Abstract
Tezin Başlığı: Kemoterapiye Bağlı Alopesi Algısı ve Yaşam Kalitesi 1.ÖZET Kemoterapinin yan etkilerinden biri olan alopesi, hastaların vücut imajını bozmakta, sosyal ve emosyonel durumlarını etkilemekte, utanma duygusunun artışına bağlı özgüven azalmasına neden olmakta ve sonuçta hastaların yaşamlarını olumsuz şekilde etkilemektedir. Bu çalışma, kemoterapiye bağlı olarak ortaya çıkan alopesinin hastaların yaşam kalitesi üzerindeki etkisini ölçebilecek bir ölçek geliştirmek, böylece yaşam kalitesini etkileyen faktörleri saptamak, hemşire ve hasta yakınlarının alopesiye ilişkin algılarını belirlemek amacıyla yapılmıştır. Veriler bilgi formları, Kemoterapiye Bağlı Alopeside Yaşam Kalitesi Ölçeği (KBAYKÖ), Alopesi Yönetim Ölçeği, Beck Depresyon Ölçeği, Beden İmajı Ölçeği ve SF-36 Yaşam Kalitesi Ölçeği kullanılarak toplanmıştır. KBAYKÖ’nin geçerlilik ve güvenilirlik çalışması kapsam geçerliliği, yapı geçerliliği, ölçüt geçerliliği; Cronbach alpha (0.88), Gutman (0.83) ve Spearman Brown (0.85) güvenilirlik katsayısı, madde analizi (p<0.001) ve test tekrar test tekniği (p<0.001) yöntemleri kullanılarak yapılmıştır. Alopesi Yönetim Ölçeğinin geçerlilik ve güvenilirlik çalışmasında ise dil geçerliliği, içerik geçerliliği, Cronbach alpha (0.75), Gutman (0.68) ve Spearman Brown (0.68) güvenilirlik katsayısı, madde analizi (p<0.001) ve test tekrar test tekniği (p<0.001) yöntemleri kullanılmıştır. Cinsiyet, çalışma durumu, alopesi şiddeti, ilave hastalık ve genel sağlık algısı özelliklerine göre yaşam kalitesi puanlarının farklılık gösterdiği belirlenmiştir. Hasta yakınlarının alopesiye ilişkin algılarının, cinsiyet ve bilgi alma durumuna göre farklılaştığı tespit edilmiş ve hasta yakınları tarafından saç dökülmesinden olumsuz etkilendiği söylenen hastaların yaşam kalitesi puanlarının düşük olduğu saptanmıştır. Hemşirelerin Alopesi Yönetim Ölçeği’nden almış oldukları puanlar ile cinsiyet, yaş, hizmet içi eğitim alma durumu ve alopesi deneyimi (aile-çevre) özellikleri arasında ilişki olduğu tespit edilmiştir. Hemşirelerin yönetim boyutundaki üç madde dışında kalanlara verdikleri cevaplarla fikir birliği içinde oldukları saptanmıştır. Chemotherapy Induced Alopecia Perception and Quality of Life Alopecia is one of the side effects of chemotherapy and it disturbs the body images of patients, affects their social and emotional status, causes the reduction in self-confidence related to the increased sense of shame, and totally, affects the patients' life negatively. This study aims were to develop a scale to measure the effects of chemotherapy induced alopecia to the patients' quality of life, to determine the factors which effects quality of life, and to identify the nurses' and patient's relatives' perceptions to the alopecia. Data were collected with informations forms, Chemotherapy Induced Alopecia Quality of Life Scale (KBAYKO), Alopecia Management Scale, Beck Depression Inventory, Body Image Scale, and SF-36 Quality of Life Scale. Validity and reliability studies of KBAYKO were done by using content, construct and criterion validity, Cronbach’s alpha (0.88), Guttman (0.83) and Spearman Brown (0.85) reliability coefficients, item analysis (p<0.001) and test-retest methods (p<0.001). In the validity and reliability study of Alopecia Management Scale, the methods: language equivalency/ adaptation, content validation, Cronbach alpha (0.75), Gutman (0.68), and Spearman Brown (0.68) reliability coefficients, item analysis (p<0.001), and test-retest technique (p<0.001) were used. It was determined that the quality of life scores show discrepancies according to the gender, employment status, the the severity of alopecia, additional disease, and general health perception. It was also found that, the alopecia related perception of the patient's relatives differentiates with respect to gender and being (obtain) informed. The quality of life scores of the the patients who were decleared to be effected negatively from hair loss by their relatives, were determined as low. A relation was found between the score of nurses taken from Alopecia Management Scale and gender, age, in-service training, and alopecia experience (family-environment) properties.
