Üveitik Glokomda Koroid Kalınlıkları İncelemesi: Arttırılmış Derinlik Görüntüleme Optik Koherens Tomografi Çalışması

Abstract

Amaç: Bu çalışmanın amacı üveitik glokom tanılı hastalar ile sağlıklı bireyleri koroid kalınlıkları açısından arttırılmış derinlik görüntüleme optik koherens tomografi (EDI-OKT) kullanarak karşılaştırmaktır.Gereç ve Yöntemler: Bu çalışma kesitsel, non-randomize klinik olgu serisi olarak planlandı. Uveitik glokom grubuna üveit ve glokom haricinde herhangi bir oküler hastalığı bulunmayan hastalar; kontrol grubuna ise sağlıklı bireyler dahil edildi. Son 3 ay içerisinde oküler cerrahi geçirmiş olan, son 3 ay içerisinde üveit atağı geçiren veya herhangi bir sistemik hastalığı bulunan bireyler çalışma dışı bırakıldı. Tüm katılımcılar tam bir göz muayenesinden geçirildi. Koroid kalınlığı ölçümü retina pigment epitelinin hiperreflektif dış kenarı ile koroid-sklera sınırı arasında, subfoveal alandan ve subfoveal alanın 500?m temporal ile 500?m nazalinden, EDI-OKT ile yapıldı.Bulgular: Çalışmaya 14 üveitik glokom hastasının 14 gözü (7 kadın, 7 erkek, ortalama yaş: 44.7±14.2) ve 20 sağlıklı kontrolün 20 gözü dahil edildi (10 kadın, 10 erkek, ortalama yaş: 44.9±16.2). Yaş, cinsiyet, lateralite, GİB, aksiyel uzunluk ve santral kornea kalınlığı açısından gruplar arasında istatistiksel olarak anlamlı fark saptanmadı.(P > 0.05) Ortalama Koroid kalınlığı UG grubunda subfoveal, temporal ve nazalde sırasıyla 325.2±94.4, 334.5±91.0, 328.6±94.4 ?m; kontrol grubunda ise 317.1±67.8, 329.5±68.3, 309.9±73.2 ?m olarak saptandı. Gruplar arasında ölçülen üç farklı noktada da koroid kalınlığı açısından anlamlı farklılık saptanmadı. Sonuç: Subfoveal ve perifoveal koroid kalınlığının üveitik glokom hastalarında sağlıklı bireylere kıyasla incelme göstermediği izlenmiştir. Bu bulgular üveitik glokom patogenezinde mekanik hipotezin, iskemik hipoteze göre daha ön planda olduğunu düşündürmektedir.

Purpose: We aimed to evaluate choroidal thickness in uveitic glaucoma (UG) patients using enhanced depth imaging optical coherence tomography (EDI-OCT) and to compare with healthy control subjects.Materials and Methods: This study was planned as a cross-sectional, non-randomized case series. The patients in UG group didn't have any ocular disease except for uveitis and glaucoma, while control group included healthy subjects. The subjects who had an ocular surgery or uveitis attack within the last 3 months or had any other systemic disease were excluded from the study. All participants underwent a complete ophthalmologic examination. Choroidal thickness was measured between outer layer of retinal pigment epithelium and inner layer of sclera, at subfoveal region and 500 ?m temporal and 500 ?m nasal to the fovea.Results: Fourteen eyes of 14 uveitic glaucoma patients (7 female, 7 male, mean age: 44.7±14.2) and 20 eyes of 20 healthy subjects (10 female, 10 male, mean age: 44.9±16.2) were included to the study. Groups were homogeneous in terms of age, laterality, IOP, axial length and central corneal thickness. (P > 0.05) Mean subfoveal choroidal thickness was 334.5±91?m in UG group and 339.7±42.3?m in control group. (P =0.82) Mean choroidal thickness in subfoveal, temporal and nasal regions was 325.2±94.4, 334.5±91.0, 328.6±94. 4?m in UG group and 317.1±67.8, 329.5±68.3, 309.9±73.2 ?m in control group, respectively. There was no statistically significant difference in any of the measured regions between the groups.Conclusion: We did not observe any choroidal thinning in subfoveal or perifoveal regions in UG compared to healthy subjects. These results suggest that mechanical hypothesis might be more prominent in the pathogenesis of UG rather than ischemic hypothesis.
