Publication: The impact of catheter ablation on quality of life and medication adherence in patients with atrial fibrillation
Abstract
Atriyal fibrilasyon (AF) düzensiz kalp atışıyla karakterize yaşam kalitesini olumsuz
etkileyen ve en sık rastlanan aritmi çeşididir. AF’si olan hastalar için, tedavinin iki temel amacı,
semptom kontrolü sağlayarak yaşam kalitesini iyileştirmek ve tromboembolizm, inme ile
ilişkili morbidite ve mortaliteyi önlemektir. Semptom yönetimi, hız kontrolü ile başlar ve daha
sonra, uzun vadede ritim kontrolünün faydalarının değerlendirilmesine kadar uzanır. Katater
ablasyon (KA), ritim kontrolünde kullanılan bir yöntem olup, hastaların yaşam kalitesini
olumlu yönde etkilemektedir. Çalışmamızın amacı, AF tanısı olan hastalarda KAnın yaşam
kalitesine ve ilaç uyumuna olan etkisinin değerlendirilmesidir. Prospektif ve gözlemsel nitelikte
olan çalışmamız, Mayıs-Eylül 2022 tarihleri arasında İstanbul’da bir aritmi polikliniğinde
yürütülmüştür. Polikliniğe başvuran ve AF nedeniyle katater ablasyon planan 50 hasta çalımaya
dahil edilmiştir. İşlem öncesinde ve sonrasında yaşam kalitesi ve ilaç uyumunu değerlendiren
Sağlıkta Yaşam Kalitesi ve Ilaç Uyumunu Bildirim Ölçeği hastalara yöneltilmiştir. Çalışmaya
dahil edilen hastaların ilaç uyumu değerlendirildiğinde, ablasyondan sonraki ilaç uyumunu
bildirim ölçeği puan ortalamasının öncesine göre istatistiksel olarak anlamlı bir şekilde daha
yüksek olduğu tespit edilmiştir (p<0,001). Aynı şekilde sağlıkta yaşam kalitesi için yöneltilen
sorularda hastaların verdiği yanıtlara göre anlamlı bir şekilde iyileşme olduğu tespit edilmiştir
(p<0,001) Sonuç olarak katater ablasyon işleminin hastaların yaşam kalitesini ve mevcut diğer
tedavilerine olan uyumunu artırabileceği ortaya konmuştur.
Atrial fibrillation (AF) is the most common type of arrhythmia characterized by irregular heartbeat and negatively affecting quality of life. The two main goals of treatment of AF are to improve quality of life by providing symptom control and to prevent thromboembolism, stroke-related morbidity and mortality. Symptom management begins with rate control and then extends to evaluating the benefits of rhythm control over the long term. Catheter ablation (CA) is a method used in rhythm control and positively affects the quality of life of patients. The aim of our study is to evaluate the effect of KA on quality of life and medication adherence in patients with AF. This prospective and observational study was conducted on an arrhythmia polyclinic in Istanbul between May and September 2022. Fifty patients who applied to the outpatient clinic and were planned for catheter ablation due to AF were included in the study. Before and after the procedure, Medical Adherence Report Scale and the Health-Related Quality of Life questionnaire, which are the scales for reporting quality of life and drug compliance in health, were applied to the patients. It was determined that the mean score of the medication adherence after ablation was statistically significantly higher than before (p<0.001). Similarly, it was determined that there was a significant improvement in the quality of life of patients (p<0.001). As a result, it was revealed that the CA procedure could increase the quality of life and medication adherence in patients with AF.
