İsviçre genel sağlık sigortası sisteminin Türkiye ile karşılaştırılarak yeni bir model önerisi

Abstract

İnsanlar, toplum içerisinde var olmaya başladıklarından beri sosyal güvencearayışına girmişlerdir. Sosyal güvenlik kavramının doğuşundan beri sosyal güvenliksistemleri evrim geçirerek bugünkü haline gelmişler ve modern toplumun ihtiyaçlarınıkarşılamak için gelişmeye devam etmektedirler. Sosyal güvenliğin en önemli bölümü,insanların sağlıklarını koruma ihtiyacına hizmet eden genel sağlık sigortalarıdır.Günümüzde genel sağlık sigortalarının en temel sorunu sürdürülebilirliğinsağlanmasıdır. Çalışmada, Türkiye genel sağlık sigorta sistemi incelenerek,sürdürülebilirliği sağlayacak yeni bir model önerisi geliştirilmiştir.Çalışma kaynak taraması metoduyla yapılmış olup, tarama Türkiye veİsviçre’nin mevcut genel sağlık sigortası sistemleri ile sınırlandırılmıştır. Karşılaştırmaiçin İsviçre’nin seçilmesinin sebepleri; İsviçre ile Türkiye’nin GSYİH’lerinin yakınseviyede oluşu, Türkiye’nin geçirmekte olduğu nüfus değişimlerini ve yaşlanmayıİsviçre’nin 50 sene önce geçirmiş olması ve İsviçre’nin sürdürülebilir ve sağlıkhizmetlerinde yüksek sigortalı memnuniyeti sağlayan bir genel sağlık sigortası sistemioluşudur.Çalışmanın sonucunda Türkiye’nin genel sağlık sigortası sistemininsürdürülebilir olmadığı ve temelden değiştirmesi gerektiğine ulaşılmıştır. Türkiye’nin şuan içerisinde bulunduğu nüfus değişimlerini daha önceden geçirmiş olan ve Türkiye’ningenel sağlık sigortası sisteminde yaşayacağı finansman problemlerini daha öncedençözmüş olan İsviçre’den örnek alınarak, Türkiye’ye yeni bir genel sağlık sigortasısistemi önerilmiştir. Ülkeler arasındaki kişi başına düşen gelir farkları göz önünealınarak Türkiye’nin gelir seviyesine uygun oranlar ve limitler hesaplanmıştır.

Since the existence within a society, mankind has been in the search for asocial security. The social security systems have been evolving since the very birth oftheir notion and reached to today’s point where they still are developing in order to meetthe needs of the modern societies. The most important segment of social security is thegeneral health insurance which serves to the purpose of maintaining the healthy being ofthe people. Today, the fundamental challenge of the general health insurance schemes isthe sustainability. In this thesis, the general health insurance scheme in Turkey isstudied, and a new general health insurance model is developed which would providesustainability.The method of study is literature review and the scope of the thesis is limitedwith the general health insurance schemes of Turkey and Switzerland. Switzerland waschosen for comparison for the reasons of; the GDPs of Switzerland and Turkey are ofthe same level, Switzerland had been through the population changes and ageing 50years ago which Turkey is currently going through, and Switzerland has a sustainablegeneral health insurance scheme which results a high rate of satisfaction in terms ofhealthcare services.The study has concluded that the general health insurance scheme in Turkey isnot sustainable and required to be changed from the core. A new general healthinsurance scheme is proposed by taking Switzerland an example, which has alreadybeen through the population changes and solved the financing problems Turkey is goingthrough today. The applicable rates and limits are calculated considering the per capitaincome differences in between the countries.
