Publication: Tinnitusun psikoakustik ölçümünün farklı yöntemlerle değerlendirilmesi
Abstract
ÖZETAmaç: Sensörinöral tip işitme kayıplı bireylerde, odyometre ve tinnometre cihazı ile belirlenen tinnitus frekansı, şiddeti ve maskelenebilme özelliği arasında uyum olup olmadığını inceleyip karşılaştırmak ve tinnometre cihazının önerdiği maskenin rezidüel inhibisyon üzerine etkisinin olup olmadığını incelemektir.Gereç ve Yöntem: Sensörinöral işitme kayıplı tinnituslu 36 birey (18-65) tinnitus frekans ölçümü, tinnitus şiddet eşlemesi, minimal maskeleme seviyesi ve rezidüel inhibisyon testleri ile değerlendirilmiştir. Bu testlerin odyometre ve tinnometrede elde edilen sonuçları karşılaştırılmıştır. Analiz ve simülasyon için SPSS ve MATLAB araçları kullanılmıştır. İstatistiksel analiz için Tek Yönlü Tekrarlı Ölçümler ANOVA, Wilcoxon signed ranks testi ve Sınıf İçi Korelasyon Katsayısı (Intraclass Correlation Coefficient- ICC) kullanılmıştır.Bulgular: Tinnometre ve odyometredeki tinnitus frekansları arasında anlamlı fark gözlenmemiştir. Odyometrede dar bant gürültü kullanıldığında elde edilen tinnitus frekansı ile tinnometre tinnitus frekansı arasında anlamlı fark gözlenmemiştir (p>0,05). Odyometrede saf ses uyaran kullanıldığındaki tinnitus frekansı ile tinnometre tinnitus frekansı arasında anlamlı farklılık gözlenmiştir (p<0,05). Dik ve düz konfigürasyona sahip bireylerin tinnitus frekansları arasında anlamlı farklılık gözlenmiştir. Unilateral ile bilateral tinnituslu bireylerin tinnitus frekansları arasında anlamlı farklılık gözlenmemiştir (p>0,05). Tinnometre tinnitus şiddet eşlemesi 5,6± 4,3 dB SL, Odyometre tinnitus şiddet eşlemesi 5,2± 4,3 dB SL düzeyindedir. Tinnometre Minimal Maskeleme Seviyesi 4,8 dB SL ± 3,7 dB SL, Odyometre Minimal Maskeleme Seviyesi 5 dB SL ± 3,7 dB SL düzeyindedir.Sonuç: Tinnitusun klinik değerlendirmesinde her 2 cihaz birbirinin yerine kullanılabilir. Ancak tedavi ve rehabilitasyon için tinnitus takibinin tinnometreyle yapılması hasta memnuniyeti açısından faydalı olacaktır.
EVALUATION OF TINNITUS PSYCHOACOUSTIC MEASUREMENT BY DIFFERENT METHODSObjective: Determining whether there is a correlation between tinnitus frequency, severity and masking ability determined by tinnometer and audiometer in patients with tinnitus and sensorineural hearing loss. Analyzing the mask recommended by the tinnometer that whether it has an effect on residual inhibition.Materials and Methods: 36 individuals (18-65) with sensorineural hearing loss tinnitus were evaluated by tinnitus frequency measurement, tinnitus intensity matching, minimal masking level and residual inhibition tests. The results obtained from the audiometry and tinnometer were compared. SPSS and MATLAB tools were used for analysis and simulation. For the statistical analysis, ANOVA, Wilcoxon signed ranks test and Intraclass Correlation Coefficient (ICC) were used. Results: There was no significant difference observed between tinnitus frequencies in audiometer and tinnometer. There was no significant difference between tinnitus frequency and tinnometer tinnitus frequency when using narrow band noise in audiometer (p> 0.05). There was a significant difference between tinnitus frequency and tinnometer tinnitus frequency when using pure tone stimulus in audiometer (p <0.05). A significant difference was observed between the tinnitus frequencies of the individuals have different audiometric configurations. There was no significant difference between individuals with unilateral and bilateral tinnitus in terms of tinnitus frequency (p> 0.05). Tinnometer tinnitus severity match was found to be 5.6 ± 4.3 dB SL, audiometry tinnitus severity match was 5.2 ± 4.3 dB SL. Tinnometer Minimal Masking Level 4.8 dB SL ± 3.7 dB SL, Odyometer Minimal Masking Level is 5 dB SL ± 3.7 dB SL.Conclusion: In the clinical evaluation of tinnitus, each of the 2 devices can be used interchangeably. However, tinnitus follow-up with tinnometer will be beneficial for patient satisfaction for treatment and rehabilitation.
