Publication: Eğitim programlarının diz osteoartriti olan hastalarda egzersiz uyumuna etkisi
Abstract
Giriş: Yaşlılarda en sık yeti yitimine neden olan osteoartritin tedavisinde egzersiz programları çok önemli yer tutar. Metod: Bu çalışma randomize kontrollü eğitim müdahale çalışmasıdır. Marmara Üniversitesi Hastanesi Aile Hekimliği Kliniği’ne başvuran 64 osteoartrit hastası çalışmaya dahil edilmiş, hastaların yarısı müdahale grubuna, diğer yarısı kontrol grubuna (standart bakım) randomize edilmiştir. Çalışmanın süresi 12 haftadır ve müdahale grubundaki tüm katılımcılara broşür ve demostratif olarak egzersiz eğitimleri derecelendirilerek verilmiştir. Kontrol grubuna yalnızca egzersizleri içeren broşür verilmiştir. Hastaların uyumları hasta günlükleri, telefon takipleri ile değerlendirilmiş ve her vizite tüm katılımcıların WOMAC skorları saptanmıştır. Bulgular: Katılımcıların ortalama yaşı 54,6 ± 9,85 ve %98,4’i kadındı. Müdahale grubunun uyumu takip boyunca yüksek düzeydeydi (ortanca %100) ve müdahale grubundaki katılımcıların ağrı ve WOMAC skorlarında belirgin iyileşme saptandı. Kontrol grubunun egzersiz programına uyumu iyi olmakla beraber, uzun dönemde uyum düzeyinin orta olduğu gösterildi. Kontrol grubundaki ağrı ve WOMAC skorundaki iyileşme müdahale grubundan daha düşüktü. Tartışma ve Sonuç: Osteoartriti olan hastalarda egzersiz programlarına uyum düzeyi genellikle düşüktür. Introduction: Exercise programs play an important role in the treatment of knee osteoarthritis which is the most common cause of disability in the elderly. Method: This is a randomised controlled, educational intervention study. Sixty-four patients who admitted to the Family Medicine outpatient clinic of Marmara University Hospital were enrolled in the study. Half of the paticipants were allocated randomly to the intervention group and the other half to the standart care group. Particiapants of the invention group were given leaflets and gradual exercise education were by demonstration. Control group received leaflet about exercises. We assessed participitans’ adherence with patients’ diaries and telephone contact. Besides, we determined WOMAC score of the particiapnts at each followup visit. Results: Mean age of the participants was 54,6 ± 9,85 and 98,4% of them were women. Adherence to exercise in inventention group was high (median 100%) and pain and WOMAC score improved significantly. Exercise adherence of the control group was good but long-term adherence was only moderate. Improvement of pain and WOMAC score was than the invention group. Conclusion: Patients with osteoarthritis do not generally adhere well to the exercise programs. Gradual exercise education with demonstration might improve adherence.
