Abstract
ÖZETBir Üniversite Hastanesinde Bildirimi Yapılan Tıbbi Hata / Olayların “Dünya Sağlık Örgütü Hasta Güvenliği Uluslararası Sınıflandırması”na Göre DeğerlendirilmesiÖğrencinin Adı: Yasemin ASLANDanışmanı: Prof. Dr. Mehveş TARIMAnabilim Dalı: Sağlık YönetimiAmaç: Bu tezin amacı, bir üniversite hastanesinde bildirimi yapılan tıbbi hata / olayları “Dünya Sağlık Örgütü Hasta Güvenliği Uluslararası Sınıflandırması”na göre değerlendirerek üniversite hastanelerinde yaşanan tıbbi hata / olaylar hakkında bilgi sahibi olmak ve iyileştirme fırsatları yakalamaktır. Gereç ve Yöntem: Araştırma İstanbul Anadolu Yakası’nda yer alan bir üniversite hastanesinde elektronik olarak 01.01.2015-30.06.2017 tarihleri arasında bildirimi yapılan 1496 tıbbi hata / olayın DSÖ’nün Hasta Güvenliği Uluslararası Sınıflandırması temeline dayanan tanımlayıcı tipte retrospektif bir araştırmadır.Bulgular: Tıbbi hata / olayların %26,20’sinin klinik süreçler, %24,67’sının ilaçlar / iv sıvılar, %13,70’inin klinik yönetim, %10,90’ının hasta kazaları ile ilgili olduğu tespit edilmiştir. Sağlık bakımıyla ilişkili enfeksiyonlar kategorisinde herhangi bir tıbbi hata / olay bildirim yapılmadığı belirlenmiştir. Tıbbi hata / olayların %57,02’sinin hastaya zarar vermeyen, %21,99’unun hafif zarar veren, %20,12’sinin orta dereceli zarar veren ve %0,87’sinin ise şiddetli zarar veren kategoride yer aldığı tespit edilmiştir.Sonuçlar: Araştırma sonucunda; bildirimi en fazla yapılan tıbbi hata / olay türlerinin klinik yönetim, klinik süreçler, ilaçlar / IV sıvılar ve hasta kazaları olduğu tespit edilmiştir. Bildirimi yapılan tıbbi hata / olayların büyük çoğunluğunun hastaya zarar vermediği görülmüştür.
SUMMARYEvaluation of Medical Error / Incidents Notified in a University Hospital According to the “World Health Organization International Classification for Patient Safety”Student’s Name: Yasemin ASLANAdvisor: Prof. Dr. Mehveş TARIMDepartment: Health ManagementObjective: The aim of this thesis is to evaluate the medical errors / incidents reported in a university hospital according to the World Health Organization International Classification for Patient Safety and to have information about the medical errors / incidents experienced in university hospitals and to have the opportunity to improve them.Materials and Methods: This is a descriptive retrospective study of 1496 medical errors / incidents reported electronically between 01.01.2015 and 30.06.2017 in a university hospital located on the Asian side of Istanbul.Results: It was found that 26.20% of medical errors / incidents were related to clinical processes, 24.67% were related to medication / iv fluids, 13,70% were related to clinical administration and 10,90% were related to patient accidents. No medical errors / incidents were reported in the healthcare related infections category. It was found that 57,02% of the medical errors / incidents were in the category of no harm, 1,99% were mildly harm, 20,12% were moderate harm, and 87% were severe harm to the patients.Conclusion: As a result of the research the most reported medical error / incident types were clinical management, clinical processes, medication / IV fluids and patient accidents. The majority of reported medical errors / incidents did not harm to the patients.
