Abstract
1. ÖZET Cerrahisiz Periodontal Tedavinin Sigara İçen ve İçmeyen Periodontitisli Hastalarda Il-34, Rankl ve Osteoprotegerin Seviyeleri Üzerine Etkisi Öğrenci Adı: Volkan ARIKAN Danışman Adı: Prof. Dr. Başak DOĞAN Periodontoloji Amaç: Bu çalışmanın amacı, cerrahisiz periodontal tedavi (CPT) sonrası evre-III-derece-C periodontitisli hastalarda sigara içmenin klinik parametreler ile RANKL, OPG ve IL-34 serum ve tükürük seviyeleri üzerindeki etkisini değerlendirmektir. Gereç ve Yöntem: Kırk evre-III-derece-C periodontitis hastası (20 sigara içen ve 20 sigara içmeyen) ve 40 periodontal olarak sağlıklı (20 sigara içen ve 20 sigara içmeyen) toplam 80 birey dahil edildi. Periodontitis gruplarına CPT uygulandı. Başlangıçta tüm bireylerin, CPT sonrası 1. ve 3. aylarda tedavi gruplarının tüm klinik periodontal parametreleri kaydedilip, tükürük ile serum örnekleri toplandı. RANKL, OPG ve IL-34'ün tükürük ve serum seviyeleri ELISA ile analiz edildi. Bulgular: Başlangıçta sadece tüm ağız sondalama derinliği (SD) ve SD>5 mm olan bölgelerin yüzdesi sigara içen periodontitis hastalarında, içmeyenlere göre daha yüksekti (p<0.05). CPT'den sonra her iki periodontitis grubunda tüm periodontal parametrelerde anlamlı iyileşme gözlendi (p<0.001). CPT'den sonra, gingival indekste, sondalamada kanama ve başlangıçta SD≥5mm olan bölgelerdeki ortalama SD'deki azalma, sigara içenlerde, sigara içmeyen periodontitis hastalarına göre daha düşüktü (p>0.05). CPT sonrası tüm periodontitisli hastalarda sadece tükürük IL-34 seviyeleri sağlıklı bireylere göre daha yüksekti (p=0.001) ve her iki periodontitis grubunda azaldı (p<0.05). Ayrıca, başlangıçta serum RANKL seviyesi sigara içen periodontitis grubunda, sigara içmeyen hem periodontitis hem de sağlıklı gruba göre yüksek bulundu (p<0.05). Serum RANKL seviyeleri, her iki periodontitis grubunda da NSPT'den sonra herhangi bir değişiklik göstermedi. Sonuç: Sigara içenler CPT'ye daha az duyarlıdır ve tükürük IL-34, evre-III-derece-periodontitis için potansiyel bir inflamatuar belirteci olabilir. The Effect of Non-Surgical Periodontal Treatment on IL-34, RANKL and Osteoprotegerin Levels in Smokers and Non-Smokers with Periodontitis Student Name: Volkan ARIKAN Supervisor Name: Prof. Dr. Başak DOĞAN Department: Periodontology Objective: The purpose of this study was to evaluate the effect of smoking on clinical parameters and the serum and saliva levels of RANKL, OPG, and IL-34 in stage-III-grade-C periodontitis patients after non-surgical periodontal treatment (NSPT). Methods: A total of 80 subjects, 40 stage-III-grade-C periodontitis patients (20-smokers and 20-non-smokers) and 40 periodontally healthy individuals (20-smokers 20-non-smokers) were included. NSPT was performed for periodontitis patients. At baseline from all subjects, at 1 and 3 months from periodontitis patients all clinical periodontal parameters were recorded and both saliva and serum samples were collected. Saliva and serum levels of RANKL, OPG, and IL-34 were analyzed by ELISA. Results: At baseline only whole mouth probing depth (PD) and percent of sites with PD>5mm were higher in smokers than non-smoker periodontitis patients (p<0.05). All periodontal parameters significantly improved in both periodontitis groups after NSPT (p<0.001). After NSPT, the reduction in gingival index, bleeding on probing, and reduction of mean PD in sites initially PD≥5mm were lower in smokers than non-smoker periodontitis patients (p>0.05). Only saliva IL-34 levels were higher in all-periodontitis patients than healthy individuals (p=0.001) and decreased in both periodontitis groups after NSPT (p<0.05). Moreover, elevated serum RANKL level was detected in smokers compared to non-smoker periodontitis or healthy ones at baseline (p<0.05). Serum RANKL levels exhibited no change after NSPT in periodontitis groups. Conclusions: The smokers are less responsive to NSPT, and saliva IL-34 can be a potential inflammatory marker of stage-III-grade-C periodontitis.
