Publication: Ameliyathanede hasta güvenliğinin incelenmesi
Abstract
AMELİYATHANEDE HASTA GÜVENLİĞİNİN İNCELENMESİ BURCU ÖĞÜN ÖZET Bu araştırma, ameliyathanede hasta güvenliğini sağlamaya yönelik yapılan uygulamaları belirlemek amacıyla tanımlayıcı olarak yapıldı. Araştırmanın evrenini ve örneklemi; İstanbul’daki Eğitim ve Araştırma Hastaneleri’nin genel ameliyathaneleri oluşturdu. Araştırmada veriler, Ameliyathane Genel Özellikleri Anket Formu ve Ameliyathanede Hasta Güvenliği Hedefleri Kontrol Listesi ile toplandı. Bulguların değerlendirilmesinde yüzdelik dağılımı kullanıldı. Araştırma sonucunda ameliyathanelerin %4.8’inde hastalar için kol bandı kullanılmadığı, cerrahi bölgeye taraf ve yer işareti uygulanmadığı ve cerrah tarafından işaretleme yapılırken hastanın katılımının sağlanmadığı, %19’unda işaretlemenin serviste yapılmadığı, %9.5’inde taraf cerrahisini içeren cerrahi invazif girişim formu oluşturulmadığı, ayrıca %71.5’inde sedyeden ameliyat masasına hasta transferinin ameliyatta kullanılan yeşil örtüler yardımıyla yapıldığı, %28.6’sında kimlik tanılamada yatak numarasının da kullanıldığı, %33.3’ünde steril alana verilen ilaçların şişelerinin ameliyat sonuna kadar saklanmadığı görüldü. Ameliyathanelerin tümünde yangın söndürme tüpleri bulunduğu, ekip içi ve ekipler arası iletişimde sorun yaşanmadığı, yüksek riskli hastaların tüm çalışanlar tarafından bilindiği, hastaya uygun ameliyat pozisyonu verilirken bası alanlarının pozisyon ekipmanlarıyla desteklendiği belirlendi. Bu sonuçlara göre ameliyathanede hasta güvenliğini sağlamaya yönelik uygulamaların/ hasta güvenliği hedeflerinin çoğunun hastanelerin tümünde gerçekleştirildiği ancak yapılmayan uygulamaların da azımsanamayacak ölçüde hasta güvenliğini tehdit edebileceği söylenebilir. This study was done descriptively to determine the practices for providing patient safety in the operating room. The study environment and the sample group consisted of the general operating room of Training and Research Hospitals in İstanbul. The data were collected with Questionnaire Form of The Operating Room General Characteristic and Check List of Patient Safety Goals in Operating Room. In the assesment of the study percentages were used. In this study result; it is found out that 4.8 % of the operating rooms weren’t used wrist band for identfy the patient, side and site marking for operative site and the surgeon weren’t involved the patient in the marking process, 19 % of them weren’t marked the site in surgery department, 9.5 % of them weren’t used a check list to verify that all documents and equipment needed for surgery side and site marking, 71.5 % of them were transferred the patient from stretcher to operating room bed with surgical drapes, 28 % of them were used bad or room number for identfy the patient, 33.3 % of them weren’t saved the medicine case which were used in sterile field to operation ending. It is found out that all of the operating rooms were have fire tubes and have no problems in communication of operating room team members and all of the operating rooms staff were known about high risk patients and for positioning the patient all of them were used suitable position equipment. According to the results, it can be concluded that all of the hospitals were done most of the patient safety goals/ practices for providing patient safety in the operating room but the other part of practices which weren’t applied, can threaten patient safety in the operating room.
