Abstract
WERNİCKE KORSAKOFF SENDROMUNDA ODYOLOJİK BULGULAR Senem ERDUR UZUT 1.ÖZET Bu tez çalışmasında; Marmara Üniversitesi Tıp Fakültesinde görülmüş Wernicke Korsakoff sendromu tanısı almış kişilerin odyolojik sonuçlarının retrospektif olarak incelenerek, normallerle karşılaştırılması ve kendi içlerinde farklılıklarının değerlendirilmesi amaçlandı. Wernicke Korsakoff tanısı almış hastaların dosyaları incelendi: olgu öyküleri ayrıntılı olarak alınan, otolojik muayeneleri yapılmış, davranım odyometrisi, Transient Evoked Otoakustik Emisyon (TEOAE), akustik immitansmetri ve işitsel beyinsapı davranımı (ABR) ile değerlendirilmiş hastalardan çalışma grubu oluşturuldu İstatistiksel analizler için NCSS (Number Cruncher Statistical System) 2007&PASS 2008 Statistical Software (Utah, USA) kullanıldı. Çalışma verileri değerlendirilirken tanımlayıcı istatistiksel metotların (Ortalama, Standart sapma) yanı sıra, normal dağılım gösteren parametrelerin iki grup arası karşılaştırmalarında Student t test kullanıldı. Parametreler arasındaki ilişkilerin incelenmesinde Pearson korelasyon analizi kullanıldı. B1 vitamini alan ve almayan WKS’liler arasında işitme eşikleri açısından istatiksel olarak anlamlı farklılık saptanmamakla birlikte,B1 almayan WKS’lilerde alçak frekanslarda düşüş gözlenmiştir. Ölüm orucu süresi açısından da işitme eşiklerinde istatistiksel olarak anlamlı bir farklılık gözlenmemiştir. 70 dB nHL de yapılan ABR incelemesinde Wernicke Korsakoff’lu grubun I, III ve V. dalga absolut latansları normallere göre istatistiksel olarak anlamlı ölçüde uzun bulunmuştur. 70 dB nHL ve 90 dB nHL de yapılan ABR incelemesinde B1 vitamini alımı ve ölüm orucu süresiyle ilgili istatistiksel olarak anlamlı bir farklılık bulunmamıştır. In this thesis project, the audiological findings of 15 patients, diagnosed Wernicke-Korsakoff syndrome at Marmara University Medical Schooll were evaluated retrospectively and assessed whether or not there has been different pattern than normal audiological findings or differences to each other. The patients’ ,who has diagnosed wernicke-korsakoff syndrome, records were examined : and the study group had been constituted by the patients who investigated by otoscopic examination, air-bone conduction thresholds, speech audiometry, Transient Evoked Otoacoustic Emission (TEOAE) and acoustic immitansmetry and Auditory Brainstem Response (ABR) test. Statistical analyisis were made by using Number Cruncher Statistical System (NCSS) 2007&PASS 2008 Statistical Software (Utah, USA). Student t test was used to compare the results of normal hearing people and Wernicke-Korsakoff syndrme (WKS) group. The relationship between the parameters were analysed by Pearson. There was no statistically significant differences between the hearing thresholds of WKS and normal hearing group with regard to B1 vitamin intake. Morever, ıt vas found no statistically significant differences on the basis of the duration of fast death between two groups. ABR test results showed that I, III and V. wave absolute latencies were significantly prolonged than those of normal hearing at 70 dB normalized Hearing Level (nHL). At the intensity level of 70 and 90 dB nHL, no statistically significant differences were observed with regard to B1 vitamin intake and the duration of fast death.
