Abstract
Nöromusküler hastalığı olan çocuklarda skolyoz önemli bir komorbitedir. Cerrahi girişimlerin öncesinde multidisipliner değerlendirme, beklenti ve hedeflerin belirlenmesi önemlidir. Bu amaçla yapılan solunum sistemi değerlendirmesi; gaz değişimi, akciğer hacimlerinin ölçümü ve etkin öksürük testleri, oksijen saturasyon ölçümü, arteriyel kan gazı analizi, spirometri, tepe öksürük akımı, maksimum inspiratuar basınç ve maksimum ekspiratuar basınç ölçümünü kapsamalıdır. Tüm bu değerlendirmelerden sonra operasyon öncesi noninvaziv pozitif basınçlı ventilasyon (NPBV) ve/veya mekanik insuflatör-eksuflatör ihtiyacı belirlenmeli ve gereğinde başlanmalıdır.
Scoliosis is one of the important risk factors in children with neuromuscular diseases. When surgery is planned, it is important to determine the multidisciplinary evaluation, expectations, and goals. Respiratory system assessment; gas exchange, measurement of lung volumes and effective cough tests, oxygen saturation measurement, arterial blood gas analysis, spirometry, peak cough flow, maximum inspiratory pressure, and maximum expiratory pressure measurement. After all the evaluations, the need for noninvasive positive pressure ventilation (NPBV) and/or mechanical insufflator-exsufflator should be determined before the operation and should be started if necessary.
