Publication: Uyku apne sendromlu hastalarda serum bakır ve çinko düzeylerinin belirlenmesi
Abstract
UYKU APNELİ HASTALARDA SERUM BAKIR VE ÇİNKO DÜZEYLERİNİN BELİRLENMESİ Obstrüktif Uyku Apne Sendromu (OUAS; Uyku Apne Sendromu=UAS) gece içinde sık tekrarlayan nefes durmaları (apne) ve azalmaları (hipopne), sonrasında oluşan oksijen desaturasyonu ve uyanıklık reaksiyonu ile karakterize bir hastalıktır. UAS’da gece içindeki oksijen değişimlerinin serbest radikal üretimini arttırarak oksidatif strese neden olduğu düşünülmektedir. UAS’ın kardiyovasküler hastalıkların morbiditesini arttırdığı bilinmektedir. Bu hastalıktaki kardiyovasküler etkilerin nedeni olarak artmış oksidatif stres yükü gösterilmektedir. Bu çalışmada UAS’lı hastalarda oksidatif stresin göstergesi olabilecek, antioksidan savunma mekanizmasının önemli yapı taşlarından Bakır (Cu) ve Çinko (Zn) elemenetleri serum düzeyleri araştırılmıştır. Çalışmaya alınan hastalar polisomnografi testinin sonucuna göre 4 gruba ayrılmıştır. Apne-Hipopne İndekslerine (AHI) göre 5’ten küçük olanlar sağlıklı kontrol, 5-14.9 hafif derecede UAS, 15-39.9 orta derecede UAS, 40 ve üstü ileri derecede UAS olarak kabul edilmiştir. Polisomnografi sonrası sabah aç karnına alınan venöz kan örnekleri -20ºC de saklanmıştır. Serum Cu ve Zn düzeyleri alevli atomik absosrbsiyon cihazında ölçülmüştür. Ayrıca tüm hastalarda açlık kan şekeri düzeyi , kan lipid profili (HDL, LDL, trigliserid, total kolestrol), üre ve kreatinin değerlendirmeye alınmıştır. Serum Cu ve Zn düzeyleri ile polisomnografik veriler Pearson korelasyonu ve one way ANOVA istatistik yöntemi ile karşılaştırılmıştır. Serum Cu seviyesinin tüm gruplarda anlamlı derecede değişmediği, serum Zn seviyesinin ise AHI arttıkça düşme eğilimde olduğu gözlenmiştir (r=-0.37, p=0.042). Bu durum ileri UAS hastalarının daha fazla oksidatif stres altında olduğunu düşündürmektedir. Angina pektoris’li hastalarda yapılan bir çalışmada yüksek oranda (50-300mg/ gün) Zn alınmasının hastalığın semptomlarını ortadan kaldırdığı belirtilmiştir. SERUM ZINC AND COPPER LEVEL DETERMINATION IN SLEEP APNEA PATIENTS Obstructive Sleep Apnea Syndrome (OSAS) is an ilness characterized by oxygen desaturation and awakweness follows from repetitive apnea and hipopnea during the night. It is thought that diversity of oxygen levels during night in OSAS cause oxidative stress with the increment of the free radical production. It is known that OSAS increases the morbidity of cardiovascular ilnesses. The reason of the cardiavascular effect of these ilness is explained to be the increased oxidative stress charge. In this study, serum levels of Copper (Cu) and Zinc (Zn) was investigated which are one of the most important elements of antioxidant defence mechanism. Patients to be taken into study were divided into 4 groups according to the result of polisomnography test. Depending on Apnea–Hipopnea Index (AHI), less than 5 were taken into account as healthy control, between 5-14.9 as mild OSAS, between 15-39.9 as moderate OSAS and above 40 as having severe OSAS. Blood samples taken after polisomnography are preserved in -20ºC. Levels of serum Cu and Zn are measured by using flame atomic absorption spectrophotometry device. Furthermore, glucose, blood lipid profile (HDL , LDL ,trigliserid , total cholestrol), urea and creatinin were taken into account in all patients. Serum Cu and Zn levels and polisomnographic results are compared with the Pearson correlation and one way ANOVA test. It is observed that Serum Cu levels don’t change significantly through all groups but Zn level has a tendency to decrease while AHI increase (r=-0.37, p=0.042). This result leads an argument of being much more under oxidative stress in high level OSAS patients. As it is mentioned in a study with Angina pectoris patients, having high dosage of Zn (50-300mg/ day) treatment of ilness symptoms.
