Publication: Metabolik sendromlu kadınlarda hemşire danışmanlığının metabolik sendrom temel bileşenlerini kontrol altına almada etkinliği
Abstract
1.ÖZET Bu araştırma; Erişkin Tedavi Paneli-ATP III tanı kriterlerine göre metabolik sendrom (MetS) tanısı alan kadınlarda, hemşirelik eğitimi ve bireysel danışmanlığın MetS temel bileşenleri üzerine etkisini değerlendirmek amacı ile randomize kontrol gruplu ve deneysel olarak yapıldı. Araştırma, İstanbul ilinde bulunan bir eğitim ve araştırma hastanesinin Obezite Polikliniğinde tedavi-takibi yapılan ve rastlantısal yöntem ile seçilen 60 deney ve 60 kontrol olmak üzere toplam 120 birey ile 25 Kasım 2010–28 Ekim 2011 tarihleri arasında yapıldı. Deney grubuna 8–10 kişilik gruplar şeklinde, bir hafta aralıklar ile üç eğitim oturumu yapıldı ve eğitim bitimini takiben ayda bir olmak üzere hastanın talebi doğrultusunda bireysel danışmanlık verildi. Kontrol grubuna ise bu süre içinde her hangi bir müdahalede bulunulmadı. Deney ve kontrol grubundaki bireyler ile yapılan ilk ve ilk izlemi takip eden 8-10 ay sonrası son izlemde vücut kompozisyonu, sistolik ve diyastolik kan basıncı ölçümleri yapıldı, hekim tarafından istemi yapılan biyokimyasal parametreleri kaydedildi. Diğer veriler, çalışmanın başında ve sonunda Hasta Tanıtım Formu, Sağlıklı Yaşam Biçimi Davranışları Ölçek II (SYBDÖ II), Fiziksel Aktivite Değerlendirme Anketi (FADA), Beslenme Takip Formu kullanılarak toplandı. Verilerin grup içi ve gruplar arası istatistiksel değerlendirilmesinde student t, Mann Whitney U, Wilcoxon, ki-kare testleri kullanıldı. Çalışma sonunda; MetS temel bileşenleri olan bel çevresi, açlık kan glikozu, sistolik ve diyastolik kan basıncı, trigliserid değerlerinde, deney grubunda istatistiksel olarak anlamlı düşüşler gözlenirken, kontrol grubunda anlamlı değişim gözlenmedi. HDL–K düzeyinde ise deney grubu son izleminde anlamlı yükselme saptandı (p<0.01). Bu bulgulara dayanarak; MetS tanısı alan kadınlarda hemşire danışmanlığının etkin bir yöntem olarak kullanılabileceği söylenebilir. Efficiency of Nurse Counseling on Getting Control of Main Components of Metabolic Syndrome in Women with Metabolic Syndrome This research was performed on women diagnosed with metabolic syndrome (MetS) according to Adult Treatment Panel-ATP III diagnosis criteria empirically and with randomize control group in order to evaluate the effects of nursing education and to evaluate the effects of performed individual consulting on MetS main components. The research was applied 25 Nov 2010-28 Oct 2011 on totally randomly chosen 120 person, 60 of them test, 60 of them control patients treated and followed-up in Obesity Policlinic of a training and research hospital in city of Istanbul. Three tutorial sessions was given to the experimental groups, each group consisting of 8-10 people every other week and patients were consulted individually upon the patients demand once a month following the termination of the tutorial. In the meantime the control group was not given any medical attention. Body composition, blood pressure were measured and biochemical parameters requested by the physician were registered for people in control and experimental group in the first meeting and in the last meeting following 8-10 months from the first meeting. The other data were collected in the beginning and in the end of the study by using Patient Diagnosis Form, Health Promotion Lifestyle Profile (HPLP II), Physical Activity Evaluation Survey, Nutrition Follow-up Form. Student t, Mann Whitney U, Wilcoxon, chi-square tests were used for the intergroup and intra-group statistical evaluation data. Statistically significant decreasing were monitored in the experimental group in evaluations of waist circumference, pre-prandial blood glucose, blood pressure, triglyceride which are main components of MetS conversely significant changes were not monitored in the control group. Significant increasing was detected in HDL cholesterol level in the last monitoring of the experimental group (p<0.01). On the basis of these findings; it is concluded that nurse counseling can be applied as an efficient method on women with MetS diagnosis.
