Histerektomili hastalarda benlik saygısının öz-bakım gücüne etkisi

Abstract

1. ÖZET Araştırma, histerektomili olguların benlik saygılarının öz-bakım güçlerine etkisini belirlemek amacıyla tanımlayıcı olarak gerçekleştirilmiştir. Araştırma, İstanbul İli Anadolu Yakasındaki en büyük Kadın Doğum-Çocuk Araştırma ve Uygulama Hastanesinde yapılmıştır. Örneklem büyüklüğü hesaplanırken Minitab kullanılmış, %5 hata payı ve %95 güven aralığında sonuç 118 bulunmuş ve araştırmaya 120 olgu alınmıştır. Olgulara postoperatif 2. günlerinde Hasta Tanılama Formu, Coopersmith Benlik Saygısı Ölçeği veBakım Gücü Ölçeği doldurulmuştur. Bulgular değerlendirilirken, SPSS for Windows kullanılmış, frekans, yüzde, ortalama, standart sapma, Cronbach Alfa Katsayıları, İndependent samples t test, One Way Anova testi, Tukey Post Hoc testi, Mann-Whitney U testi ile Pearson Korelasyon analizi kullanılmıştır. Olguların benlik saygısı puan ortalaması 59.937.03 bulunmuşken, öz-bakım gücü puan ortalaması 80.3111.78 bulunmuş, benlik saygısı ile öz-bakım gücü arasında pozitif yönlü anlamlı ilişki olduğu saptanmıştır (p<0.05). Olguların benlik saygısı ve öz-bakım gücü düzeyleri ile tanıtıcı özelliklerinin çoğu arasında anlamlı ilişki saptanmışken (p<0.05); egzersiz durumu, uyku düzeni, tıbbi tanı, özgeçmiş özellikleri, abortus durumları arasında ilişki saptanmamıştır (p>0.05). Öz-bakım gücü düzeyleri ile çalışma durumu ve alkol kullanma durumu arasında anlamlı ilişki saptanmış benlik saygısı düzeylerinde ilişki bulunamamıştır. Hemşirenin histerektomili olgularda benlik saygısının artmasına paralel olarak öz-bakım gücününde arttığını bilmesi, vereceği bakım ve eğitimin bu iki kavram ilişkisini göz önünde bulundurularak planlaması önemlidir. SUMMARY THE EFFECT OF THE SELF ESTEEM ON SELF-CARE AGENCY IN HYSTERECTOMY PATIENTS Research was descriptively performed to determine the effect of the self esteem over the self-care of cases with hysterectomy. The research made in the largest Gynecology – Children Research and Application Hospital of Anatolian Side of Istanbul. 120 patients were participated to the research. Minitab program was used in calculating the sample size and 118 results found in 5% margin of error and 95% confidence interval. Patient Assessment Form, Coopersmith Self-Esteem Scale and Self-Care Agency Scale filled to the cases in their second postoperative days. Results are evaluated by using SPSS for Windows program, frequency, percentage, average, standard deviation, Cronbach's Alpha Internal Consistency Coefficients, Independent samples t-test, One Way ANOVA test, Tukey's Post Hoc test, Mann-Whitney U test and Pearson Correlation Analysis was used. The mean self-esteem score of the cases was found 59.93 ± 7.03, the mean self-care agency score was found 80.31 ± 11.78 and a positive significant relationship were found between self-esteem and self-care agency (p<0.05). Significant relationship was detected between self-esteems, self-care agencies and most of the introductive characteristics of the cases, whereas no relationship detected between exercise statuses, sleep patterns, medical diagnosis’s, personal background characteristics and abortion cases. Significant relationship was detected between self-care agency via work status and alcohol use but no relation was found in self esteem levels. Having knowledge of the increase of self care agency and self esteem; and planning the given education and care considering the relationship between these two concepts is important for nurses. Therefore, the affect of this surgical intervention over self-esteem of patients can be held in minimum levels and the self-care activities can be performed independently as soon as possible.
