PTCA sonrası kanama komplikasyonu ve sıklığını etkileyen faktörlerin değerlendirilmesi

Abstract

1. ÖZET Araştırma, perkütan translüminal koroner anjiyoplasti(PTCA) uygulanan hastalarda işlem sonrası gelişen kanama komplikasyonun ve sıklığını etkileyen faktörleri değerlendirilmesi amacıyla yapıldı. Çalışmaya Eylül 2006- Temmuz 2007 tarihleri arasında Kartal Koşuyolu Yüksek ihtisas Eğitim ve Araştırma Hastanesi Koroner Yoğun Bakım Ünitesine yatırılarak acil ya da elektif olarak PTCA uygulanan 173 hasta dahil edildi. Veriler, literatür bilgisi doğrultusunda geliştirilen “Bilgi formu” ve “PTCA Sonrası Kanama Değerlendirme Formu” kullanılarak yüzyüze görüşme yöntemiyle elde edildi. Verilerin değerlendirilmesinde tanımlayıcı istatistiksel metodların (Ortalama, Standart sapma, frekans) yanı sıra parametrik verilerin karşılaştırılmasında Student t test kullanıldı. Nonparametrik verilerin karşılaştırılmasında ise Ki-Kare testi kullanıldı. Araştırma grubumuza katılan hastaların %82.1’i erkekler oluşturdu. Grubun ortalama yaşı 53.8411.34 yıl olarak bulundu. Hastaların %58.4’ü orta öğretim mezunuydu. Araştırmamızda ileri yaş, kadın cinsiyet, hipertansif, diyabetik olan ve sheat çekimi öncesi ACT değeri yüksek olan hastalarda PTCA sonrası kanama görülme oranının daha fazla olduğu belirlendi. Araştırma sonucunda belirlenen risk faktörleri doğrultusunda risk azaltıcı stratejilerin geliştirilip uygulanması ve hemşirelerin daha aktif rol alacağı başarılı ekip çalışması ile PTCA sonrası kanama komplikasyonun önlenmesi veya sıklığının azaltılması mümkün gözükmektedir. EVALUATION OF POST-PTCA BLEEDING COMPLICATIONS AND ITS RELATED FACTORS This study has been done to evaluate the postprocedural bleeding complications and the factors affecting its frequency in patients undergoing percutaneous transluminal coronary angioplasty (PTCA). This study involved 173 patients that have been internalized and undergone emergent or elective PTCA in Kartal Koşuyolu Yüksek İhtisas Education and Research Hospital between the dates of 01.09.2006 and 01.07.2007. The data were collected with the method of face to face interview, by using the Data Questionnaire and the Form for Evaluation of the Post-PTCA Bleeding, which have been prepared in accordance with the literature. In statistical analysis of the data, besides the defining statistical methods (mean, standart deviation, frequency), student t test was used in the analysis of the parametric data. In the analysis of the nonparametric data, chi-square test was used. 82.1% of the participants were male. The mean age of the participants was 53.8411.34. 58,4% of the participants were graduated from the secondary education. In our study, advanced age, female gender, hypertension, diabetes mellitus and high ACT levels were found to be increasing the frequency of bleeding complications. With development and application of risk reducing strategies according to the risk factors determined in our study and with a successful team work in which the nurses would take more active roles, the prevention or the reduction of the frequency of the bleeding complications seems to be possible.
