İşitme cihazı kullanımının temporal çözünürlük üzerine etkisi

Abstract

Amaç: Çalışmanın amacı unilateral ve bilateral işitme cihazı kullanımının temporal çözünürlük üzerine etkisini belirlemektir. İkincil amaç temporal çözünürlük ile gürültüde konuşmayı anlama performansı arasında ve bu testler ile bireyin sübjektif algısı arasındaki ilişkiyi ortaya koymaktır. Gereç ve Yöntem: Sensörinöral işitme kayıplı bilateral, unilateral işitme cihazı kullanan ve işitme cihazı kullanmayan olmak üzere 3 gruptaki 60 katılımcı Gürültüde Boşluk Tanıma (GBT) testi ve Türkçe Matriks Cümle (TMC) testi kullanılarak değerlendirildi. Tüm testler işitme cihazlı ve cihazsız olarak uygulandı. Katılımcıların sübjektif algısı Konuşma, Uzaysal Algı ve İşitme Kalitesi (KUİK) ölçeği ile değerlendirildi. Bulgular: GBT testinin yaklaşık eşik sonuçları gruplar arasında benzerdi. Ancak unilateral işitme cihazı kullanan grupta doğru yanıt yüzdesi bilateral işitme cihazı kullanan gruba kıyasla daha düşük bulundu. Unilateral işitme cihazı kullanan katılımcıların cihazsız koşulda sağ kulaktan elde edilen yaklaşık eşik ve doğru yanıt yüzdesi düşük elde edildi. TMC testi sonuçları bilateral işitme cihazı kullanıcılarında daha iyi bulundu. GBT testi ile TMC testi arasında ve bu testler ile sübjektif algı arasında tutarlı bir ilişki bulunmadı. Sonuç: Bilateral işitme cihazı kullananlarda gürültüde konuşmayı performansı daha iyidir. Tek taraflı işitme cihazı kullanımı GBT testinin doğru cevap yüzdesini olumsuz etkilemektedir. Bilateral işitme kaybında unilateral işitme cihazı kullanımı sağ kulakta olsa bile temporal çözünürlük etkilenmektedir. Bu testler arasında ilişkinin bulunmaması, bu değerlendirme yöntemlerinin birbirini tahmin etmede kullanılamayacağını göstermektedir.

Objective: The primary goal of our study was to determine the effects of using unilateral and bilateral hearing aids on temporal resolution. The other was to reveal the relationship between temporal resolution and speech understanding performance in noise, and between these tests and the subjective perception. Material and Methods: Sixty participants with sensorineural hearing loss in 3 groups which are bilateral, unilateral hearing aid users, and not using a hearing aid were evaluated with the Gaps In Noise (GIN) test and the Turkish Matrix Sentence Test (TURMatrix). All test were conducted with and without hearing aids. The Speech, Spatial and Quality of Hearing Scale (SSQ) was used for subjective perception. Results: The approximate threshold (A. th.) was similar between the groups, but the percentage of correct responses was found to be lower in the unilateral hearing aid group. Both the values of GIN obtained from the right ear without hearing aids were low in the unilateral group. TURMatrix results were better in bilateral hearing aid users. There was no consistent relationship between the GIN test and TURMatrix test and between these tests and SSQ. Conclusion: Bilateral hearing aids is advantageous for speech recognition in noise. Unilateral device use affected the percentage of correct responses, but not the A. th. Even if the unilateral hearing aid is used in the right ear in bilateral hearing loss affects the temporal resolution obtained especially in the right ear. These evaluation methods cannot be used to predict each other because of absence of relationship.
