Although the most common cause of visual impairment in diabetic patients is diabetic macular edema, the pathogenesis is still not fully understood. A full understanding of physiological mechanisms would allow sensible combination of treatment options. In this manuscript changes in blood retina barrier, effects of vitreoretinal interface and functional proteins having effects on diabetic macular edema, especially vascular endothelial growth factor, are being reviewed.
