Publication: Meme kanseri cerrahisi geçiren hastalarda hemşirelikte vaka yönetimi sürecinin ameliyat sonrası iyileşme ve yaşam kalitesine etkisi
Abstract
Amaç: Bu araştırma, meme kanseri nedeniyle cerrahi müdahale geçiren kadın hastalarda hemşirelikte vaka yönetimi uygulamasının ameliyat sonrası iyileşme düzeyi ve yaşam kalitesi üzerindeki etkilerini değerlendirmeyi amaçlamaktadır. Gereç ve Yöntem: Bu tanımlayıcı ve ilişki arayıcı çalışma, 01 Kasım 2023 - 30 Nisan 2024 tarihleri arasında İstanbul’daki bir özel hastanenin genel cerrahi kliniğinde yürütüldü. Araştırmanın evrenini meme kanseri nedeniyle cerrahi müdahale planlanan tüm hastalar hedef alıp, örneklem 78 gönüllü hastadan oluşturuldu. Veriler; “Hasta Tanılama Formu”, “Ameliyat Sonrası İyileşme İndeksi (ASİİ)” ve “Meme Kanseri Tedavisinin Fonksiyonel Değerlendirilmesi Ölçeği (FACT-B)” aracılığıyla yüz yüze görüşmelerle toplandı. Veriler SPSS programında analiz edilmiş; uygun istatistiksel testler kullanılarak p<0,05 anlamlılık düzeyi kabul edildi. Araştırma için etik kurul ve kurum izinleri alınmıştır. Bulgular: Çalışmaya katılan 78 kadının yaş ortalaması 51,2±9,12 yıl olup, katılımcıların çoğunluğu evli, ev hanımı ve çocuk sahibidir. Ameliyat Sonrası İyileşme İndeksi (ASİİ) ve Meme Kanseri Tedavisinin Fonksiyonel Değerlendirilmesi (FACT-B) puanları sırasıyla 2,01±0,22 ve 69,60±6,08 olarak belirlendi. Lineer regresyon analizi, ASİİ ve FACT-B toplam puanlarını etkileyen değişkenlerin sırasıyla %39,9 ve %41,5’ini açıkladığını gösterdi (p < 0,001). Çalışma sonucunda ASİİ ve FACT-B orta düzeyde etkilendiği görüldü. Sonuç: Sonuç olarak, tez bulguları meme kanseri cerrahisi geçiren hastalarda hemşirelik vaka yönetimi sürecinin, ameliyat sonrası iyileşme ve yaşam kalitesine olumlu etkileri olduğunu göstermektedir.
Objective: This study aims to evaluate the effects of case management practice in nursing on postoperative recovery level and quality of life in female patients undergoing surgery for breast cancer. Materials and Methods: This descriptive and correlationalstudy was conducted in the generalsurgery clinic of a private hospital in Istanbul between November 01, 2023 and April 30, 2024. The population of the study included all patients who were planned to undergo surgery for breast cancer, and the sample consisted of 78 volunteer patients. Data were collected through face-to-face interviews using the “Patient Identification Form”, “Postoperative Recovery Index (PBRI)” and “Functional Assessment of Breast Cancer Treatment Scale (FACT-B)”. Data were analyzed in the SPSS program; p<0.05 significance level was accepted using appropriate statistical tests. Ethics committee and institutional permissions were obtained for the study. Results: The mean age of the 78 women who participated in the study was 51.2±9.12 years and the majority of the participants were married, housewives and had children. Postoperative Recovery Index (PRI) and Functional Assessment of Breast Cancer Treatment (FACT-B) scores were 2.01±0.22 and 69.60±6.08, respectively. Linear regression analysis showed that 39.9% and 41.5% of the variables affecting the total scores of ASII and FACT-B, respectively (p < 0.001). As a result of the study, ASII and FACT-B were moderately affected. Conclusion: In conclusion, the findings of this thesis show that the nursing case management process has positive effects on postoperative recovery and quality of life in patients undergoing breast cancer surgery
