Publication: Gömülü mandibular üçüncü molar dişlerin çekiminden sonra uygulanan hyaluronik asit’in tükürük ve doku parametreleri üzerine etkisinin incelenmesi
Abstract
ene Cerrahisi ZET Amaç: Mandibular yirmi yaş diş çekimden sonra uygulanan hyaluronik asitin antienflamatuar ve antioksidan özelliklerini çeşitli biyokimyasal, histolojik ve sitolojik parametreler aracılığı ile tükürük ve erken dönem yumuşak doku iyileşmesi üzerine etkilerinin incelenmesidir. Gereç ve Yöntem: Bu çalışmaya mandibular yirmi yaş diş çekim endikasyonu olan kırk hasta dahil edildi. Hastalar yirmi tanesi deney grubu, yirmi tanesi kontrol grubu olacak şekilde randomize olarak iki gruba ayrıldı. Deney grubundaki hastalara diş çekiminden hemen sonra 0,2 ml 0,8 % hyaluronik asit uygulandı. Diş çekiminden hemen ardından ve bir hafta sonra doku ve tükürük örnekleri alındı. Antienflammatuar ve antioksidan etki histolojik olarak lökosit, fibrosis, anjiyogenezis ve ödem ölçülerek; sitolojik olarak kollajen, lökosit ve nekroz değerlendirilerek; biyokimyasal olarak GSH, LPO ve kollajen içeriği ölçülerek yapıldı. Bulgular: Histolojik ve sitolojik değerlendirmelerde hyaluronik asit uygulanan hastalarda bir hafta sonra istatististiksel olarak anlamlı bir biçimde daha az enflammatuar yanıt geliştiği gözlemlenmiştir (p<0,05). Oksidatif stresin değerlendirilmesi için karşılaştırılan biyokimyasal parametreler arasında istatistiksel olarak anlamlı bir fark bulunamamıştır (p>0,05). Sonuçlar: Hyaluronik asit antienflamatuar ve antiödematöz etki göstermiş fakat antioksidan etki göstermemiştir. Konu ile ilgili daha net sonuçların ortaya konulması için daha geniş hasta grubunda ve daha uzun dönem sonuçların değerlendirilmesi gerekmektedir. Purpose: The purpose of this study was to measure antienflammatory and antioxidant efficiency of hyaluronic acid application on salivary and tissue parameters after third molar extraction. Material and Methods: 40 patients included in this study. Patients randomly divided in two groups. 0,2 ml 0,8 % HA was applied immediate after third molar extraction within the HA group (n: 20). Nothing was applied to the control group (n: 20). Change in inflammatory reaction and oxidative stress was compared histologically with leucocyte, fibrosis, angiogenesis and edema parameters; cytologically with collagen, leucocyte and necrosis parameters; biochemically with GSH, LPO and collagen content parameters). Results were compared in between immediate after extraction and 1 week after surgery. Bivariate analyses were used to assess the differences between the HA and control groups for each study variable Results: There was statistically significant level of leucocyte infiltration and angiogenesis between groups . HA group showed less leucocyte infiltration and more angiogenesis then control group (p<0,05). There was no statistically significant difference of oxidative stress parameters between groups (p>0,05). Conclusion: Hyaluronic acid had anti-inflammatory and anti-edematous effect following M3 extraction. However, oxidative stress levels did not change.
