Abstract

Bu arastırma acil servislerde çalısan 322 hemsirenin tükenmislik ve is doyumları ile, bu parametrelerin yasam kalitesine etkisini belirlemek amacıyla yapılmıstır. Arastırmada veriler Hemsire Tanım Formu, Maslach Tükenmislik Ölçegi, Minnesota Doyum Ölçegi ve Türkçe Dünya Saglık Örgütü Yasam Kalitesi Ölçegi–Kısa Form ile toplanmıstır. Tükenmislik duygusal tükenme, duyarsızlasma ve kisisel basarı; is doyumu içsel, dıssal ve toplam doyum olarak degerlendirilmistir. Verilerin analizi SPSS-version 10 ile yapılmıstır. Acil hemsirelerinin %54,3’ü duygusal tükenmislik, %54,7’si duyarsızlasma ve %46’sı kisisel basarı noksanlıgı yasamaktadır. Yas arttıkça duyarsızlasma azalmaktadır; çocuk sahibi olmayanlar daha fazla duygusal tükenmislik; devlet hastanesinde çalısanlar her boyutta daha fazla tükenmislik; acil servislerde çalısmayı kendisi seçenler, her boyutta daha az tükenmislik yasamaktadır. Hemsirelerin içsel, dıssal ve toplam doyum puanları orta düzeyde olup, en fazla içsel doyum alınmaktadır. En fazla doyum saglanan alanlar hizmet, güvenlik, yetenek kullanma, yetki, ekip uyumu, basarı, moral; en az doyum saglanan alanlar çalısma sartları, ücret, yükselme/ terfi olanagı, takdir/ onore edilme ve bagımsızlıktır. Evli olmak içsel, dıssal ve toplam is doyumunu; çocuk sahibi olmak içsel ve toplam is doyumunu olumlu yönde etkilemektedir. Yönetici hemsirelerin; acil serviste çalısmayı kendisi seçenlerin is doyumları daha yüksektir. Duygusal tükenme ve duyarsızlasma arttıkça is doyumu ve yasam kalitesi azalmakta; kisisel basarı arttıkça is doyumu ve yasam kalitesi artmaktadır. İs doyumu ve yasam kalitesi arasında pozitif anlamlı iliski vardır.
Job Satisfaction, Burnout and Their Effects on Quality of Life Among Emergency Departments’ Nurses The objective of the study was to evaluate burnout and job satisfaction level and to explore their effects on quality of life (QOL). Sample was 322 nurses from emergency units. The survey consisted of demographic information, Maslach Burnout Inventory, Minnesota Satisfaction Questionnaire, and The World Health Organization Quality of Life Instrument- Brief Form. We evaluated burnout in three dimensions including emotional exhaustion, depersonalisation, and lack of personal accomplishment; job satisfaction in two dimensions including intrinsic and extrinsic, besides total. We analysed collected data with SPSSversion 10. The proportion of emotional exhaustion, depersonalisation and personal accomplishment were 54,3%, 54,7%, and 46% respectively. Depersonalisation was negatively correlated with age; emotional exhaustion was higher among nurses who do not have child; burnout was higher among nurses working at state hospitals, was lower among nurses self-chosing emergency department to work. Nurses’ job satisfaction level was moderate. Intrinsic satisfaction was higher than extrinsic satisfaction. Nurses were most satisfied with nature of social service, job security, ability utilization, authority, team interaction, personal achievement and morale values; least satisfied with working conditions, compensation, advancement, recognation, and independence. The factors effecting job satisfaction in a positive way were being married, having children, working at adminisrative position and being self-chosed emergency department to work. Emotional exhaustion and depersonalisation were negatively associated with job satisfaction and QOL, while personal accomplishment was positively associated with job satisfaction and QOL. There was a positive correlation between job stisfaction and quality of life.
