Abstract
Kardiyak Operasyon Geçiren Bireylere Tele Hemşirelik Yoluyla Uygulanan Transteoretik Model Temelli Girişimlerin Sigara Bırakmaya Etkisi Öğrenci Adı: Elif KARAKAN Danışman Adı: Prof. Dr. Saime EROL Hemşirelik Amaç: Çalışmanın amacı, kardiyak operasyon geçiren bireylere tele hemşirelik yoluyla uygulanan Transteoretik Model temelli girişimlerin sigara bırakmaya etkisini belirlemektir. Yöntem: Çalışma, yarı deneysel tasarım ile İstanbul’da bir kalp hastanesinde (Şubat-Kasım 2021) yürütüldü. Örneklemi, 60 deney, 60 kontrol, olmak üzere 120 kardiyak operasyon geçiren yetişkin birey oluşturdu. Deney grubuna üç ay boyunca, tele hemşirelik ile transteoretik model temelli sigara bırakma girişimleri uygulandı. Veriler, sosyo-demografik tanılama, Fagerström nikotin bağımlılık, transteoretik model öz etkililik ve karar alma ölçekleri kullanılarak toplandı. Verilerin analizinde, tanımlayıcı istatistikler, Mann Whitney-U testi, Kruskal Wallis varyans analizi kullanıldı. Bulgular: Girişim sonrası, deney grubunda sigara bırakma oranı %62,75, kontrol grubunda %26 bulundu (p<0,05). Deney grubunun öz etkililik ölçeği kilo kontrolü hariç diğer alt boyut puanları, kontrol grubuna göre anlamlı yüksek bulundu (p<0,05). Sigara içmenin yararları ölçeği puanları deney grubunda kontrol grubuna göre daha düşük, sigara içmenin zararları puanları daha yüksek bulundu (p<0,05). Deney grubu, kontrol grubuna göre değişim aşamalarında daha hızlı ilerledi (p<0,05). Sonuç: Çalışmanın sonucunda, kardiyak operasyon geçiren bireylere tele hemşirelik yoluyla TTM temelli girişimler sigara bırakmada etkili bulundu.
The Effect of Telenursing Interventions Based on the Transtheoretical Model on Smoking Cessation among Individuals Undergoing Cardiac Surgery Student Name: Elif KARAKAN Name of Supervisor: Prof. Dr. Saime EROL Department : Nursing Aim: The aim of the study was to determine the effect of telenursing interventions based on the Transtheoretical Model (TTM) on smoking cessation among individuals undergoing cardiac surgery. Method: The study was conducted in quasi-experimental design at a cardiology hospital in Istanbul (February-November 2021). The sample consisted of 120 adults–60 in the experimental group and 60 controls–who had undergone cardiac surgery. Transtheoretic model-based smoking cessation attempts were applied to the experimental group with tele-nursing for three months. Data were collected by means of a sociodemographic questionnaire, the Fagerström Nicotine Dependence Scale, and the self-efficacy and decisional balance constructs of the Transtheoretical Model. Descriptive statistics, the Mann-Whitney U test and the Kruskal-Wallis Analysis of Variance were used in the data analysis. Results: At the conclusion of the intervention, the smoking cessation rate in the experimental group was 62.75% and 26% in the control group (p<0,05). The self-efficacy scale of the experimental group displayed significantly higher levels compared to the control group in the sub-scales, with the exception of weight control (p<0,05). Compared to the controls, the scores of the experimental group on the pros of smoking scale were lower and higher in the cons scale (p<0,05). The experimental group progressed more rapidly than the control group in the stages of change (p<0,05). Conclusion: We found at the end of the study that TTM-based telenursing was effective in achieving smoking cessation among individuals undergoing heart surgery.
