Anestezi sonrası bakım ünitesine (ASBÜ) alınan genel cerrahi hastalarının komplikasyon gelişimi açısından değerlendirilmesi

Abstract

ANESTEZİ SONRASI BAKIM ÜNİTESİNE (ASBÜ) ALINAN GENEL CERRAHİ HASTALARININ KOMPLİKASYON GELİŞİMİ AÇISINDAN DEĞERLENDİRİLMESİ Araştırma, genel cerrahi girişimi geçirmiş hastaların anestezi sonrası bakım ünitesinde izlenmesi, komplikasyonların belirlenmesi ve gelişen komplikasyonlardaki risk faktörlerini analiz etmek amacıyla prospektif çalışma olarak yapıldı. Araştırma evrenini İstanbul’da bir üniversite hastanesinin anestezi sonrası bakım ünitesine alınan hastalar oluşturdu. Evreni temsil edecek örneklem sayısı 230 hasta olarak belirlendi. Hastalar ameliyat öncesi ve anestezi sonrası bakım ünitesinde izlenmiş, yüz yüze görüşme yöntemi kullanılarak elde edilen veriler, araştırmacı tarafından hazırlanan “Hasta İzlem Formu”na kaydedilmiştir. Araştırmada; yaş, cinsiyet, kronik hastalık, beden kitle indeksi (BKİ), sigara ve alkol kullanma durumu, ASA sınıfı, cerrahi girişimin süresi, cerrahi girişimin türü, derlenme skoru, ASBÜ’nde komplikasyon gelişimi arasındaki ilişki incelendi. Verilerin değerlendirilmesinde yüzdelik, ortalama, ki kare ve t-testi kullanıldı. Araştırma sonunda, ağrı, hipotermi, hipoksemi, bulantı ve kusmanın en fazla görülen komplikasyonlar olduğu saptanmıştır. Komplikasyon gelişiminde cerrahi girişim türü, cerrahi girişim süresi ve yaş en önemli risk faktörleri olarak belirlenmiştir. Diğer risk faktörleri ile komplikasyon gelişimi arasında istatistiksel açıdan anlamlı bir farklılık olmadığı bulunmuştur. Komplikasyon gelişen hastaların ASBÜ’nde daha fazla kaldıkları, derlenme skorlarının daha düşük olduğu belirlenmiştir. ASBÜ’nde hastaların risk faktörlerinin belirlenmesi; komplikasyonların erken fark edilmesini kolaylaştıracak ve daha ciddi komplikasyonların oluşumunu önleyecektir. Komplikasyon, anestezi sonrası bakım ünitesi, genel cerrahi EVALUATION OF COMPLICATIONS AT POST ANESTHESİA CARE UNIT FOR PATIENTS AFTER GENERAL SURGERY The aim of this study was to follow up the patients after general surgery at post anesthesia care unit to determine complications and risk factors for them. The scope of this research covered the patients in post anesthesia care unit of a university hospital in Istanbul. The number of patients evaluated in this study was 230. A “Patient Follow up Form” was prepared by the researcher to collect data from the patients, with face to face interviewing, at pre and post anesthesia care units. In this study; risk factors such as age, sex, body mass index (BMI), chronic illnesses, smoking, alcohol consumption, ASA score, duration and type of surgery, hospitilization score were used to determine the relations with complications at post anesthesia care unit. All data were evaluated by using percentages, chi-square and t-test. Pain, hypothermia, hypoxemia, nausea and vomiting were found as most common complications. Type and duration of surgery and age were the most important risk factors for the development of the complications. Statistically, there was no relation of complications with other risk factors. Lower recovery scores were noted for patients with complications and these patients have stayed more at post anesthesia care unit. As a result, following the risk factors in post anesthesia care unit will help to notice complications early and prevent occurence of more severe complications.
