Primipar annelerin emzirme motivasyonları ve etkileyen faktörler

Abstract

Amaç: Bu çalışma Primipar Emzirme Motivasyon Ölçeği’nin primipar anneler içinTürkçe’ye uyarlaması yapılarak, emzirme motivasyonlarını etkileyen faktörleribelirlemek amacıyla yapılan metodolojik ve tanımyacı tipte bir çalışmadır.Gereç ve Yöntem: Araştırma Ümraniye Eğitim ve Araştırma Hastanesi KadınDoğum Jinekoloji Polikliniği’ne Aralık 2017- Aralık 2018 tarihleri arasında başvurankadınlarla yapılmış metodolojik ve tanımlayıcı tipte bir çalışmadır. Örneklembüyüklüğü evreni belli olan örneklem seçimi formülü ile hesaplanarak 400 kadınçalışmaya dahil edilmiştir. Verilerin toplanmasında literatür doğrultusundaaraştırmacı tarafından hazırlanan Tanıtıcı Bilgi Formu ile Primipar EmzirmeMotivasyon Ölçeği kullanıldı.Bulgular: Emzirme motivasyon ölçeği alt boyutlarından emzirmeye verilen değer altboyutu puan ortalamaları, üniversite mezunu olan annelerde daha yüksek(78,74±11,96)’ti (p<0.05). Emzirme motivasyon ölçeği alt boyutlarından ebe desteğialt boyutu puan ortalamaları, emzirmede eşinden, akrabalarından destek alanannelerde (18,82±7,59) istatistiksel anlamlı olarak daha yüksek olduğu saptanmıştır(p<0.05). Emzirme motivasyon ölçeği alt boyutlarından özetkililik alt boyutu puanortalamaları aile içinde fiziksel şiddete uğradığını belirten annelerde (35,00±9,01),kendini mutsuz depresif ve kaygılı hisseden annelerde (39,89±7,19) istatistikselanlamlı olarak daha düşük olduğu saptanmıştır (p<0.05). Emzirme motivasyon ölçeğialt boyutlarından ebe desteği alt boyutu puan ortalamaları aile içinde fiziksel şiddeteuğradığını belirten annelerde (14,08±6,47), kendini mutsuz depresif ve kaygılıhisseden annelerde (16,60±7,54) istatistiksel anlamlı olarak daha düşük olduğusaptanmıştır (p<0.05). Emzirme motivasyon ölçeği alt boyutlarından ebe desteği altboyutu puan ortalamaları emzirmeye ilişkin bilgi alan annelerde (19,73±7,23),emzirmede eşinden, akrabalarından destek alan annelerde (18,82±7,59) istatistikselanlamlı olarak daha yüksek olduğu saptanmıştır (p<0.05).Sonuçlar: Emzirme Motivasyon Ölçeği’nin Türkçe versiyonunun annelerinemzirme motivasyonunu belirlemede geçerli ve güvenilir bir araç olduğubelirlenmiştir. Primipar annelerde; öğrenim düzeyi, emzirme desteği alma durumu,depresyon riski ve kaygı durumu, aile içi şiddete maruz kalma, emzirmede ebedesteği ve özetkililik algısı ile emzirme motivasyonu ilişkili bulunmuştur.

Objective: This study is a methodological and descriptive study conducted todetermine the factors affecting the motivation of breastfeeding by adapting thePrimipar Breastfeeding Motivation Scale to Turkish for primiparous mothers.Materials and Methods: This is a methodological and descriptive study carried outwith the women admitted to Ümraniye Training and Research Hospital Obstetricsand Gynecology Clinic between December 2017 - December 2018. 400 women wereincluded in the study by calculating the sample size by the formula of choosing thesample for an obvious population. The Introductory Information Form prepared bythe researcher in accordance with the literature, and the Primiparous BreastfeedingMotivation Scale were used for the collection of data.Findings: The mean scores of the sub-dimension of value given to breastfeeding,one of the sub-dimensions of the breastfeeding motivation scale, were higher inmothers who were university graduates (78.74±11.96) (p<0.05). It was determinedthat the mean scores of the sub-dimension of midwifery support, one of the subdimensionsof the breastfeeding motivation scale, were statistically significantlyhigher in mothers who received support from their husbands and relatives inbreastfeeding (18.82 ± 7.59) (<0.05). It was determined that the mean scores of thesub-dimension of self-efficacy, one of the sub-dimensions of the breastfeedingmotivation scale, were statistically significantly lower in mothers who indicated thatthey were exposed to physical violence in the family (35.00 ± 9.01) and in motherswho felt unhappy, depressed and anxious (39.89 ± 7.19) (p<0.05). It was determinedthat the mean scores of the sub-dimension of midwifery support, one of the subdimensionsof the breastfeeding motivation scale, were statistically significantlylower in mothers who indicated that they were exposed to physical violence in thefamily (14.08±6.47) and in mothers who felt unhappy, depressed and anxious(16.60±7.54) (p<0.05). It was determined that the mean scores of the sub-dimensionof midwifery support, one of the sub-dimensions of the breastfeeding motivationscale, were statistically significantly higher in mothers who received informationabout breastfeeding (19.73±7.23) and in mothers who received support from theirhusbands and relatives in breastfeeding (18.82±7.59) (p<0.05),Conclusions: It was determined that the Turkish version of the BreastfeedingMotivation Scale was a valid and reliable tool in determining the breastfeedingmotivation of mothers. The level of education, state of receiving breastfeedingsupport, depression risk and anxiety state, exposure to violence in the family,midwifery support and self-efficacy perception in breastfeeding and breastfeedingmotivation were found to be associated in primiparous mothers.
