Publication: Şizofren hastaların umut düzeylerinin yaşam kalitesine etkisi
Abstract
1. ÖZET Bu çalışma; bir ruh ve sinir hastalıkları eğitim ve araştırma hastanesinde yatarak tedavi gören şizofren hastaların umut düzeyinin yaşam kalitesine olan etkisini belirlemek amacıyla tanımlayıcı ve ilişki belirleyici olarak yapıldı. Araştırmanın evrenini; sağlık bakanlığına bağlı bir ruh ve sinir hastalıkları eğitim ve araştırma hastanesinin genel psikiyatri kliniklerinde yatarak tedavi gören şizofren hastalar oluşturdu. Evreni temsil edecek örneklemi hastanenin sekiz genel psikiyatri servisinde 2008 yılında yatarak tedavi görmüş, ICD-10’a göre Şizofrenik Bozukluk tanısı almış 180 hasta oluşturdu. Verilerin toplanmasında hastaların kişisel özelliklerini belirlemek için “Hasta bilgi formu”; umut düzeyini ve yaşam kalitesi belirlemek için “Herth Umut Ölçeği”, “Dünya Sağlık Örğütü Kısa Yaşam Kalitesi Ölçeği” kullanıldı. Tüm veriler bilgisayar ortamında değerlendirildi. Verilerin değerlendirilmesinde; Ortalama standart sapma, frekans dağılımı, Oneway Anova testi, Student t ve ileri analizde ise Tukey HDS testi kullanıldı. Çalışmada elde edilen bulguların değerlendirilmesinde istatiksel analizler için bilgisayar kullanıldı. Araştırmaya katılan 180 şizofreni hastasının ortalama yaşları 38.26±8.97, %53,3’ü kadın %46.7’si erkek, %65.6’sı hiç evlenmemiş, %54.4 ilköğretim mezunu, %90.6’sının sosyal güvencesinin olduğu, %42.2’sinin 1-2 kez hastaneye yattığı bulgusuna ulaşıldı. Hastaların toplam umut puanı ortalaması 53.80±14.91, yaşam kalitesi alt ölçeklerinden fiziksel alan puanı 22.94±5.33, psikolojik alan puanı 20.11±4.64, sosyal alan puanı, 8.58±2.07, çevresel alan puanı 27.79±5.66, kültürel alan puanları; 31.43±6.03 olarak tespit edildi. Hastaların yaşam kalitesinin alt ölçekleri ile umut ölçeğinin alt ölçekleri arasında istatiksel olarak anlamlı bir ilişki saptandı. Umut ölçeği ve yaşam kalitesi ile meslek, sosyal güvence, yatış sayısı ve ilaçlarını düzenli kullanma arasında anlamlı ilişkiler belirlendi. Çalışmanın sonucunda şizofreni hastaların yaşam kalitelerini etkileyecek, umutlarının azalmasına neden olacak etmenlerin ortadan kaldırılması için eğitim programlarının uygulanması, rehabilitasyon merkezlerinin aktif kullanılması önerilerinde bulunuldu. THE EFFECT OF HOPE LEVEL ON THE QUALİTY OF LİFE OF SCHIZOPHRENIC PATIENTS This study was conducted in the Mental Hospital Research and Education Center with schizophrenic in- patients to determinate the effects of hope levels on their quality of life as definer and determinant of relation. The population of this study is schizophrenic in-patients in general psychiatry clinic in the Mental Hospital Research and Education Center. 180 subjects diagnosed with schizophrenic impairment according to ICH-10 are the sample representing the population and these subjects took in-patient treatment in eight general clinics of the hospital in 2008. In data collection, Herth Hope Scale and World Health Organisation Short Quality of Life Scale were used to obtain the hope level and quality of life. Additionally, Patient Information Form were administered to determinate the personal characteristics of the patients. In this study, all data was assessed in electronic environment and Average Standart Deviation, Frequency Distribution, Oneway Anova test, Student t and in advanced analysis Tukey HDS were used in the assessment of the data. Findings were evaluated in computer program for the statistical analysis. The median age was 38.26±8.97, %53.3 were female, %46.7 were male, %65.6 were never married before, %54.4 were graduated from the primary school, %90,6 had social security, %42,2 were hospitalized for 1 or 2 times of these 180 schizophrenic patients who were participated in the study. The average total hope score is 53.80± 14.91 and we found physical area score as 22.94±5.33, social area score as 8.58±2.07, environmental area as score 27.79±5.66, cultural area score as 31.43±6.03. There was a statistically significant relationship in subjects between subscales of quality of life and subscale of hope scale. Hope scale and quality of life have significant relationship in social security, job, administration of drugs regularly, and number of in-patient treatment. In conclusion, it was suggested that the education program should be administered to schizophrenic patients in order to abolish the agents which effects their quality of life and causes decrease in their hope levels and the patients should go to rehabilitation centers actively.
