Publication: Doğumda kesintisiz ebe desteğinin doğum sürecine etkileri
Abstract
Amaç: Araştırma doğumda kesintisiz ebe desteğinin doğum sürecine etkilerini belirlemek amacıyla planlandı. Gereç ve Yöntem: Çalışma, Aralık 2017- Aralık 2018 tarihleri arasında İstanbul Medeniyet Üniversitesi Göztepe Eğitim Araştırma Hastanesi Kadın Hastalıkları ve Doğum Kliniği’ne başvuran kadınlarla yapılmış randomize kontrollü deneysel niteliktedir. Güç analizi sonucuna göre alınma kriterlerine uygun 70 kadın (35 deney ve 35 kontrol) çalışmaya dahil edilmiştir. Verilerin toplanmasında literatür doğrultusunda araştırmacı tarafından hazırlanan Tanıtıcı Bilgi Formu ve VAS kullanılmıştır. Deney grubundaki gebelere doğum eylemi boyunca kesintisiz ebe desteği verilmiş, kontrol grubundaki gebelere çalışmanın yapıldığı klinikteki rutin bakım uygulanmıştır. Kesintisiz doğum desteği bilgi desteği, fiziksel rahatlatma, duygusal destek ve savunuculuk bileşenlerini kapsamıştır.Bulgular: Çalışmada aktif fazdan itibaren toplam doğum süresi deney grubunda kontrol grubuna göre 2 saat 21 dakika daha kısa sürmüştür (p<0,05). Doğumun geçiş fazında deney grubunda kontrol grubuna göre; ağrı ile başetme düzeyi (8,69±0,83) artarken; anksiyete puanı (2.00±0,84) düşmüştür (p<0,05). Kontrol grubunda deney grubuna göre indüksiyon girişimi (%97,1) ve perineal travma oranı (%42,9) artmıştır (p<0,05). Kontrol grubundaki kadınlar emzirmeye deney grubuna göre 29,24±17,06 dakika daha geç başlamışlardır (p<0,05). Kadınların yarısı (%54,1) savunuculuk desteğinin çok önemli olduğunu belirtmiştir.Sonuçlar: Her kadının doğumunda kesintisiz ebe desteği alması sağlanmalıdır.
Objective: This study was planned to determine the effects of continuous midwife support during labor. Materials and Methods: The study was randomized and controlled with women who applied to Gynecology and Obstetrics Clinic of Göztepe Training and Research Hospital between December 2017 and December 2018. According to the results of power analysis, 70 women (35 experiments and 35 controls) were included in the study. The data were collected the using the VAS and pregnant diagnosis form developed by the researcher in accordance with the literature review. While continuous midwife support was given to the pregnant women during the delivery in the experimental group, the control group was given routine care in the clinic where the study was performed. Continuous birth support contained the information support, physical relief, emotional support and advocacy components.Findings: In the study, the total labor time from the active phase was 2:21 minutes shorter in the experimental group than the control group (p <0,05). According to the control group in the experimental group in the transition phase of labor; the level of coping with pain (8.69 ± 0.83) increased; anxiety score (2.00 ± 0.84) decreased (p <0.05). In the control group, induction (97.1%) and perineal trauma (42.9%) increased compared to the experimental group (p <0.05). Women in the control group started breastfeeding 29,24 ± 17,06 minutes later than the experimental group (p <0,05). Half of the women (54.1%) stated that advocacy support was very important.Conclusions: It should be ensured that every woman receives uninterrupted midwife support at birth.
