Abstract
ÖZETBEHÇET HASTALIĞINDA KRONİK BAKIM MODELİ KAPSAMINDA HASTA KAYNAKLI ÇIKTILARIN DEĞERLENDİRİLMESİÖğrencinin Adı: Elif Naz ÇAKIR Danışman: Prof. Dr. Gonca MUMCU lık Bilimleri Fakültesi, Sağlık Yönetimi Bölümü, Sağlık Yönetimi Ana Bilim Dalı Amaç: Bu araştırmanın amacı, Behçet hastalığında kronik bakım modeli kapsamında hasta kaynaklı ölçekleri incelemektir.Gereç ve Yöntem: Bu geriye dönük çalışmaya, 339 Behçet hastası (K/ E: 179/ 160, ort. yaş: 36,13±9,81) dahil edilmiştir. Veriler; sağlık hizmeti kullanımı, tedavi beklentisi, iş günü kaybı, iş ve günlük yaşam ile ilgili Work Productivity and Activity Impairment (WPAI) ölçeği, ağız sağlığı ile yaşam kalitesini incelemek için Oral Health Impact Profile-14 (OHIP-14), hastalık aktivitesine yönelik Behçet’s Syndrome Activity Score (BSAS) ve Mukokütan indeksi (MI) içeren yapılandırılmış bir form ile toplanmıştır.Bulgular: Araştırma grubunda, iş günü kaybı ile muayene sıklığının ilişkili olduğu belirlendi (r:0,45 p=0.000). Ayrıca, araştırma grubunda WPAI-günlük yaşam aktivitesinde azalma da gözlenmiştir. Diş hekimine muayene gidiş ile OHIP-14 puanı ilişkilidir (r:-0,25 p=0.000). Aracı analizde; oral ülserin aktif olmasının OHIP-14 puanının artmasına neden olduğu belirlenmiştir (p=0.0000). Aracı etki olarak tedavi beklentisinin olumsuz olması OHIP-14 puanının artışına neden olmaktadır (p=0.0001). Ayrıca yaş’ın ikinci aracı değişken olarak OHIP-14 puanının artışında etkili olduğu da gözlenmiştir (p=0.0053). Hastalık şiddetinin oral ülser ile yaş arasındaki ilişkide moderatör etkiye sahip olduğu da belirlenmiştir (p=0.0040).Sonuç: Kronik hastalık yönetiminde, iş günü kaybı ve günlük aktivitede azalmanın olduğu belirlenmiştir. Oral sağlıkla ilişkili yaşam kalitesi, oral ülser aktivitesi, tedavi beklentisi ve yaştan etkilendiği görülmüştür.

Data were collected by a structured questionnaire regarding use of healthcare services, treatment expectation, work-day loss, work productivity and activity impairment (WPAI) questionnaire, Oral Health Impact Profile-14 (OHIP-14) questionnaire for oral health related quality of life and disease activity indices regarding Behçet syndrome activity scale (BSAS) and Mucocutaneous index (MI).Results: Work-day loss that was associated with the number of medical visits (r:0,45 p=0.000) and daily activity impairement were also seen in the group. The duration for dental visit was corrrelated with OHIP-14 score (r: -0,25 p=0.000). In the mediation analysis; it was found that OHIP-14 score was increased by the presence of oral ulcer (p=0.0000). The negative expectation of the treatment as a mediator in the indirect path (p = 0.0001) and age as a second mediator was also associated with poor OHIP-14 score (p = 0.0053). Disease severity was a moderator in the relationship between presence of oral ulcer and age (p=0.0040).Conclusion: In chronic disease maanagement, work-day loss and daily activity impairement were seen. Oral health related quality of life was affected by oral ulcer activity, treatment expectation and age in the group.
