admission and follow-up were evaluated. Results: C-reactive protein levels correlated with PCT levels on admission in all patients. Baseline PCT levels were markedly higher (>0.5µg/L) in 12% of CF and 10% of non-CF bronchiectasis patients, however, baseline CRP values were markedly higher (>5mg/L) in 96% of CF and non-CF bronchiectasis patients (p=0.760 and p=0.100, respectively). Baseline CRP and PCT levels were positively correlated with hospitalization length (r=0.501, p=0.001 and r=0.289, p=0.04, respectively) in CF patients, but not in non-CF bronchiectasis. Conclusion: Our study shows the potential utility of these biomarkers to determine the severity of the exacerbation particularly predicting hospitalization length in CF patients. Both biomarkers could be able to guide antibiotic treatment of infective exacerbations in CF and non-CF bronchiectasis patients.

    Red Blood Cell Transfusion in the Intensive Care Unit.

    IMPORTANCE Red blood cell (RBC) transfusion is common among patients admitted to the intensive care unit (ICU). Despite multiple randomized clinical trials of hemoglobin (Hb) thresholds for transfusion, little is known about how these thresholds are incorporated into current practice. OBJECTIVE To evaluate and describe ICU RBC transfusion practices worldwide. DESIGN, SETTING, AND PARTICIPANTS International, prospective, cohort study that involved 3643 adult patients from 233 ICUs in 30 countries on 6 continents from March 2019 to October 2022 with data collection in prespecified weeks. EXPOSURE ICU stay. MAIN OUTCOMES AND MEASURES The primary outcome was the occurrence of RBC transfusion during ICU stay. Additional outcomes included the indication(s) for RBC transfusion (consisting of clinical reasons and physiological triggers), the stated Hb threshold and actual measured Hb values before and after an RBC transfusion, and the number of units transfused. RESULTS Among 3908 potentially eligible patients, 3643 were included across 233 ICUs (median of 11 patients per ICU [IQR, 5-20]) in 30 countries on 6 continents. Among the participants, the mean (SD) age was 61 (16) years, 62% were male (2267/3643), and the median Sequential Organ Failure Assessment score was 3.2 (IQR, 1.5-6.0). A total of 894 patients (25%) received 1 or more RBC transfusions during their ICU stay, with a median total of 2 units per patient (IQR, 1-4). The proportion of patients who received a transfusion ranged from 0% to 100% across centers, from 0% to 80% across countries, and from 19% to 45% across continents. Among the patients who received a transfusion, a total of 1727 RBC transfusions were administered, wherein the most common clinical indications were low Hb value (n = 1412 [81.8%]; mean [SD] lowest Hb before transfusion, 7.4 [1.2] g/dL), active bleeding (n = 479; 27.7%), and hemodynamic instability (n = 406 [23.5%]). Among the events with a stated physiological trigger, the most frequently stated triggers were hypotension (n = 728 [42.2%]), tachycardia (n = 474 [27.4%]), and increased lactate levels (n = 308 [17.8%]). The median lowest Hb level on days with an RBC transfusion ranged from 5.2 g/dL to 13.1 g/dL across centers, from 5.3 g/dL to 9.1 g/dL across countries, and from 7.2 g/dL to 8.7 g/dL across continents. Approximately 84% of ICUs administered transfusions to patients at a median Hb level greater than 7 g/dL. CONCLUSIONS AND RELEVANCE RBC transfusion was common in patients admitted to ICUs worldwide between 2019 and 2022, with high variability across centers in transfusion practices.

    Validation and reliability of the Turkish version of the inflammatory bowel disease questionnaire for ulcerative colitis and Crohn's disease

    Background/Aims: Inflammatory bowel diseases (IBD) impair patients' quality of life (QoL). The IBD Questionnaire (IBDQ) is created to measure the health related QoL specific for IBD. We planned to investigate the validation and reliability of the Turkish translation of IBDQ. Materials and Methods: Patients filled self-report questionnaires (Turkish IBDQ [TrIBDQ] and 36-Item Short Form Health Survey [SF36]) themselves under a physician's supervision, and they were free to ask questions about the questionnaires. The participants then filled the same questionnaires after at least two weeks. Construct validity, discriminant ability, reliability, and susceptibility to change were analyzed separately for the patients with IBD. Intra-class correlation coefficient was used to assess test-retest reliability. Cronbach's alpha values were used to assess internal consistency. Results: A 100 patients enrolled in the study, 53 with Crohn's disease (CD) and 47 with ulcerative colitis (UC). We found a moderate-to-high positive correlation between the TrIBDQ domains and the SF-36 dimensions. In UC and CD, TrIBDQ was able to differentiate active disease and remission. We found that Cronbach's alpha for TrIBDQ domains ranged from 0.76 to 0.94 in CD and 0.79 to 0.92 in UC. The total Cronbach's alpha for TrIBDQ was 0.96 in CD and 0.95 in UC. Sensitivity-to-change analyses of the bowel, systemic, and emotional scores showed statistically significant differences between their baseline and follow-up values. Conclusion: TrIBDQ is a valid and reliable tool for assessing the QoL in Turkish speaking patients who have IBD. Thus, it can be used in clinical research and practice.

    Anxiety and depression in patients with chronic obstructive pulmonary disease and their relation to serum vitamin D levels

    Objectives: To determine whether serum vitamin D levels were related to the risk of anxiety and depression among patients with chronic obstructive pulmonary disease (COPD), as well as their spirometry parameters, disease severity, and other clinical findings. Materials and Methods: Ninety-two stable patients with COPD aged over 18 years with no exacerbations in the previous month were included. Symptom severity was assessed using the modified Medical Research Council (mMRC) dyspnea scale and COPD Assessment Test (CAT) scores. Spirometry tests were performed and previous hospitalization histories were recorded to determine ABCD groups of the patients according to their combined COPD assessments. Their Body mass index, airflow Obstruction, Dyspnea and Exercise capacity (BODE) indices were calculated and the Hospital Anxiety and Depression Scale (HADS) was administered. Serum vitamin D levels were determined using immune electrochemiluminescence. Results: According to the HADS scores, 16 patients (17.4%) had the risk of anxiety and 29 (31.5%) had the risk of depression. The risk of anxiety differed significantly (P=0.016 Fischer's exact test) between the ABCD groups. Anxiety scores were positively correlated with both mMRC (r=0.315; P=0.002) and CAT (r=0.445; P=0.000) scores. Similarly, depression scores correlated positively with mMRC (r= 0.249; P=0.017) and CAT (r=0.373; P=0.000) scores. Neither anxiety nor depression scores were significantly correlated with % of predicted FEV1. Serum vitamin D levels were inversely correlated with anxiety (r=-0.215; P=0.039) and depression (r=-0.244; P=0.019) scores. Anxiety scores were positively correlated with BODE although not statistically significantly (r=0.199; P=0.058). Conclusion: A negative correlation was revealed between serum vitamin D levels and HADS scores, and symptom scores were positively correlated to HADS scores, independent of % of predicted FEV1.
