dc.description.abstractColumnar cell lesions of the breast are lesions that have slight or pronounced cytological and structural atypia originating from the dilated terminal ductal lobular unity. In this case report we have presented the case of a 48 year old woman with a 2 cm tumor on her left breast. The diagnosis of a fine needle aspiration biopsy of the tumor was "malignant epithelial cells". This was followed by a tru-cut biopsy which was reported as a fibroadenoma. Five days after the tru-cut biopsy, the patient underwent a lumpectomy and sentinel lymph node biopsy operation with the frozen section procedure and reported as cellular fibroadenoma versus benign philloides tumors with tumor negative sentinel lymph node. On microscopic examination of the lesion, a well delineated lesion was observed with the ducts dilated where the lumens were commonly open and surrounded with a pseudostratified low columnar epithelium. The case was diagnosed as an atypical columnar cell hyperplasia. This case is presented as it is not only a rare lesion of the breast, but also there are diagnostic difficulties in both the fine needle aspiration biopsy, tru-cut biopsy and frozen sections.
