Serebral palsili çocuklarda osteopatik ev egzersiz programı ve geleneksel masajın konstipasyon üzerine etkisinin karşılaştırılması

Abstract

Amaç: Konstipasyonu olan serebral palsili (SP) çocuklardauygulanan osteopatik ev egzersiz geleneksel masajın konstipasyonu iyileştirmedeki etkisini araştırmak ve yöntemlerin etkinliğini birbirleri ile karşılaştırmaktır.Gereç ve Yöntem: Konstipasyonu olan ve bunun tedavisi için herhangi medikal ilaç kullanmayan 29 SP’li çocuk a) Evde uygulanan osteopatik egzersiz Geleneksel masaj grubu c) Kontrol grubu olarak rastgele olarak 3 gruba ayrıldı. Grupların demografik verileri, SP tipleri, Kaba Motor Fonksiyonel Sınıflama Sistemi (KMFSS), Çocuklar için Fonksiyonel Bağımsızlık Ölçeği (PFBÖ) kaydedildi. Tedavi öncesi ve sonrası Modifiye Konstipasyon Değerlendirme Skalası (MKDS), Roma III kriterleri, Bristol dışkı skalası (BDS)uygulandı.Bulgular: Olguların yaş ortalaması 12,2±3,76 olarak bulundu. Osteopatik grup ve gelenekselmasaj grubunda tedavi sonrası konstipasyon semptomlarında anlamlı iyileşme görüldü, (A grubu p=0,000014) (B grubu p=0,003) Bristol dışkı skalasındaki değişime göre sadece osteopati grubu anlamlı bulundu (p=0,026)MKDS alt parametreleri ayrı ayrı incelendiğinde osteopatik ev egzersiz daha üstün bulundu.Sonuçlar: Konstipasyonu iyileştirmede geleneksel masaj ve osteopatik ev egzersiz etkilidir. SP’li çocuklarda osteopatik ev egzersiz geleneksel masajdan daha hızlı ve etkili bulunmuştur. Osteopatik ev egzersiz serebral palside kabızlığın iyileştirilmesinde kullanılabilir.

Objective: The aim of this study was to investigate the effect of traditional massage and osteopathic home exercise program on the treatment of constipation in children with cerebral palsy (CP) and to compare the effectiveness of the methods.Materials and Methods: Twenty-nine children with CP who had constipation and did not use any medication for treatment were randomized and divided into 3 groups; a) Osteopathic exercise program at home b) Traditional massage group c) Control group. Demographic data of the groups, CP types, Gross Motor Functional Classification System (GMFCS), Functional Independence Scale for Children (WeeFIM) were recorded. Modified Constipation Assesment Scale (MCAS), Rome III criteria, Bristol stool scale (BDS) were examinedbefore and after treatment.Results: The mean age was 12.2 ± 3.76 years. Osteopathic group and traditional massage group showed significant improvement in constipation symptoms after treatment, (Group A p = 0.000014) (Group B p = 0.003) Only osteopathy group was found significant according to the change in Bristol stool scale (p = 0.026)Osteopathic home exercise program was found to be superior when MCAS sub-parameters were examined separately.Conclusions: Traditional massage and osteopathic home exercise program are useful in improving constipation. Osteopathic home exercise program in children with CP was found to be faster and more effective than traditional massage. Osteopathic home exercise program can be used to improve constipation in cerebral palsy.
