Abstract
1.ÖZET İnsanların en değerli varlığı kuşkusuz sağlıklarıdır. Bu nedenle, sağlıkta sıfır hatayı hedefleyen hasta ve çalışan güvenliğine odaklı sağlık bakım hizmeti sunumu zorunluluktur. Çalışma, 29 Nisan 2009 tarih 27214 sayılı Sağlık Kurum ve Kuruluşlarında Hasta ve Çalışan Güvenliğinin Sağlanması ve Korunmasına İlişkin Usul ve Esaslar Hakkında Tebliği’nin; hasta kimlik bilgilerinin tanımlanması ve doğrulanması, ilaç güvenliğinin sağlanması, hasta düşmelerinin önlenmesi, mavi kod uygulaması maddelerinin perinatal kliniklerde uygulanması için bir rehber oluşturmak ve oluşturulan bu rehber kapsamında kliniklerde alınan hasta güvenliği önlemlerinin, tıbbı hata oranlarına, hizmette kalite standartları denetimleri ‘hasta ve çalışan güvenliği’ alt başlığından alınan puana etkisini saptamak amacıyla tek gruplu klinik-deneysel olarak yapıldı. Araştırma Zeynep Kamil Kadın ve Çocuk Hastalıkları EAH’ de, gerekli izinler alındıktan sonra tek merkezli olarak Mayıs 2009- Temmuz 2011 tarihleri arasında gerçekleştirildi. Veri toplama araçları olarak; çalışmacılar tarafından geliştirilen Ebe- Hemşireleri Tanıtıcı Bilgi Formu, Hasta Güvenliği Önlemlerin Etkinlik ve İş Yükü Analizi Formu, ISO kapsamında hastane HKK tarafından oluşturulan Olay Bildirim Formu, Filiz (2009) ülkemize valide edilen Hasta Güvenliği Kültürü Hastane Anketi ve S.B. Strateji Daire Başkanlığı Hizmette Kalite Standartları Rehberi kullanıldı. Hasta güvenliği uygulamalarına ilişkin alınan önlemlerin, tıbbı hatalara ilişkin tutulan olay bildirim sayısını azatlığı (f:5.395, p:0.009), örneklemimizi oluşturan perinatal kliniklerde tutulan olay bildirim sayısının hastane geneline oranla azaltığı (t: -38.861, p:0.000) ve HKS denetimleri hasta güvenliği alt başlığından alınan puanı yükselttiği (f:7.008, p:0.001) saptandı. Çalışmamızda, yasal düzenlemelerin hasta güvenliği uygulamalarının kabulünü olumlu etkilediği ve HKS denetimleri hasta güvenliği puanını yükselttiği sonucuna varıldı. Sonuçlar doğrulturunda, ulusal hata bildirim raporlama sisteminin oluşturulması, kurumlarda hasta güvenliği kültürü ortamının desteklenmesi, HKS setinde “perinatal bakım” ın ayrı alt başlık olarak değerlendirilmesi önerildi. lıkta kalite, hasta güvenliği, yasal mevzuat, tıbbı hata, perinotoloji 1.
Adjusting The Patient Safety Cautions In Perinatal Clinics And Development Of Implementation’s Guides Health of people is their most valuable asset. Therefore, promoting the quality concept in health by the state ownership and supporting it by the legal regulations are useful. This study has been conducted in clinical-experimental type with a single-group in order to determine the impact of The Principles and Procedures of Providing and Protecting the Patient and Employee Safety in Health Organizations with the number 27214 and dated 29 April 2009 on the on the patient identification and verification, ensuring drug safety, patient fall prevention, creating a guide to implement the blue code in prenatal clinics to evaluate its effect on the patient safety measures in clinics, on the medical error rates and also on the points obtained by the sub-heading “patient and employee safety” of service quality standards’ controls. The research has been performed as single-center focused in Zeynep Kamil Women and Children's Diseases Education and Research Hospital after obtaining necessary permission. Data collection tools were midwife-nurses Introductory Information Form, the Event Notification Form, the Patient Safety Measures’ Effectiveness and Workload Analysis Form, Hospital Patient Safety Culture Survey, Service Quality Standards Guide. We found that the measures taken on the implementation of patient safety decrease the number of notification events held on medical errors (f: 5,395, p = 0.009), and also decrease the number of notification events held in perinatal clinics which are forming our sample than the hospital in general (t = -38,861, p = 0.000) and raises the patient safety sub-title points in HQS controls of (f: 7,008, p = 0.001). In our study, we also found that the legal regulations have a positive impact on the adoption of patient safety practices and raises the patient safety sub-title points in HQS controls. Regarding our findings, we proposed that, national error reporting system, should be organised, patient safety culture environment in the institutions should be supported and perinatal care should be considered as a separate heading in the HKS kit. quality in health, patient safety, legal legislation, medical error, perinatology
