Hekim ve hemşirelerin kanserli çocuklara yönelik palyatif bakım uygulamaları

Abstract

1. ÖZET Araştırma, hekim ve hemşirelerin palyatif bakıma yönelik bilgileri ile terminal dönemdeki çocuklara yönelik palyatif bakım uygulamalarının değerlendirilmesi amacı ile gerçekleştirilmiştir. Örnekleme, 8 hastanenin pediatrik onkoloji servis ve pediatri servislerinde çalışan 100 hekim ve hemşire alınmıştır. Veriler tanıtıcı bilgi formu ve Palyatif ve Yaşam Sonu Bakım İzlem Aracı ile toplanmıştır. Katılımcıların %68'i palyatif bakımı hastayı rahatlatmak ve yaşam kalitesini artırmak olarak tanımlamıştır. Katılımcıların %89’u çocuğun yaşam kalitesinin sürdürülmesinin, % 86’sı ağrı yönetiminin önemli olduğunu belirtmiştir. Katılımcılar terminal dönem hastalarda en çok karşılaştıkları semptomların %52 ile iştahsızlık, %41 ile ağrı, %33 ile depresyon olarak belirtmişlerdir. Katılımcıların %88’i terminal dönemde ağrısı olan çocuğa düzensiz bir şekilde opiod verdiklerini, %12’si tolerans gelişmesinden korktukları için opioid vermediklerini belirtmişlerdir. Katılımcıların % 76 ile % 93’ü arasının diğer semptomların yönetiminde doğru yaklaşım gösterdikleri belirlenmiştir. Katılımcılardan yalnızca % 19’u terminal dönem bakım becerilerinin yeterli olduğunu, % 82’si yaşam sonu bakım eğitimine gereksinim duyduklarını belirtmiştir. PALLIATIVE CARE PRACTICE OF PHYSICIANS AND NURSES FOR THE CHILDREN WITH CANCER Study was performed to evaluate the knowledge of the physicians and nurses for palliative care and palliative care practices of them for children with end-stage cancer. One hundred physicians and nurses working in pediatric oncology clinics and pediatric clinics of 8 hospitals were included into the sample. Data were collected with descriptive information sheet and Palliative and End-of-Life Care Devices. Sixty-eight percent of the participants described the palliative care as to relieve the patient and to improve the quality of his/ her life. Eighty-nine percent of the participants stated that maintaining the quality of life of the child was important and eighty-six of the participants stated that pain management was important. Participants indicated that the most common symptoms in end-stage patients encountered by them were loss of appetite, pain and depression, with a rate of 52%, 41% and 33%, respectively. While eighty-nine percent of the participants stated that they administered opioids irregularly in end-stage child with pain, 12% of the participants indicated that they did not administer opioids since they were afraid of tolerance development. It was determined that 76-93% of the participants showed correct approach in management of the other symptoms. Only 19% of the participants stated that their abilities in end-stage care were sufficient and 82% of them indicated that they needed for training in end-of-life care. Although palliative care practices of the participants are generally correct, training will be useful for providing exact information to all of the practitioners and to ensure correct application by all of them.
