Abstract
OKUL DÖNEMİNDEKİ ÇOCUKLARDA SANAT TEMELLİ BİR HEMŞİRELİK GİRİŞİMİ ÖZET Hastalık ve hastanede yatma, çocuğun gelişimini olumsuz etkiler. Hastaneye yatan okul dönemi çocuğunun, bağımsızlığı, arkadaş ilişkileri, okul ve oyun aktiviteleri engellenmektedir. Hemşire, çocuğun ağrılı işlemler, stresli durumlar ile baş etmesini kolaylaştıran terapötik oyun tekniklerini kullanır. Bu araştırmanın amacı, hastanede yatan okul dönemi 8-12 yaş grubu çocuğa oyuncak yaptırılarak hastane süreciyle baş etmesine yardımcı olacak ve yaşam kalitesini arttıracak, sanat temelli bir hemşirelik girişiminin geliştirilmesidir. Araştırma 20 Haziran-17 Ağustos 2012 tarihleri arasında Hatay ilinde, Hatay Antakya Devlet Hastanesi’nde yapılmıştır. Araştırma karma tip (nitel ve nicel) yapılmıştır. Araştırmanın örneklemini, evrenden belirlenen, hastaneye yatma nedenine bakılmaksızın en az 3 gün hastanede yatan 8-12 yaş grubu 15 kontrol, 15 deney grubu olarak toplam 30 çocuk, 30 refakatçi, bakım veren 20 hemşire oluşturmuştur. Sanat temelli bir hemşirelik girişimi için materyal olarak, temiz tıbbi atıklar kullanılmıştır. Deney ve kontrol grubundaki çocuklara HHBAÖ, refakatçilere PSBMÖ uygulanmıştır. Çocuklar 104 malzeme kullanarak, 37 oyuncak yapmış, 43 farklı oyun oynamıştır. Nicel bulgularda, HHBAÖ ilk test puanları kontrol ve deney grubunda anlamlı değilken (p>0.05), son test puanları deney grubunda anlamlı bulunmuştur (p<0.000). Refakatçilerin PSBMÖ ilk test puanları kontrol ve deney grubunda anlamlı değilken (p>0.05), son test puanları deney grubunda anlamlı bulunmuştur (p<0.000). Nitel bulgularda, çocuk, hemşire ve refakatçiler için 8 tema bulunmuştur. Sanat temelli hemşirelik girişiminin, okul dönemi hastanede yatan çocukların hastanede ki yaşam kalitesini olumlu etkilediği, ebeveynlerin sağlık bakım memnuniyetini olumlu etkilediği ve hemşirelerin çocuğa bakım verirken kullanılabileceği etkin bir girişim olduğu sonucuna varılmıştır. An Art-Based Nursing Intervention for School Age Children Diseases and hospitalization affect the development of the child negatively. Hospitalization sets an obstacle to the independency, relations with friends, school and game activities of the school age child. A nurse uses therapeutic game techniques in order to ease child’s coping with painful processes and stressful situations. The aim of the study is to develop an art-based nursing intervention that help the child’s overcome hospitalization process and increase his quality of life by making the school age child, between the ages of 8-12, create new toys. The study was conducted between the dates of 20th June and 17th August, 2012. in Hatay Public Hospital. The study was conducted as mixed-type that is both qualitative and quantitive. The sample of the study consists of 30 children,15 of them for control and 15 of them for study groups, between the ages of 8-12 and hospitalized at least 3 days and 30 attendants and 20 nurses as caregivers. Clean medical waste was used as material for the art-based nursing intervention. HHBAÖ and PSBMÖ were applied to study and control group’s children and attendants as well. Children created 37 kinds of toys by using 104 different types of material and they also played 43 different types of games. As for quantitve findings; the first test scores of HHBAÖ were not significant for study and control groups (p>0.05) whereas the last test scores were significant study group (p<0.000). While attendants’ PSBMÖ first test scores were not significant for study and control groups (p>0.05) , the last test scores were found as significant for the study group (p<0.000). As for qualitative findings, 8 themes were found for the child, nurse and attendants. It was concluded that the art-based nursing intervention affect not only the quality of life of the school age child but also the health care satisfaction of the parents positively. Furthermore; it is an effective intervention for the nurses who care for the children. Hospitalization, Nursing Intervention, School Period, Therapeutic Game.
