Hipertansiyonu olan hastaların öz bakım davranışları

Abstract

Amaç: Bu araştırmanın amacı hipertansiyonu olan hastaların öz bakım davranışlarının belirlenmesidir. Gereç ve Yöntem: Araştırmanın örneklemini İstanbul’da bir eğitim ve araştırma hastanesinin acil servisine ve kardiyoloji polikliniğine başvuran önceden hipertansiyon tanısı almış 250 hasta oluşturdu. Veriler ‘Hasta Tanılama Formu’ ve ‘Hipertansiyon Öz-Bakım Profili: Davranış Ölçeği (HÖBP-DÖ)’ kullanılarak toplandı. HÖBP-DÖ 20 maddeden oluşan 4’lü likert tipi bir ölçektir. Alınabilecek en yüksek toplam puan 80, en düşük puan 20’dir. Ölçekte toplam puanın artması daha iyi bir öz bakım uygulamasını temsil etmektedir. Verilerin analizinde istatistiksel anlamlılık p<0,05 olarak kabul edildi. Bulgular: Araştırmaya katılan hastaların yaş ortalaması 62,86±8,11 idi. Hastaların çoğunluğu kadın (%67,2) idi. Hastaların ortalama sistolik kan basıncı değeri 111,16±50,33; diyastolik kan basıncı değeri 65,79±30,11 idi. Hastaların öz bakım davranış puanlarının ortalaması 54,82±5,41 ve ölçeğin güvenirlik katsayısı 0,792 olarak bulundu. Hastaların yaşına, medeni durumuna, çalışma durumuna, birlikte yaşadığı kişilere, hiperlipidemi varlığına, ilaçlarını düzenli kullanma, düzenli sağlık kontrolüne gitme, kan basıncını kontrol altında tutma durumuna, hekim başvuru sıklığına, sigara ve alkolü bırakmanın hastalık yönetiminde faydalı olduğunu düşünme durumlarına göre öz bakım davranış puan ortalamalarında fark bulundu. Sonuç: Hipertansiyonu olan hastaların öz bakım davranışları orta düzeyde bulundu. Kan basıncı kontrolünde ve hipertansiyona bağlı komplikasyonların yönetiminde öz bakım davranışlarının sürekliliği oldukça önemlidir. Hemşireler hasta takibinin sürekliliğini sağlamalıdır.

Aim: The aim of this study is to determine the self care behaviors of patients who has hypertension. Material and Method: The sample of the study has been consisted of 250 patients having previous diagnosis of hypertension who applied to the emergency department and cardiology clinic of a training and research hospital in Istanbul. Data were collected by using ‘Patient Diagnosis Form’ and ‘The Behavioral Scale of Hypertension Self-Care Profile (HBP-SCP)’ is a 4-point likert-type scale consisted of 20 items. The highest total score might be 80, the lowest score might be 20. An increase in the total score on the scale represents a better self care implementation. Statistical significance was accepted as p<0,05. Result: The mean age of the patients participating in the study was 62,86±8,11. The majority of the patients were women (67,2%). The average of systolic blood pressure value of the patients was 111,16±50,33; diastolic blood pressure value was 65,79±30,11. The average of self care behavior scores of patients was 54,82±5,41, the reliability index of the scale was 0,792. It was observed that the self care behavior average scores differed according to age, marital status of patients, working condition, the people they live with, the presence of hyperlipidemia, regular use of medications, regular health check-ups, keeping blood pressure under control, frequency of doctor visits, thinking the benefits of smoking and alcohol cessation in disease management. Conclusion: It was found that the self care behaviors of the patients who have hypertension were on medium level. Continuity of self care behaviors is very important in the control of blood pressure and in the management of complications related to hypertension. Nurses should ensure the continuity of patient follow-up.
