We report the case of a 40-year-old woman whose chest radiograph had been known to be abnormal for 20 years who presented with dyspnea, cyanosis, and episodes of respiratory tract infection that had increased markedly during the preceding 10 months. The diagnosis of pulmonary microlithiasis suspected from radiographs was confirmed by transbronchial biopsy and bronchoalveolar lavage.
