ÖZET AÇIK KAPANIS OLGULARINDA ALISKANLIK KIRICI APAREY UYGULAMASINDAN ÖNCE, UYGULAMA SIRASINDA VE SONRASINDA DL BASINCINDA OLUSAN DEGSKLKLERN DEGERLENDRLMES Dil itimine sahip açık kapanıs bozuklugu olan çocuklarda sabit alıskanlık kırıcı uygulanması ile dil basıncında olusan degisiklikler diyafram tipi basınç ölçer ile degerlendirilmistir. Çalısma grubu 13, kontrol grubu 6 kisiden olusmustur. Çalısma grubundaki çocuklara alıskanlık kırıcı uygulanmıstır. Dil basınç ölçümleri üst ve alt santral kesiciler, üst 1. azı disi ve alıskanlık kırıcı apareyin üzerinden istirahat pozisyonu ve yutkunma esnasında yapılmıstır Ölçümler aparey agıza uygulanmadan önce, hemen sonra, uygulamayı takiben birinci hafta, 1. ay, 3. ay, 6. ay, 10. ay ve apareyin çıkarılmasını takiben 2. ayda ölçülmüstür. Kontrol grubunun ölçümleri baslangıçta, 10. ve 12. ayda gerçeklestirilmistir. Çalısma grubunda agız içi basınç dengesinde degisimler olmus ve açık kapanıs anlamlı derecede azalmıstır. Üst azı disinde istirahat dil basınçları baslangıç degerlerinden düsük kalırken, yutkunma basınçları 10 ay boyunca düsüs göstermis ve 10. ayda apareyin çıkarılmasını takiben baslangıç degerlerine dönmüstür. Üst santral kesici diste, istirahat ve yutkunma dil basınçları baslangıç degerlerinden düsük kalmıstır. Alt santral kesici diste istirahat dil basınçları baslangıç degerlerinden düsük kalmıs, yutkunma basınçlarında degisim gözlenmemistir. Alıskanlık kırıcı üzerinden yapılan ölçümlerde ise istirahat ve yutkunma dil basınçlarında anlamlı düsüsler gözlenmistir. Kontrol grubunda ise istirahat ve yutkunma dil basınçlarındaki degisimler anlamsızdır. Sabit alıskanlık kırıcıların 10 aylık uygulanması ve 2 aylık takibi sonucunda dilin uyguladıgı basınçlardaki degisiklik istatistiksel olarak anlamlı degildir. stirahat pozisyonundaki dil basınçları ise 12 ay sonunda baslangıç degerlerine dönmemis; düsük kalmıstır. Aygıt üzerinden yapılan basınç ölçümleri, dilin bu süre içinde alıskanlık kırıcı aygıtla olusan yeni duruma adapte oldugunu göstermektedir. 3 kapanıs, dil basıncı, basınç ölçer, alıskanlık kırıcı, dil adaptasyonu. 1.
Tongue pressure changes were evaluated before, during, and after crib appliance therapy, using diaphragm-type pressure transducer in mixed dentition open bite cases. Study group consisted of 13 patients, and control group consisted of 6 patients. Crib appliance was applied to the patients in study group. Tongue pressures were measured from the upper and lower central incisors, upper first molar and from the crib appliance itself during rest position and swallowing. Measurements were performed before the insertion, at the time of insertion, one week after the insertion, four weeks after the insertion and 3rd, 6th, and 10th months after the insertion and 2 months after the removal of the appliance. Measurements of the control group were performed at the beginning, 10th month, and 12th month. In study group, therapy resulted in a disturbance in intraoral pressure equilibrium and open bite value showed significant decreases. Resting tongue pressures on upper first molar remained lower than initial values while swallowing pressures showed a gradual decrease during 10-month treatment period, and returned to initial values after the appliance removal. Resting and swallowing pressures on upper central incisor remained lower than initial values. Resting pressure on lower central incisor remained lower than initial values and swallowing pressure remained the same. Resting and swallowing pressures on crib appliance showed significant decreases. In control group, resting and swallowing pressure changes were insignificant. Changes in tongue pressure at the end of 10-months crib therapy and 2 months follow-up period were insignificant. Resting tongue pressures remained lower than initial values.
