Kişilerin sağlık hizmeti talebinde devlet, özel ve üniversite hastanelerine başvurmalarını etkileyen faktörlerin incelenmesi

Abstract

Bu çalışmada, kişilerin sağlık hizmeti taleplerinde devlet hastanesi, üniversite hastanesi ve özel hastaneye başvurmalarına etki eden faktörlerin belirlenmesi amaçlanmıştır. Bu amaçla 30 Eylül - 8 Kasım 2002 tarihleri arasında, bir üniversite hastanesi, bir devlet hastanesi ve bir özel hastanenin, dahiliye, genel cerrahi, göz hastalıkları ve KBB polikliniklerinde hastalara anket uygulanmıştır. Her bir hastaneden 250 olmak üzere toplam 750 hastaya uygulanan anket sonucunda hastaların bu hastanelere başvurularında etkili olabilecek sosyo-demografik, ekonomik, çevresel ve hastaneyle ilgili faktörler incelenmiştir. Araştırma sonucunda her üç hastanede de araştırmaya katılan hastaların, bu hastanelere başvurularında etkili olan en önemli faktör olarak bağlı bulundukları sağlık güvenlik kuruluşunun, bu hastanenin masraflarını karşılaması bulunmuştur. Üniversite hastanesinde araştırmaya katılan hastaların %56,8'i, devlet hastanesinde araştırmaya katılan hastaların %68,4'ü, özel hastanede araştırmaya katılan hastaların %52'si bu faktörü, başvurularında etkili olan en önemli üç neden içinde belirtmişlerdir. Bu oran araştırmaya katılan tüm hastalarda %59,1'dir. Hastanelere başvuruda etkili olan diğer faktörler ise sırasıyla; teşhis ve tetkik imkanlarının daha iyi olması, hastanenin oturduğu yere yakın olması, hekimlerin daha ilgili davranması ve daha fazla zaman ayırması, bir yakının ya da tanıdığın tavsiyesi, muayene olacağı hekimi tanıma, personelin daha ilgili davranması, kolay randevu alınabilmesi, bir hekimin tavsiyesi, muayene olmak için daha kısa süre bekleme, ücretlerin uygun olması ve sevktir. Kişilerin başvurdukları hastaneleri belirlerken etkilendikleri en önemli faktör bağlı bulunduğu sağlık güvenlik kuruluşu olduğundan, sağlık planlamacılarının ve sağlık güvenlik kuruluşlarının hizmet planlamalarında bu konuyu dikkate almaları gerekmektedir. This study aims to determine the factors that affect patients' choices in consulting public hospital, private hospital or university hospital for their health care demands. With this purpose, a questionnaire has been conducted with patients of an university hospital, a public hospital and a private hospital in internal diseases, general surgery, ophthalmology, and ENT (Ear, nose and throat) outpatient clinics, between the dates of 30 September - 8 November 2002. 250 in each hospital, 750 parents have been studied in total, and socio-demographic, economic, environmental and hospital-related factors that may have an impact on the patients' choices have been documented. It is found that, in all three cases, the most important factor concerning the patients' decision to consult a given hospital was whether the expenses of the hospital were covered by the social security/ health insurance firm they are related to. 56,8% of patients in the university hospital, 68,4% of the patients in the state hospital, and 52% of the patients in the private hospital stated this among three important factors determining their choice. The percentage was 59,1% of all the patients that contributed to the survey. Other factors that were found affective were, in respective order: better diagnosis/ examination opportunities, the hospital being close to where they live, the doctors being more concerned and dedicating more time, recommendations from relatives or friends, knowing the doctor personally, the staff being more concerned, ease in getting appointments, referrals/ recommendations from other physicians, less waiting time for an examination, lower expenses, and referrals. Since the most important factor that determines which hospitals people choose to consult is the health insurance/ social security firm they are related with, this information has to be taken into account by the strategic planners in health industry and these security/ insurance firms themselves, while they are making plans for future development.
