İnfertil çiftlerde evlilik uyumu ve depresif durumun in vitro fertilizason-embriyo transferi (IVF-ET) sonuçlarına etkisi

Abstract

Bu çalışma; infertil çiftlerde evlilik uyumu ve infertiliteye bağlı yaşanan depresif durumunun, In Vitro Fertilizasyon (IVF) ve Embriyo Transferi (ET) sonuçlarına etkisini belirlemek amacıyla tanımlayıcı, kesitsel ve karşılaştırmalı olarak planlanıp uygulanmıştır. Çalışma; Sağlık Bakanlığına bağlı bir Eğitim ve Araştırma Hastanesinin Üremeye Yardımcı Tedavi Merkezine IVF-ET tedavisi için başvuran çiftlerde gerçekleştirilmiştir. Çiftler infertilite nedenine göre, her grupta 35 çift olmak üzere, Kadın Faktörlü İnfertilite Grubu (KFİ), Erkek Faktörlü İnfertilite Grubu (EFİ) ve Nedeni Bilinmeyen İnfertilite Grubu (NBİ) olmak üzere 3 gruba ayrılmıştır. Verilerin toplanmasında; 30 sorudan oluşan Vaka Tanılama Formu, 32 maddelik Çift Uyum Ölçeği (ÇUÖ), 21 maddelik Beck Depresyon Envanteri (BDE), tedavi sürecini ve tedavinin sonucunu takip etmek amacıyla Vaka Takip ve Tedavi Sonucunu Değerlendirme Formu kullanılmıştır. Verilerin toplanması iki aşamada, araştırmacı tarafından gerçekleştirilmiştir. Elde edilen veriler, SPSS programında uygun istatistiksel yöntemlerle değerlendirilmiştir. Bulgularımızda; kadınların 31-35 yaş arasında dağılım gösterdiği ve yaş ortalamasının 31.67±4.83 olduğu, erkeklerde ise çoğunluğun 36 yaş ve üzerinde olup yaş ortalamasının 35.12±5.10 olduğu, her üç grupta da kadın ve erkeklerde ilkokul mezunu olanların, çoğunlukta olduğu saptanmıştır. İnfertilite faktörüne göre gruplara ayrılan çiftlerin, ÇUÖ toplam puan ortalamalarının birbirine çok yakın olduğu, ancak BDE toplam puan ortalamasının üç grupta da kadınlarda (özellikle KFİ grubundaki kadınlarda) erkeklerden daha yüksek olduğu bulunmuştur. ÇUÖ ve BDE toplam puan ortalamaları ile sağlıklı embriyo oluşumu, embriyo transferi yapılma ve gebelik oluşma durumu açısından istatistiksel olarak anlamlı ilişki olduğu (p<0.05) saptanmıştır. nfertilite, Evlilik Uyumu (ÇUÖ), Depresif Duygular (BDE)

THE EFFECT OF MARITAL ADJUSTMENT and DEPRESSIVE MOOD IN INFERTILE COUPLES ON THE RESULTS OF IN VITRO FERTILIZATION-EMBRYO TRANSFER (IVF-ET) This study was planned and carried out in a definitive, sectional and comparative manner in order to determine the effect of marital adjustment and depressive mood connected to infertility in infertile couples on the results of In Vitro Fertilization (IVF) and Embryo Transfer (ET). The study was carried out on the couples who applied to an Assistant Reproduction Treatment Center of a Training and Research Hospital affiliated to the Ministry of Health, for IVF – ET treatment. The couples were separated into 3 groups according to the reasons for infertility, as Infertility Group with Female Factor, Infertility Group with Male Factor and Infertility Group with Unknown Reasons and each group involved 35 couples. While collecting the data, Case Definition Form comprising 30 questions, Dyadic Adjustment Scale (DAS) of 32 articles, Beck Depression Inventory of 21 articles and the Form for Case Follow-up and Evaluation of Treatment Result aimed at monitoring the treatment process and the result of the treatment, were used. The data were collected in two phases by the researcher. The collected data were evaluated by appropriate statistical methods in the SPSS program. We determined in our findings that the women were in a range of age of 31 to 35 and the average age was 31.67±4.83 and the age of men was mostly 36 and over and the average age was 35.12±5.10 and elementary school graduates were in majority in all three groups. It was found that the DAS total point averages of the couples which were separated into groups according to infertility factor were quite similar however, BDI total point average was higher in women than men in all three groups (particularly in the women in the Infertility Group with Female Factor). It was determined that there was significant relation (p<0.05) between the DAS and BDI total point averages and the formation of healthy embryo, embryo transfer and the formation of pregnancy.
