Publication: Karaciğer transplantasyonunda hastaların ve ailelerin sorunları, gereksinimleri ve bakıma katılım düzeylerinin incelenmesi
Abstract
1. ÖZET Araştırma, karaciğer transplantasyonunda hastaların ve ailelerinin sorunları gereksinimlerini ve bakıma katılım düzeylerini belirlemek amacıyla tanımlayıcı olarak yapıldı. Çalışma 01.01.2000-09.06.2006 tarihleri arasında İstanbul Üniversitesi İstanbul Tıp Fakültesi Genel Cerrahi Karaciğer ve Safra Yolları Cerrahi Biriminde karaciğer transplantasyonu yapılmış 33 çocuk hastanın aileleri, 40 yetişkin hastanın aileleri ve 41 yetişkin hasta ile yapıldı. Araştırmada veriler, çocuk ailelerinin gereksinimleri, yetişkin hasta ailelerinin gereksinimleri, yetişkin hasta gereksinimleri olmak üzere, üç değişik anket formu kullanılarak toplandı. Araştırmada elde edilen bulgular değerlendirilirken, istatistiksel analizler için SPSS (Statistical Package for Social Sciences) for Windows 10.0 kullanıldı. Araştırma verileri değerlendirilirken tanımlayıcı istatistiksel metodların (Ortalama, Standart sapma) yanı sıra niteliksel verilerin karşılaştırılmasında Ki-Kare testi ve Fisher Exact Ki-Kare testi kullanıldı. Sonuçlar %95 güven aralığında, p<0,05 anlamlılık düzeyinde değerlendirildi. Çocuk hasta ailelerinin %60.6 (n=20)’sının maddi sorunlar, %42.4 (n=14)’ünün ilaç ve malzeme temini, %24.2 (n=8)’sinin kalacak yer, % 39.4 (n=13)’ünün bakımı üstlenecek kişilerin sayısının az olması nedenleriyle sıkıntı yaşadıkları, %42,4 (n=14)’ünün psikolojik desteğe gereksinimleri olduğu belirlendi. Hastaların %85.4 (n=35)’ünün eski sağlığına kavuşmayı beklediği, %68.3 (n=28)’ünün beklentilerine ulaştığı, belirlendi. Yetişkin hastaların %70.7 (n=29)’sinin karaciğer naklinden çok memnun, %26.8 (n=11)’inin memnun olduğu belirlendi. Yetişkin hasta ailelerinin %92.5 (n=37)’inin ilaç temini, %52.5 (n=21)’inin maddi, % 20 (n=8)’si kalacak yer problemi olduğu, %35 (n=14)’ inin ise psikolojik destek gereksinimi olduğu belirlendi. Problems And Needs Of The Liver Transplant Patients And Their Families And The Level Of Their Contribution To Patıent Care. The purpose of the study was to identify the problems and needs of the liver transplant patients and their families, and their contribution to the patient care. The Study covered 33 child patient families, 41 adult patients families and 40 adult patients, who had kidney transplant operations at the University of Istanbul, Faculty of Medicine General Surgery Department, Liver and Bile Duct Unit. Data was collected using three different question forms; needs of the child patient families, needs of the adult patient families and the needs of the adult patients. For the evaluation of data, statistical analysis was carried out using SPSS (Statistical Package for Social Sciences) for Windows 10.0. For data evaluation statistical methods (median, standard deviation) are used and to compare quantitative data Chi-squared test and Fisher exact Chi-squared tests are used. Results were considered meaningful at p<0,005, with a confidence level of 95%. It was identified that 60.6% (n=20) of the child patient families faced financial problems, 42.4% (n=14) had difficulty in supplying medicine and materials, 24.2% (n=8) had problems finding accomodation, 39.4% (n=13) had difficulty because of insufficient number of people to support care activities, and 42.4% (n=14) required psycological support.
