Abstract
Doğum Eyleminde Sp6 Noktasına Uygulanan Basının Gebelerde Algılanan Doğum Ağrısına ve Doğum Eyleminin Süresine Etkisi ÖZET Araştırma, doğum eyleminde SP6 noktasına uygulanan basının (akupresürün) gebelerde algılanan doğum ağrısına ve doğum eyleminin süresine etkisini belirlemek amacıyla yapılmış randomize kontrollü deneysel bir çalışmadır. Trabzon Kadın Doğum Hastanesi’nde Temmuz 2009 – Nisan 2010 tarihleri arasında yapılan çalışmaya örneklem kriterlerine uyan 50 deney ve 50 kontrol grubu olmak üzere toplam 100 gebe alınmıştır. Verilerin değerlendirilmesinde; Ki-kare Testi, Fisher Ki-kare testi, Yatest Ki-kare testi, Mann Whitney U, Kruskal Wallis testi, ortalama, standart sapma, minimum, maksimum değerler, sayı ve yüzdelikler kullanılmıştır. Araştırmada gebelere eylemin latent fazında uygulama ile ilgili bilgilendirme yapılıp onay alındıktan sonra Gebe Tanıtım Formu uygulanmıştır. Her iki gruba eylem süresince standart hemşirelik bakımı verilmiş olup deney grubuna farklı olarak doğumun 1. evresinde SP6 noktasına servikal dilatasyon 2-3cm iken 15 kez (kontraksiyon süresince), 5-6cm iken 10 kez, 9-10cm iken 10 kez olmak üzere toplam 35 kez akupresür uygulanmıştır. Gebelerin ağrı algılamasını belirlemek için her uygulama sonunda Görsel Kıyaslama Ölçeği (GKÖ) kullanılmıştır. Erken postpartum dönemde ise her iki gruba Doğum Sonu Görüşme Formu ve GKÖ uygulanmış olup deney grubuna farklı olarak “Aküpresür Uygulamasına İlişkin Soru Formu uygulanmıştır. Doğum eyleminin süresi her iki grup içinde partograf ile ölçülmüş olup, servikal dilatasyon 3cm den tam servikal dilatasyona (10cm) kadar geçen süre ve tam servikal dilatasyondan fetüsün doğumuna kadar geçen süre olmak üzere iki zaman periyodunda değerlendirilmiştir. Araştırmada, doğum eyleminde SP6 noktasına uygulanan akupresürün doğum ağrısı algısını azaltmada etkisi olduğu ve eylem süresini yaklaşık bir saat kısalttığı ayrıca anne konforunu olumlu yönde etkilediği bulunmuştur (p<0.05). Akupresür uygulamasının doğum eyleminin süresini kısaltmada ve doğum ağrısının kontrol edilmesinde kullanılabileceği önerilmektedir. The research was a randomized controlled and experimental trial performed to determine the effect of pressure (acupressure) applied on SP6 acupoint during labor on labor pain perceived by the pregnant women and the labor length. 100 pregnant women 50 in the experimental group, 50 in the control group- having research criteria were included in the research between July 2009 and April 2010 at Trabzon Birth Hospital. Chi-square, Fisher Chi-square, Yatest Chi-square, Mann Whitney U, Kruskal Wallis tests, means, standard deviation, minimum and maximum, numeric and percentile values were used to analyze the data. Pregnancy Identification Forms were administered to the pregnant women after providing them with relevant information about the trial during the latent phase of the labor and their approvals were obtained. Both groups received standard nursing care during the labor and filled in Postpartum Interview Form and Visual Analog Scale (VAS) during the early post partum period. However, experimental group also filled in “Questionnaire Form about Acupressure Application” and they were applied acupressure 35 times in total on SP6 acupoint during the first phase of the labor: 15, 10, and 10 times while cervical dilatation was 2-3cm, 5-6cm and 9-10cm long respectively. VAS was used at the end of each acupressure application to determine pain perception. Labor length of groups was measured with partograph and cervical dilatation was assessed in two stages: from the time of 3cm dilatation to the time of full dilatation (10cm) and from the time of full dilatation to the time of delivery. The research showed that acupressure applied on SP6 acupoint was effective in decreasing labor pain perception and shortened labor nearly one hour and affected the comfort of the mothers positively (p<0.05).
