dc.description.abstractAmaç:Prognatik anomaliler hasta üzerinde ciddi sosyal ve fonksiyonel bozukluklar yaratır. Bu nedenle bu tür anomalilerin estetik ve fonksiyonel olarak rehabilitasyonu hastanın psikolojik ve dental tedavisi açısından gerekli olmaktadır. Bu vaka raporunda mental retarde hastada parafonksiyona bağlı olarak gelişmiş pseudoprognatik maloklüzyonun protetik rehabilitasyonu anlatılmıştır. Gereç ve Yöntem:Yirmi bir yaşında mental retarde erkek hasta Marmara Üniversitesi Diş Hekimliği Fakültesi Protetik Diş Tedavisi Anabilim Dalına yemek yiyememe şikayeti ile başvurmuştur. Ekstraoral muayenede hastanın sürekli dudaklarını emdiği ve alt çenesini önde konumlandırdığı görülmüştür. İntraoral muayenede 12 ve 21 numaralı dişlerin eksik olduğu 11 numaralı dişin palatal pozisyonda ve alt çenenin sınıf I kapanışını engellediği ve oklüzyonu sınıf III’e yönlendirdiği tespit edilmiştir. 13-22 nolu dişler arası sabit protetik restorasyon yapılmasına ve 11 numaralı dişin pozisyonunun değiştirilmesine karar verilmiştir. Tüm destek dişler chamfer bitim sınırı ile prepare edilmiş ve sınıf I kapanış sağlanmıştır. Hazırlanan metal destekli seramik restorasyonun oklüzal ilişkileri düzenlenmiş ve cam iyonomer siman (Ketac Cem 3M ESPE, ABD) ile simante edilmiştir. Bulgular: Diş erken teması sonucu meydana gelen pseudoprognatik vakada sabit protetik tedavi ile erken temas kaldırılmış ve hastada sınıf I oklüzyon sağlanmıştır. Hastanın 6 aylık klinik takibi sonucunda fonksiyonel, estetik ve fonasyonel bir şikayeti olmamıştır. Sonuç:Erken temas noktaları sebebiyle meydana gelmiş pseudoprognati vakalarında primer temas alanları ortadan kaldırılarak ideal oklüzyon tesis edilmesi özellikle engelli hastalarda estetik, fonksiyon ve fonasyonun sağlanması açısından kritiktir.
dc.description.abstractObjective:Prognathic anomalies can cause serious social and functional disorders. Therefore, aesthetic and functional rehabilitation of such anomalies is necessary for the psychological and dental rehabilitation of the patient. In this case, the prosthetic rehabilitation of a pseudoprognathic mentally retarded patient with malocclusion due to the parafunction was reported. Material and Method:A twenty-one-year old mentally retarded male patient referred to the Marmara University Faculty of Dentistry, Department of Prosthodontic with the complaint of disability in chewing. The extraoral examination revealed that the prognathic patient moved his lower and upper lips and jaw in all direction continously. The right maxillary central incisor in the palatal position was acted as a primer contact area and changed the occlusion from class I to class III. The right maxillary lateral and the left maxillary central insicor were missing. The fixed partial denture was planned to restore function and aesthetic of the patient. Root canal treatment was applied to the right maxillary central incisor due to excessive incisal reduction. After the preparation of number 11, the occlusion of the patient corrected to class I. Fixed partial metal ceramic restorations was constructed and cemented using glass ionomer cement (Ketac Cem, 3M ESPE, USA). Results: The pseudoprognatic patient’s occlusion was changed to class I occlusion by the elimination of primary contact.The patient has no functional, aesthetic and phonational complaint after 6 months follow-up period. Conclusion: Adjusting primary contact areas caused by pseudoprognathism to ensure ideal occlusion plays a crucial role for providing aesthetics, function and phonation in disabled patients.
