Publication: Tip 1 diyabetli çocukların yaş dönemlerine göre isülin uygulama becerileri
Abstract
1.ÖZET Tip 1 diyabet, uygun genetik zeminde ve çevresel faktörlerin etkisiyle pankreas beta hücrelerinin otoimmün yıkımına bağlı mutlak insülin yetersizliği ile ortaya çıkan hastalık tablosudur. İnsülinin mutlak yetersizliği söz konusu olduğundan dolayı diyabetli birey hayat boyu eksojen insüline bağımlıdır. Diyabet yönetiminde başarı sağlanabilmesi için çocuk ve ailenin insülin uygulama ilkelerini yeterli düzeyde bilmesi gerekir. Araştırma, tip 1 diyabetli çocukların yaş dönemlerine göre insülin uygulama becerilerini değerlendirmek amacıyla tanımlayıcı olarak planlanmıştır. Araştırma evrenini, Trakya Üniversitesi Sağlık Araştırma ve Uygulama Merkezi Çocuk Endokrinoloji Polikliniği’nde takip edilen çocuklar, örneklem grubunu ise Trakya Üniversitesi Sağlık Araştırma ve Uygulama Merkezi Çocuk Endokrinoloji Polikliniği’ne kayıtlı tip 1 diyabet tanısı konmuş 0-18 yaş arası toplam 60 çocuk oluşturmuştur. Veri toplanmasında, literatür bilgisi doğrultusunda araştırmacı tarafından geliştirilen; Diyabetli Çocuk Tanılama Formu, İnsülin Uygulama Bilgi Formu ve İnsülin Uygulama Beceri Formu; veri analizinde Ki-kare, Oneway Anova, Mann-Whitney U ve Kruskal Wallis testleri kullanıldı. Sonuçlar Ortalama SD ya da sayı-yüzde olarak ifade edildi. 7-18 yaş arası çocukların insülin uygulaması toplam beceri performansında başarı oranı % 65,96 14,89 olarak saptandı. Toplam beceri performansında yaş gruplarına göre başarı oranı 7-12 yaş arası çocuklarda % 61,0 17,17; 13-18 yaş arası çocuklarda ise % 71,0 10,11’dir. İnsülin uygulaması toplam becerilerde elde edilen bu değerler yaş gruplarına göre istatiksel olarak anlamlı farklılık göstermektedir ( p=0,044; p<0,05 ). Araştırmada, insülin uygulama toplam beceri performansı ile HbA1c, diyabet yaşı ve cinsiyet arasında istatiksel olarak anlamlı bir ilişki saptanmamıştır. yabet, çocukluk yaş dönemleri, insülin uygulama becerisi, kan gukoz ölçüm becerisi. 2.
INSULIN APPLICATION SKILLS OF TYPE 1 DIABETIC CHILDREN ACCORDING TO DIFFERENT AGE PERIODS Type 1 diabetes is an illness which is caused by absolute insulin insufficieny due to the destruction of pancreatic beta cells by the autoimmune system as a result of both genetic and environmental factors. Because of absolute insulin insuffieciency, the diabetic patients depend on exogenic insulin during their life. In order to be succesful in managing diabetes both the child and the family should know enough about the insulin application procedure. This research is planned in a descriptive way to assess the skills of type 1 diabetic children in insulin application according to different age periods. The population of this study is composed of children who are followed at Child Endocrinology Polyclinic at Trakya University Health Research and Application Center. The sample of this study is composed of 60 children between the ages 0-18 who have been diagnosed by type 1 diabetes and registered to the Child Endocrionology Polyclinic at Trakya University Health Research and Application Center. Diagnosing a Diabetic Child; Insulin Application Information and Insulin Application Skill Forms were developed and used by the researcher to collect the data for this study. For data analysis following statistical tests are used: chi-square, Oneway Anova, Mann-Whitney U and Kruskal Wallis. The results are signified as average; standard deviation and percentage. The success rate of children between ages 7-18 in total skill performance for insulin injection was detected as 65,96 14,89 %. According to different age groups, total skill performance is 61,0 17,17 %; among children betwen ages 7 and 12; and 71,0 10,11 % among children between ages 13 and 18. These obtained values show statistically significant difference depending on different age groups. In this research no statistically significant correlation has been detected between insulin injection skill performance HbA1c, diabetes age and sex.
